What Is Considered a High Temperature for an Adult?

For adults, a temperature of 100.4°F (38°C) or higher, measured orally, is considered a fever. This is the threshold most healthcare providers use. But that single number doesn’t tell the whole story, because your baseline temperature, how you measure it, your age, and even the time of day all affect what counts as “high” for you specifically.

The Standard Fever Threshold

The widely accepted cutoff for a fever in adults is 100.4°F (38°C) taken by mouth. Rectal and ear thermometers typically read about 1°F (0.6°C) higher than oral thermometers, so the same fever measured rectally would show around 101.4°F. Armpit readings run 0.5 to 1°F lower than oral, meaning an armpit temperature of 99.4°F could still indicate a true fever.

These differences matter more than most people realize. If you’re using a forehead or armpit thermometer and get a reading of 99.5°F, that may actually represent a fever, not a borderline number. Oral thermometers placed under the tongue with the mouth closed for a full minute remain the most reliable option for home use.

Fever Grades: Low, Moderate, and Dangerous

Not all fevers carry the same level of concern. A helpful way to think about it:

  • Low-grade fever: 100.4°F to about 102°F (38°C to 38.9°C). Common with mild infections and usually manageable at home.
  • Moderate fever: 102°F to 103°F (38.9°C to 39.4°C). You’ll likely feel noticeably unwell, with aches, chills, or fatigue.
  • High fever: 103°F (39.4°C) and above. Adults at this level most often look and act visibly sick.
  • Hyperpyrexia: Above 106.7°F (41.5°C). This is a medical emergency that can damage the brain, heart, lungs, kidneys, and liver. It can cause seizures, confusion, loss of consciousness, and permanent brain damage.

The jump from a moderate fever to a high fever is where the body starts struggling. At 103°F and above, dehydration accelerates, heart rate climbs, and confusion can set in. Hyperpyrexia is rare but life-threatening, and it requires immediate emergency care.

Why Your Body Temperature Isn’t Always 98.6°F

The old standard of 98.6°F as “normal” is actually an average from a study done in the 1800s. In reality, normal body temperature varies from person to person and throughout the day. Your temperature is lowest in the early morning and highest in the late afternoon or evening, sometimes by a full degree. So a reading of 99°F at 8 p.m. may be perfectly normal, while the same reading at 6 a.m. could signal something is off.

Whether you’re naturally a morning person or a night owl also shifts when your temperature peaks. Morning types hit their high point earlier in the day, while evening types peak later, with roughly a two-hour difference between them. This is why tracking your own baseline over a few days when you’re healthy gives you a much more useful reference point than relying on 98.6°F.

Fever Thresholds Are Lower for Older Adults

Adults over 65 run cooler at baseline. Research on nursing home residents found that the average healthy temperature was 97.7°F, nearly a full degree below the traditional “normal.” This means an older adult with a reading of 99°F could already have a significant fever, even though that number wouldn’t raise alarms in a younger person.

The best approach for older adults is to define a fever as 1.4°F above their individual baseline. If you don’t know someone’s usual temperature, a reading of 99°F or higher is a reasonable rule of thumb for concern. This matters because older adults are more vulnerable to infections like pneumonia and urinary tract infections, and a delayed response can allow those infections to progress quickly. A “low” fever in an older person deserves the same attention as a higher one in a younger adult.

What Actually Happens During a Fever

A fever isn’t a malfunction. It’s your immune system deliberately raising your internal thermostat. When your body detects an infection, immune cells release signaling molecules that act on the hypothalamus, the part of your brain that controls temperature. The hypothalamus raises its target temperature, and your body responds by constricting blood vessels near the skin (which is why you feel cold) and sometimes triggering shivering to generate heat. These processes continue until your blood temperature matches the new, higher set point.

This elevated temperature actually helps your immune system fight off invaders. Many bacteria and viruses reproduce less efficiently at higher temperatures, and certain immune cells work better when the body is warmer. That’s why mild to moderate fevers don’t always need to be treated with medication. The discomfort is real, but the fever itself is doing useful work.

Managing a Fever at Home

For fevers under 103°F in otherwise healthy adults, home care is usually enough. Over-the-counter pain relievers like acetaminophen and ibuprofen both reduce fever effectively. The key safety rule with acetaminophen is to never exceed 4,000 milligrams in 24 hours, which is easier to hit than people expect since acetaminophen appears in dozens of combination cold and flu products. Always check labels.

Beyond medication, staying hydrated is the single most important thing you can do. Fever increases fluid loss through sweating and faster breathing, and dehydration makes every symptom worse. Water, broth, and electrolyte drinks all help. Light clothing and a comfortable room temperature let your body release heat naturally. Ice baths and alcohol rubs are old remedies that can actually backfire by causing shivering, which raises your core temperature further.

When a Fever Signals Something Serious

A fever on its own is rarely dangerous for healthy adults, but certain combinations of symptoms point to more urgent problems. A high temperature paired with a stiff neck, severe headache, confusion, or a rash that doesn’t fade when you press on it can indicate meningitis. Rapid breathing, a racing heart, and feeling much sicker than you’d expect from an ordinary cold are early warning signs of sepsis, a dangerous immune overreaction to infection. Notably, sepsis can also cause an abnormally low temperature, below 96.8°F (36°C), which is just as concerning as a high one.

A fever of 103°F or above that doesn’t respond to medication within a few hours, or any fever lasting more than three days without improvement, warrants medical evaluation. The same is true for fevers that return after seeming to resolve, which can suggest a secondary infection or a condition that needs specific treatment rather than just time and rest.
