A temperature of 100.4°F (38°C) or higher, measured rectally, is considered a fever in babies. That single number applies across infancy, but how urgently you need to respond depends heavily on your baby’s age. A 100.4°F reading in a 2-week-old is a medical emergency, while the same temperature in an 8-month-old is usually manageable at home.
Why the Thermometer Type Matters
Not all thermometers give equally reliable readings in babies. A rectal thermometer is the gold standard for children 3 and under because it measures internal body temperature directly. Forehead (temporal) thermometers can be used for babies 3 months and older but are less precise. Ear thermometers work best from about 6 months on, once the ear canal is large enough for an accurate reading. Armpit readings are the least accurate of all, and if you use one, you’ll need to add about half a degree to a full degree Fahrenheit to approximate the true temperature.
If your baby feels warm and you get a borderline reading from a forehead or armpit thermometer, follow up with a rectal check before making decisions. A rectal thermometer with a flexible tip and a small amount of petroleum jelly is all you need. The reading takes about 10 to 15 seconds with most digital models.
Age Makes All the Difference
The 100.4°F threshold stays the same regardless of age, but what you do next changes dramatically in the first few months of life.
Birth through 7 days: Newborns in the first week are in a category of their own. Their immune systems are so immature, and the risk of serious bacterial infection (particularly from bacteria picked up during delivery) is high enough, that any fever requires immediate emergency evaluation.
8 days through 2 months: A rectal temperature of 100.4°F or higher still warrants urgent medical attention the same day. The American Academy of Pediatrics has specific clinical guidelines for this age group because babies this young can look perfectly fine while harboring a dangerous infection. Don’t wait to see if the fever resolves on its own.
3 to 6 months: A fever in this range is less automatically alarming, but temperatures above 102°F or a fever lasting more than a day still calls for a conversation with your pediatrician.
Over 6 months: By this age, most fevers are caused by common viral infections and can be monitored at home. The temperature itself matters less than how your baby is acting.
What Fever Actually Does in the Body
Fever isn’t the illness. It’s your baby’s immune system fighting back. When the body detects a pathogen, it raises its internal thermostat on purpose. That higher temperature stimulates the production of white blood cells and antibodies while making the environment less hospitable for bacteria and viruses to reproduce. Fever also helps redirect iron to the liver, which starves certain bacteria of a nutrient they need to multiply.
There’s even evidence that fever in the first year of life may have long-term immune benefits. A study in the Journal of Allergy and Clinical Immunology found that babies who experienced fevers during their first year were less likely to develop allergies later in childhood. So while a fever is uncomfortable, it’s a sign that the immune system is doing exactly what it should.
Common Causes of Infant Fever
Viral infections are by far the most frequent cause of fever in babies. Colds, flu, and COVID-19 top the list, and most of these resolve on their own within one to two weeks. Ear infections, throat infections, and roseola (a virus that causes a high fever followed by a rash) are other common culprits.
Less commonly, bacterial infections like urinary tract infections, pneumonia, or meningitis can cause fever. These are more serious and require antibiotics. One pattern worth watching for: a viral illness that seems to improve and then suddenly worsens, bringing a higher fever or new symptoms like ear pain. That can signal a secondary bacterial infection that developed on top of the original virus.
How Long Fever Typically Lasts
Most viral fevers peak within the first two to three days and clear up within a week, sometimes stretching to two weeks for viruses like the flu. A fever that persists beyond five days without improving is worth a call to your pediatrician, even if your baby seems otherwise okay. The same goes for symptoms that initially get better and then come back worse. That rebound pattern can indicate the infection has shifted from viral to bacterial.
Managing Fever at Home
For babies 2 months and older with a low-grade fever who are eating, sleeping, and behaving relatively normally, home care is often all that’s needed. Acetaminophen (Tylenol) can be given to babies 8 weeks and older, dosed by weight rather than age, every 4 to 6 hours as needed, with a maximum of 5 doses in 24 hours. Ibuprofen (Motrin, Advil) is an option starting at 6 months, given every 6 to 8 hours, up to 4 doses per day. Always use the dosing syringe that comes with the product and go by your baby’s weight.
Keep your baby hydrated with breast milk, formula, or an electrolyte solution if they’re old enough. Dress them in light clothing. A lukewarm bath can help them feel more comfortable, but avoid cold water or rubbing alcohol, both of which can cause shivering and actually raise the body’s core temperature.
Signs That Need Immediate Attention
The number on the thermometer is only part of the picture. How your baby looks and acts tells you more than the temperature alone. A baby with a 101°F fever who is playful, eating, and making eye contact is generally in better shape than a baby with a 100.5°F fever who is limp and unresponsive.
The warning signs that signal something more serious include lethargy or listlessness (your baby is unusually difficult to wake or seems “floppy”), inconsolable crying that sounds different from their normal cry, refusal to eat, and a change in skin color such as a pale, mottled, or bluish tint. A bulging soft spot on the head, a stiff neck, or a rash that doesn’t fade when you press on it are also red flags. Any of these, at any temperature, warrant immediate medical evaluation.
For babies under 3 months, a single rectal reading of 100.4°F or higher is reason enough to seek care right away, even if they look perfectly fine. Young infants can deteriorate quickly, and the stakes of a missed bacterial infection are too high to take a wait-and-see approach.
