A normal ALT level in dogs falls between 17 and 95 U/L, based on Cornell University’s reference intervals. Anything above 95 U/L is considered elevated, but the degree of elevation matters far more than the simple fact that the number is high. A mildly elevated reading might mean nothing serious, while a level several times above normal signals active liver cell damage that needs investigation.
How Severity Breaks Down by Number
ALT (alanine aminotransferase) is an enzyme concentrated inside liver cells. When those cells are damaged or inflamed, ALT leaks into the bloodstream, and the blood level rises. The higher the number, the more liver cells are affected.
An ALT level two to three times above the normal range (roughly 190 to 285 U/L) is considered a clinically significant finding. Below that threshold, your vet may recommend monitoring over time rather than launching an aggressive workup, especially if your dog seems healthy otherwise.
The most dramatic spikes happen during acute liver injury. ALT can rise sharply within 24 to 48 hours of damage and peak as high as 100 times the normal range (potentially above 9,000 U/L) within the first five days. Numbers that extreme typically point to severe inflammation, infection, or poisoning. But even a modest, persistent elevation shouldn’t be ignored. A chronically elevated ALT, even in the low hundreds, can be the first detectable sign of chronic hepatitis, which can quietly progress toward liver failure if left untreated.
Common Causes of Elevated ALT
Liver disease is the most obvious explanation, but it’s far from the only one. ALT can rise from problems that start entirely outside the liver, including pancreatitis, intestinal disease, diabetes, and Cushing’s disease (where the adrenal glands overproduce cortisol). This is why a single high ALT reading doesn’t automatically mean your dog has a liver problem.
Medications are another frequent culprit. Steroids, including those in ear and eye drops, can push ALT upward. Phenobarbital, a common seizure medication, reliably raises ALT and another liver enzyme called ALP over weeks of use. Research suggests these increases often reflect the liver ramping up enzyme production rather than actual liver damage, but your vet will want to track the trend.
Certain breeds carry a genetic predisposition to chronic hepatitis. Labrador Retrievers, Bedlington Terriers, West Highland White Terriers, and Dobermans are all at higher risk. In some of these dogs, copper accumulates in the liver over time, causing progressive damage that will continue unless treated. Breed history is something your vet factors in when interpreting the numbers.
Older dogs sometimes develop a benign condition called vacuolar hepatopathy, where water, fat, or carbohydrates accumulate inside liver cells. This can bump ALT up without posing a serious threat, though it still needs to be distinguished from more harmful conditions.
Toxins That Cause Rapid ALT Spikes
Some household substances can send ALT skyrocketing within hours. Xylitol, the sugar substitute found in sugar-free gum, candy, and some peanut butters, is one of the most dangerous. Dogs that ingest enough xylitol can develop severe liver failure, with liver enzyme elevations detectable as early as 4 to 12 hours after ingestion. Visible signs of liver damage, like vomiting, lethargy, and yellowing of the gums, may not appear for 24 to 48 hours, which makes it deceptively easy to miss the early window. In one study, 62.5% of dogs showing clinical signs of xylitol-related liver injury died or were euthanized despite aggressive treatment.
Toxic mushrooms, blue-green algae, and certain plants can cause similarly rapid liver damage. If your dog’s ALT spiked suddenly and you suspect they got into something, that context changes the urgency dramatically.
Symptoms to Watch For
Here’s what makes elevated ALT tricky: many dogs with early or moderate liver disease show no symptoms at all. The liver has enormous reserve capacity, and damage can accumulate quietly before anything is visible from the outside. This is why ALT elevations are often caught on routine bloodwork before anyone suspects a problem.
When symptoms do appear, they can include:
- Loss of appetite or reduced interest in food
- Vomiting or diarrhea
- Lethargy or unusual tiredness
- Jaundice, a yellow tint to the gums, whites of the eyes, or skin
- Abdominal swelling from fluid accumulation
- Neurologic changes like disorientation or circling, which can occur when the liver can no longer filter toxins from the blood
A dog with elevated ALT and no symptoms is in a very different situation than a dog with elevated ALT, jaundice, and vomiting. Both need follow-up, but the timeline and intensity of that follow-up differ.
What Happens After a High ALT Result
A single ALT reading is a starting point, not a diagnosis. Your vet will look at the full picture: medical history, breed, medications, physical exam findings, and the rest of the blood panel. Other liver-related values on the same panel, like ALP, bilirubin, and albumin, help clarify whether the liver is just irritated or actually losing function.
If the elevation is mild and your dog is otherwise healthy, the first step is often to recheck the value in a few weeks. A number that was temporarily high and returns to normal may not need further investigation.
For persistent or significant elevations, the next steps typically include abdominal ultrasound to visualize the liver’s size, texture, and blood flow. Ultrasound can reveal changes in the liver surface, tissue density, gallbladder wall thickness, and bile duct visibility, all of which help narrow the possibilities. If imaging suggests a specific problem, a liver biopsy may be recommended for a definitive diagnosis, though vets usually exhaust non-invasive options first.
Dietary and Nutritional Support
If your dog has confirmed liver disease, diet plays a real role in management. One common misconception is that protein should be heavily restricted. According to UC Davis veterinary nutritionists, protein restriction is not recommended for most dogs with liver disease. The liver actually needs adequate protein to produce albumin and other essential compounds, and unnecessary restriction can lead to malnutrition.
The exception is a condition called hepatic encephalopathy, where the liver can no longer filter toxins effectively and neurologic symptoms develop. In those cases, the type of protein matters more than the amount. Meat-based proteins tend to trigger more severe symptoms compared to dairy or vegetable sources. Cottage cheese and egg-based diets are commonly used, and vets will often slowly increase protein intake to find the highest amount the dog can tolerate without symptoms.
For breeds prone to copper storage disease, dietary copper is restricted and zinc may be increased to block copper absorption in the gut.
Antioxidant supplements can help protect remaining liver cells from further damage. Vitamin E, vitamin C, SAMe (S-adenosylmethionine), and milk thistle extract are the most commonly recommended. These are often given together in veterinary liver support products, typically on an empty stomach for best absorption. Your vet can guide you on whether supplementation makes sense for your dog’s specific situation and at what point in the process to start.
