Dropsy is not a disease itself but a symptom of internal organ failure, most often kidney or gill dysfunction, that causes a betta fish to swell with fluid and its scales to protrude outward like a pinecone. By the time you notice the classic pinecone appearance, the condition is advanced and the survival rate is roughly 25% even with treatment. Understanding what’s happening inside your fish can help you act quickly or, better yet, prevent it from developing in the first place.

Why Bettas Are Vulnerable to Dropsy

Freshwater fish like bettas live in water that is less dense than their body tissues. Because of this difference, water constantly tries to push into the fish’s body through passive diffusion, mainly through the skin and gills. A healthy betta’s kidneys and gills work continuously to pump that excess water back out. It’s a nonstop balancing act, and when either organ system is compromised, the fish starts taking on water it can’t get rid of.

That excess water collects between the scales, around the internal organs, and throughout the body cavity. Left unchecked, the fluid buildup leads to the swollen, pinecone-like appearance that defines dropsy, and eventually to full organ failure.

What Causes the Organ Damage

There is no single cause. The infectious agents that can trigger kidney or gill failure number in the hundreds, which is part of what makes dropsy so difficult to treat. The most common culprits are bacterial infections, particularly gram-negative bacteria that thrive in freshwater aquariums. But viral infections, parasites, and even chronic stress can damage these organs over time.

Poor water quality is the most frequent underlying trigger. Elevated ammonia and nitrite levels are directly toxic to gill tissue. When gills are damaged, the fish loses one of its two main tools for removing excess water. Chronic exposure to even mildly poor water conditions weakens the immune system, leaving the fish vulnerable to opportunistic infections that attack the kidneys. Sudden temperature swings, overcrowding, and aggressive tankmates all compound the stress that opens the door to organ failure.

How to Recognize Dropsy

The hallmark sign is pineconing: scales that stand away from the body instead of lying flat, giving your betta the unmistakable look of a pinecone when viewed from above. This happens because fluid accumulates in the skin between the scales, pushing them outward. The belly visibly bloats, sometimes dramatically, and the fish may appear round from every angle rather than just slightly full after eating.

Before the pineconing becomes obvious, you may notice subtler changes. Your betta might become lethargic, lose interest in food, or spend more time resting at the bottom or floating near the surface. The eyes can bulge outward (a condition called popeye), and the gills may look pale or swollen. Feces may become stringy or discolored. These early signs are your best window for intervention.

Dropsy vs. Constipation

A bloated betta isn’t always a betta with dropsy. Constipation and swim bladder issues can both cause visible swelling, and panicking over a constipated fish is common among betta owners. The key difference is the scales. In constipation, the belly swells but the scales remain flat against the body. In dropsy, the scales protrude outward because fluid is collecting underneath them. Look at your fish from directly above. If the scales fan out like a pinecone, it’s dropsy. If the body is round but the scales are smooth, you’re likely dealing with a digestive issue that responds well to fasting for a day or two and offering a blanched pea.

Treatment Options

Treating dropsy is genuinely difficult, and honesty matters here: once full pineconing has developed, most fish do not survive. The kidneys are likely already failing, and reversing that level of damage is rare. That said, early-stage cases caught before severe pineconing can sometimes be pulled back with aggressive treatment.

Isolate and Improve Water Quality

Move your betta to a clean hospital tank with warm, stable water (78 to 80°F). Pristine water quality reduces the workload on compromised kidneys and gills. Keep ammonia and nitrite at zero and perform frequent small water changes to maintain those levels.

Epsom Salt Baths

Epsom salt (magnesium sulfate) can help draw out excess fluid. For a short bath, dissolve 1 teaspoon of Epsom salt per gallon of clean, temperature-matched water and let your betta sit in it for 15 to 20 minutes before returning it to the hospital tank. This is a supportive measure, not a cure. It eases the fluid burden but doesn’t address the underlying infection or organ damage.

Antibiotics

Because most aquarium bacterial infections involve gram-negative bacteria, broad-spectrum antibiotics designed for that class are the standard approach. Kanamycin-based treatments are among the most commonly recommended. These can be dosed directly into the water (once every 48 hours for up to three doses) or mixed into food for fish that are still eating. Mixing medication with food tends to deliver it more effectively to the internal organs where the infection lives. If your betta is still accepting food, that’s the preferred route.

Slightly raising the salinity of the hospital tank can also help by reducing the osmotic pressure on your fish. When the surrounding water is slightly saltier, less water passively pushes into the fish’s body, giving the kidneys some relief. Aquarium salt at a low dose (1 teaspoon per gallon) is sometimes used for this purpose, though it should not be combined with certain medications, so check compatibility first.

Realistic Expectations

Even with prompt treatment, dropsy carries a poor prognosis. Early estimates from experienced fishkeepers put survival at around 25% when caught early. Once the scales have fully pineconed and the fish has stopped eating, the internal damage is typically too severe to reverse. At that stage, some owners choose humane euthanasia (clove oil is the most commonly recommended method) rather than prolonging suffering.

Fish that do recover often remain fragile. The kidneys may never fully regain normal function, making the fish more susceptible to future episodes. Recovered bettas need exceptionally stable, clean water conditions going forward.

How to Prevent Dropsy

Since dropsy is the end result of organ failure rather than a specific infection, prevention comes down to keeping your betta’s kidneys and gills healthy over the long term. That means two things above all else: clean water and a strong immune system.

Test your water regularly and keep ammonia and nitrite at zero. Nitrates should stay below 20 ppm through regular water changes. A heated, filtered tank of at least 5 gallons gives your betta enough stable volume to avoid the temperature swings and waste spikes that come with smaller containers. Avoid sudden changes in water parameters when doing water changes.

Diet matters more than many owners realize. Bettas are insectivores, and a varied diet supports the immune function that keeps infections from gaining a foothold. High-quality betta pellets should form the base of their diet, supplemented with frozen or live foods like brine shrimp, bloodworms, and daphnia. Overfeeding is a common problem. Uneaten food decays quickly, spikes ammonia levels, and stresses the organs that are already working hard to maintain fluid balance. Feed small portions once or twice daily, only as much as your betta can finish in about two minutes.

Stress suppresses the immune system in fish just as it does in humans. Aggressive tankmates, a tank with no hiding spots, bright lights with no cover, and frequent disruptions all contribute to chronic stress that leaves your betta vulnerable. A calm, well-planted environment with consistent routines goes a long way toward keeping the internal organs healthy enough to do their job.
