What Is Fucidin Cream Used For and How Does It Work?

Fucidin cream is a topical antibiotic used to treat bacterial skin infections. Its active ingredient, fusidic acid, is particularly effective against Staphylococcus aureus, the bacterium behind many common skin problems like impetigo, infected cuts, and inflamed hair follicles.

Conditions Fucidin Cream Treats

Fucidin cream targets a specific range of bacterial skin infections. It works against Staphylococcus aureus and Streptococcus species, which are responsible for the majority of skin infections people encounter in daily life. The main conditions it treats include:

  • Impetigo (school sores): A highly contagious infection that causes crusty, honey-colored sores, most common in children.
  • Folliculitis: Infected hair follicles that appear as small red bumps or white-headed pimples.
  • Infected cuts and abrasions: Minor wounds that have become red, swollen, or started producing pus.
  • Infected eczema: Atopic dermatitis that has been colonized by bacteria, causing the rash to worsen, weep, or crust over.
  • Erythrasma: A bacterial skin infection causing reddish-brown patches, typically in skin folds.
  • Paronychia: Infection of the skin around the fingernails or toenails.

Fucidin can be used on its own for mild, localized infections or alongside oral antibiotics for more widespread problems.

How It Works

Fusidic acid kills bacteria by interfering with their ability to build proteins. Bacteria need a molecule called Elongation Factor-G to assemble proteins on their ribosomes (the tiny machines inside cells that manufacture proteins). Fusidic acid locks onto this molecule and prevents it from detaching from the ribosome, effectively jamming the protein-building machinery. Without new proteins, the bacteria can’t grow, repair themselves, or multiply, so the infection clears.

How Effective Is It?

For localized skin infections like impetigo, fusidic acid performs well. A review published in BMJ Evidence-Based Medicine found that topical antibiotics, including fusidic acid, had better cure rates than placebo, oral erythromycin, and disinfecting agents. No single topical antibiotic outperformed another, but fusidic acid and mupirocin (another common topical antibiotic) emerged as the two most effective options. Both were considered preferable to oral antibiotics for localized infections, based on both efficacy and fewer side effects.

Fucidin vs. Fucidin H

You may see two versions of this cream on a prescription. Plain Fucidin contains only the antibiotic fusidic acid. Fucidin H also contains hydrocortisone, a mild steroid that reduces redness, swelling, and itching. The combination version is typically prescribed when a skin condition like eczema has become infected, because it treats the bacterial infection and the underlying inflammation at the same time.

The application schedules differ. Plain Fucidin cream is usually applied 3 or 4 times a day, while the steroid-containing version is applied twice a day. Your pharmacist’s label will specify which schedule to follow.

How to Apply Fucidin Cream

Apply a thin layer to the affected area 3 to 4 times a day (or twice daily for the steroid combination). Wash your hands before and after application unless the infection is on your hands. Treatment courses are typically kept short, usually no longer than 7 to 10 days, to reduce the risk of bacteria developing resistance to the antibiotic. Using it for longer than recommended, or applying it to uninfected skin “just in case,” increases the chance that it won’t work when you actually need it.

Side Effects

Fucidin cream is well tolerated by most people. The most common side effect is mild skin irritation at the application site, including stinging, itching, or redness. This affects fewer than 1 in 100 users. Serious side effects are rare, occurring in fewer than 1 in 1,000 people. In very rare cases, a severe allergic reaction (anaphylaxis) can occur, with symptoms like swelling of the face or throat, difficulty breathing, or a widespread rash. This requires emergency medical attention but is exceptionally uncommon with a topical cream.

Resistance: Why Treatment Length Matters

Bacteria can develop resistance to fusidic acid, and this is a growing concern for clinicians. Some bacteria produce a family of proteins called FusB that protect the drug’s target, allowing the bacteria to keep building proteins even in the presence of the antibiotic. This is why prescriptions are kept short. Using Fucidin for extended periods, applying it to large areas of skin, or using leftover cream from a previous prescription all increase the likelihood that resistant bacteria will emerge. If your infection hasn’t improved after a full course, the bacteria may need a different antibiotic rather than more of the same one.

Safety During Pregnancy and Breastfeeding

Fusidic acid cream is generally considered safe during breastfeeding, but practical precautions matter. If you need to apply it anywhere near your breasts or nipples (for example, to treat impetigo), wash the cream off before feeding your baby. Cream is easier to wash off than ointment, making it the better choice if you’re nursing. After applying Fucidin anywhere on your body, wash your hands before picking up your baby to avoid transferring the medication to their skin. There is no evidence that fusidic acid affects fertility in men or women.

Who Should Not Use It

The only absolute contraindication is a known allergy to fusidic acid, sodium fusidate, or any of the inactive ingredients in the cream. If you’ve had a previous reaction to Fucidin or a similar product, let your prescriber know so they can choose an alternative like mupirocin. Fucidin cream is not effective against fungal infections, viral infections like cold sores, or conditions that simply look like infections but aren’t. If your skin problem isn’t caused by susceptible bacteria, the cream won’t help and may delay appropriate treatment.
