If the combination of symptoms and signs matches a pattern they’ve seen before, they recognize it quickly and land on a likely diagnosis within seconds. This fast, intuitive process is called type 1 thinking. It’s automatic, experience-driven, and efficient. A seasoned emergency physician who sees a patient clutching their chest with specific vital sign changes doesn’t need to run through a mental checklist; the pattern fires instantly.

When the pattern isn’t familiar, the clinician shifts into type 2 thinking: slower, deliberate, analytical reasoning. This is where they consciously weigh possibilities, order tests, and work through the problem step by step. It’s grounded in evidence-based medicine, which follows a five-step cycle: defining a clinically relevant question, searching for the best available research, critically appraising that research, applying the findings to the specific patient, and then evaluating how well the approach worked. This cycle keeps decisions anchored to data rather than habit or tradition alone.

In practice, this means a clinician diagnosing a condition will draw on published research about which treatments have the best outcomes, what side effects to expect, and which patient populations respond differently. But evidence alone isn’t enough. The same research might point toward a treatment that’s highly effective on average yet completely impractical for a particular patient because of their other health conditions, their daily routine, or their personal values. Modern clinical decision making increasingly treats the patient as a partner rather than a passive recipient. Shared decision making is a collaborative approach in which the clinician and patient work together to evaluate the available options, weigh the likely benefits and harms of each, and select a course of action that fits the patient’s preferences, goals, and real-life constraints.

This can look different depending on the situation. Sometimes it involves comparing treatment options side by side: one medication might be more effective but carries heavier side effects, while another is gentler but slower-acting. The clinician brings the medical knowledge; the patient brings their priorities. A construction worker with a knee injury may value a faster return to physical activity, while a retired person with the same injury may prioritize avoiding surgical risk. Both are valid, and the “right” decision depends on the person making it.

Shared decision making also means addressing uncertainty honestly. Clinicians don’t always have clear-cut answers, and acknowledging that openly, rather than defaulting to paternalistic confidence, typically leads to care plans that patients actually follow through on because the plan reflects what matters to them.

Cognitive Biases That Derail Judgment

Even skilled clinicians are vulnerable to predictable thinking errors. These cognitive biases can distort the reasoning process in ways that lead to missed diagnoses or unnecessary treatments.

Cognitive biases aren’t the only threat to good clinical decisions. The environment in which clinicians work plays a major role. A scoping review of context factors in clinical decision making found that the single most frequently cited influence, appearing in 65% of the studies reviewed, was time pressure. Rushed visits, strict scheduling, and limited time with each patient all compress the space available for careful reasoning.

Beyond time, about 10% of the factors identified related to the clinician’s personal state: workload, mental and physical stress, sleepiness, and confidence levels. Fatigue and high workload can push clinicians toward faster, more intuitive thinking even in situations that call for slow, deliberate analysis. Inadequate experience, gaps in knowledge, and incomplete patient information compound the problem further, contributing to misdiagnoses and treatment errors.

How Technology Is Changing the Process

Clinical decision support systems have been used in some form since the 1960s, when the earliest versions helped pharmacists catch drug allergies and dangerous drug interactions. Today’s systems are far more sophisticated, integrating patient data with medical knowledge databases to flag potential diagnoses, suggest tests, or warn about medication conflicts.

The evidence suggests these tools make a measurable difference. One study using an interrupted time series analysis found that after implementing a clinical decision support system, the consistency between a patient’s initial admission diagnosis and their final discharge diagnosis increased by about 6.7 percentage points. Before the system was in place, the match rate was roughly 70%; afterward, it rose to nearly 73%. That gap represents thousands of patients whose conditions were identified more accurately from the start.

These systems don’t replace clinical judgment. They function more like a second set of eyes, catching patterns a busy clinician might overlook and surfacing relevant evidence at the point of care.
