What Is CMP Serum or Plasma? A Blood Test Explained

A CMP (comprehensive metabolic panel) serum or plasma is a routine blood test that measures 14 substances in your blood to check how well your kidneys, liver, and metabolism are working. The “serum or plasma” part refers to the liquid portion of your blood used for the test. Most people encounter this term on a lab order or test result and simply want to know what it means and what it measures.

What a CMP Actually Measures

The comprehensive metabolic panel checks 14 different markers in a single blood draw. These markers fall into a few natural groups based on what part of your body they assess.

Blood sugar: Glucose, your body’s main energy source. A normal fasting level is 74 to 106 mg/dL.

Electrolytes: Sodium, potassium, chloride, and bicarbonate. These are electrically charged minerals that regulate fluid balance and the acid-base balance in your body. When electrolytes are off, you can experience muscle cramps, fatigue, irregular heartbeat, or confusion.

Kidney markers: Blood urea nitrogen (BUN) and creatinine. Both are waste products that healthy kidneys filter out of your blood and remove through urine. Normal BUN runs 10 to 20 mg/dL. Normal creatinine is roughly 0.6 to 1.2 mg/dL for men and 0.5 to 1.1 mg/dL for women. When these numbers climb, it suggests your kidneys aren’t filtering as efficiently.

Liver markers: Three enzymes (ALT, AST, and ALP), plus bilirubin, albumin, and total protein. ALT and AST are enzymes produced mainly in the liver; elevated levels can signal liver inflammation or damage. ALP is another liver enzyme that also rises with bone disorders. Bilirubin is a yellowish waste product from the breakdown of old red blood cells, and your liver is responsible for clearing it. Albumin and total protein reflect how well your liver produces the proteins your body needs.

Calcium: Most calcium lives in your bones and teeth, but the small amount circulating in your blood is critical for nerve signaling, muscle contraction, and heart rhythm.

What “Serum or Plasma” Means

When your blood is drawn, it gets separated into cells and liquid. That liquid portion is what the lab actually tests. “Serum or plasma” tells you which form of that liquid was used, and the distinction comes down to clotting.

Plasma is the liquid that remains when clotting is prevented. The lab adds an anticoagulant to the collection tube so the blood stays fluid, then spins it to separate the cells. Plasma contains everything dissolved in your blood: proteins, hormones, nutrients, waste products, and clotting proteins like fibrinogen.

Serum is the liquid that remains after blood is allowed to clot naturally. During clotting, fibrinogen gets converted into fibrin (the mesh that forms the clot), so serum is essentially plasma minus the clotting proteins. The lab lets the blood sit, a clot forms, and the remaining clear liquid is serum.

For most CMP components, the results are virtually identical whether the lab uses serum or plasma. One small exception is glucose: plasma glucose readings tend to run slightly higher than whole blood measurements, roughly 7% higher. This is a known technical difference that labs account for in their reference ranges, so it doesn’t affect how your doctor interprets the result.

How the Blood Is Collected

A CMP requires a standard blood draw, usually from a vein in your arm. The tube the lab uses depends on whether they’re collecting serum or plasma. For serum, labs typically use a gold-topped tube called a serum separator tube (SST). It contains a clot activator that speeds up natural clotting, plus a gel barrier that separates the serum from the clot when the tube is spun in a centrifuge. For plasma, labs use tubes containing an anticoagulant that prevents clotting altogether.

Most labs performing a CMP use serum separator tubes, making serum the more common specimen type. But both are considered acceptable, which is why your lab slip may say “serum or plasma” rather than specifying one.

Why Doctors Order a CMP

A CMP is one of the most commonly ordered blood panels because it provides a broad snapshot of your metabolic health in a single test. Doctors use it for several purposes: as part of a routine annual physical, to monitor chronic conditions like diabetes or kidney disease, to check on the effects of certain medications (particularly those processed by the liver or kidneys), or to investigate symptoms like fatigue, nausea, or swelling that could have metabolic causes.

You’ll typically be asked to fast for 10 to 12 hours before the test because eating can temporarily raise your glucose level and skew that particular result. The other 13 markers are less affected by food, but fasting keeps everything standardized.

CMP vs. Basic Metabolic Panel

You may also see a BMP, or basic metabolic panel, on lab orders. The BMP measures 8 of the same 14 substances: glucose, calcium, the four electrolytes (sodium, potassium, chloride, bicarbonate), BUN, and creatinine. It leaves out all the liver markers and proteins. A doctor who specifically wants to check liver function, or who wants a more complete picture, will order the full CMP instead.

Reading Your Results

Your lab report will list each of the 14 values alongside a reference range. These ranges can vary slightly between labs because different equipment and methods produce slightly different baselines. A result flagged as “high” or “low” doesn’t automatically mean something is wrong. A single abnormal value often needs context: your hydration level, recent meals, medications, and even the time of day can shift certain markers temporarily.

Patterns matter more than individual numbers. For example, BUN and creatinine rising together is a stronger signal of kidney trouble than either one alone. Similarly, ALT and AST both being elevated points more clearly toward liver issues than a bump in just one. Your doctor looks at how all 14 values relate to each other, your symptoms, and your medical history before drawing any conclusions.
