What Is Considered a Low Testosterone Level?

A total testosterone level below 300 ng/dL is the widely accepted clinical cutoff for low testosterone in men. This threshold comes from the American Urological Association and is used by most doctors as the starting point for evaluating testosterone deficiency. But that single number doesn’t tell the whole story, because age, the time of day your blood is drawn, and how much of your testosterone is actually available to your body all influence what “low” means for you specifically.

The 300 ng/dL Cutoff for Men

The American Urological Association classifies any total testosterone reading below 300 ng/dL as a reasonable cutoff supporting a diagnosis of low testosterone. Most lab reports you’ll see from your doctor use a reference range somewhere between 300 and 1,000 ng/dL for adult men, so anything flagged as below range typically falls under that 300 threshold.

That said, 300 ng/dL is a population-wide benchmark, not an age-adjusted one. A study in The Journal of Urology looking at men aged 20 to 44 found that average morning testosterone levels shift meaningfully by age bracket:

  • Ages 20 to 24: average of 501 ng/dL
  • Ages 25 to 29: average of 514 ng/dL
  • Ages 30 to 34: average of 456 ng/dL
  • Ages 35 to 39: average of 438 ng/dL
  • Ages 40 to 44: average of 430 ng/dL

A 25-year-old with a reading of 310 ng/dL technically clears the 300 cutoff, but he’s well below the average for his age group. Some clinicians take this context into account when deciding whether symptoms warrant further investigation, even if the lab result doesn’t carry a formal “low” flag.

Free Testosterone: The Number That Often Matters More

Total testosterone measures everything in your blood, including testosterone bound to proteins that your body can’t actually use. The most important of these proteins is called SHBG (sex hormone-binding globulin). When SHBG levels are high, it locks up more of your testosterone, leaving less available for your muscles, brain, and other tissues. You can have a total testosterone level that looks perfectly normal while the amount your body can actually access is too low.

This is why doctors sometimes order a free testosterone test alongside the total. The Endocrine Society uses a reference range of 50 to 200 pg/mL for calculated free testosterone in men, with values below 50 pg/mL considered low. If your total testosterone is borderline or your symptoms don’t match a normal-looking total number, free testosterone can reveal what’s really going on. Conditions like obesity, aging, liver disease, and thyroid problems can all push SHBG higher and effectively lower your usable testosterone without changing the total number much.

Low Testosterone in Women

Women produce far less testosterone than men, but it still plays an important role in sex drive, energy, muscle strength, and mood. The thresholds are much lower: a total testosterone below 25 ng/dL in women under 50, or below 20 ng/dL in women 50 and older, suggests androgen deficiency. For free testosterone, the corresponding cutoffs are below 1.5 pg/mL for women under 50 and below 1.0 pg/mL for those over 50.

The most common symptom is decreased sexual desire, often showing up as fewer sexual thoughts and fantasies rather than a sudden, dramatic change. Fatigue, muscle weakness (particularly noticeable in physically active women), reduced arousal, and difficulty reaching orgasm are also reported. These symptoms overlap with many other conditions, which is one reason testosterone deficiency in women often goes unrecognized.

Why Morning Testing Matters

Testosterone levels fluctuate throughout the day and are highest in the early morning. Blood draws for testosterone testing should happen between 7 and 10 a.m. to capture your peak level. A sample taken in the afternoon can read significantly lower and may produce a misleadingly low result. Fasting before the test is also recommended, as eating can temporarily lower testosterone readings.

One low reading isn’t enough for a diagnosis. The Endocrine Society recommends confirming the result by repeating the morning fasting blood draw on a separate day. Testosterone can dip temporarily from poor sleep, acute illness, stress, or certain medications, so a single snapshot doesn’t reliably reflect your baseline.

What Low Testosterone Feels Like

Some men with levels below 300 ng/dL have no noticeable symptoms at all. Others experience a combination of sexual and non-sexual changes:

  • Low sex drive is the most commonly reported symptom
  • Erectile difficulties, particularly trouble maintaining erections
  • Increased body fat, especially around the midsection
  • Loss of muscle size and strength, even with regular exercise
  • Fatigue that doesn’t improve with adequate sleep
  • Mood changes, including depression and difficulty concentrating
  • Sleep problems such as insomnia
  • Bone loss over time, raising the risk of fractures

A diagnosis of hypogonadism (the clinical term for testosterone deficiency) requires both consistently low lab values and symptoms that match. A low number alone, without symptoms, generally doesn’t lead to treatment. And symptoms alone, without confirmed low levels, point toward investigating other causes first.

Why Lab Results Vary Between Tests

If you’ve had testosterone tested at two different labs and gotten noticeably different numbers, the testing method may be partly responsible. The CDC runs a Hormone Standardization Program that certifies labs whose testosterone assays fall within 6.4% of a gold-standard reference method across a range of 2.50 to 1,000 ng/dL. Not every lab participates in this program, and uncertified assays can produce less consistent results. If your result is borderline, it’s worth confirming with a repeat test, ideally at the same lab using the same method, to reduce this kind of variability.

Beyond lab differences, day-to-day fluctuations in your own body are real. Sleep quality the night before, recent intense exercise, alcohol consumption, and body weight all influence where your testosterone lands on any given morning. This is exactly why guidelines call for at least two separate confirmatory tests before drawing conclusions from a number.
