Glaucoma in dogs is a condition where fluid builds up inside the eye, raising internal pressure to levels that damage the optic nerve and retina. Normal eye pressure in dogs sits around 10 to 19 mmHg, and readings above 25 mmHg with visible symptoms point to glaucoma. Left untreated, the condition is painful and can lead to permanent blindness, sometimes within hours in acute cases.
How the Eye’s Drainage System Fails
The eye constantly produces a clear fluid called aqueous humor, which nourishes internal structures and maintains the eye’s shape. This fluid is made by a structure behind the iris called the ciliary body, and it normally drains out through a tiny angle where the cornea and iris meet. The system works like a slow-running faucet with an open drain: fluid flows in, fluid flows out, and pressure stays balanced.
Glaucoma develops when that drain gets blocked or narrowed. Fluid keeps being produced at the same rate, but with nowhere to go, pressure inside the eye climbs. That rising pressure compresses the optic nerve and blood vessels at the back of the eye, gradually killing the cells responsible for vision. The damage is irreversible once it occurs, which is why early detection matters so much.
Primary vs. Secondary Glaucoma
Dogs develop glaucoma through two distinct pathways, and the distinction affects both treatment and prognosis.
Primary glaucoma is an inherited condition. Dogs with primary glaucoma are born with a structural defect in the drainage angle of the eye. The angle may be too narrow, or the tiny ligaments that keep it open may be malformed (a condition called pectinate ligament dysplasia). Abnormal protein deposits can also clog the drainage pathway, adding further resistance. Primary glaucoma has been documented in at least 42 predisposed breeds, and it typically affects one eye first before eventually developing in the other.
Secondary glaucoma results from another eye problem that physically blocks drainage. Inflammation inside the eye (uveitis), lens displacement, tumors, bleeding, or advanced cataracts can all push tissue into the drainage angle and obstruct it. Secondary glaucoma is more common overall and can affect any breed.
Breeds at Higher Risk
Primary glaucoma shows a strong genetic component, with certain breeds appearing far more often in clinical data. American Cocker Spaniels, Basset Hounds, Chow Chows, Shar-Peis, Boston Terriers, Shih Tzus, and Siberian Huskies all carry elevated risk in North America. English Springer Spaniels, Flat-Coated Retrievers, Great Danes, and Samoyeds are particularly prone to the drainage angle malformation that leads to angle-closure glaucoma.
Age of onset varies by breed. A retrospective study spanning nearly a decade found that Shiba Inus developed glaucoma earliest, around 3 years of age on average, while Yorkshire Terriers and Toy Poodles tended to be diagnosed closer to 10 or 11 years old. Most breeds fell in the 7 to 9 year range, with American Cocker Spaniels averaging about 7.8 years and Shih Tzus about 8.1 years at diagnosis. If you own a predisposed breed, regular eye pressure checks starting in middle age can catch problems before permanent damage sets in.
Signs to Watch For
Glaucoma can appear suddenly (acute) or develop gradually (chronic), and the signs look different depending on the stage.
In an acute episode, the eye becomes visibly painful. Your dog may squint, paw at the affected eye, or shy away from being touched on that side of the head. The white of the eye often turns red from engorged blood vessels, and the cornea (the clear front surface) may take on a bluish or cloudy haze. The pupil in the affected eye is frequently dilated and doesn’t respond normally to light. Some dogs become lethargic, lose their appetite, or seem generally “off” because the pain is significant, similar to a severe migraine in people.
Chronic glaucoma develops more quietly. You might notice the eye gradually enlarging, which happens because sustained pressure stretches the eyeball over weeks or months. The eye may look slightly bigger than the other one, and vision loss may only become apparent when your dog starts bumping into furniture or misjudging steps on one side. By the time the eye is visibly enlarged, substantial and often irreversible damage has already occurred.
How Glaucoma Is Diagnosed
Veterinarians measure eye pressure using an instrument called a tonometer. The two types used most often are rebound tonometers and applanation tonometers, both reliable and relatively quick. Rebound tonometry doesn’t even require numbing drops, making it well-tolerated by most dogs. Applanation tonometry does need a topical anesthetic but is equally accurate.
A reading above 25 mmHg alongside clinical signs is consistent with a glaucoma diagnosis. Technique matters: if the dog’s neck is compressed by restraint, the head is positioned below the heart, or there’s accidental pressure on the eyeball, the reading can be falsely elevated. Your vet will take care to position your dog properly before testing.
For dogs in predisposed breeds, a specialized exam called gonioscopy can evaluate the drainage angle before glaucoma develops. This helps determine whether the angle is narrow or malformed, giving you and your vet a head start on monitoring or preventive treatment for the unaffected eye.
Medical Treatment
The first goal of treatment is bringing eye pressure down quickly, especially in acute cases. Most dogs with glaucoma end up on a combination of eye drops that work through different mechanisms.
One common class of drops reduces how much fluid the eye produces by blocking an enzyme in the ciliary body. These can cut fluid production by 40% to 60%, which significantly lowers pressure. Another class works on the drainage side, helping fluid exit through an alternative outflow pathway in dogs. Beta-blocker drops both slow fluid production and modestly improve outflow. Your vet will often combine two or three of these medications, each given multiple times a day.
In an emergency, when pressure is dangerously high and vision is at immediate risk, your vet may use an intravenous treatment that rapidly pulls fluid out of the eye by changing the balance of fluid pressure between the blood and the eye’s interior. This buys time while other medications take effect.
Surgical Options
When medications alone can’t control pressure, surgery becomes the next step. The approach depends on whether the eye still has useful vision.
For eyes with remaining vision, procedures aim to reduce fluid production or improve drainage. Laser-based surgery can selectively target the cells that produce aqueous humor, lowering production enough to bring pressure into a safe range. Drainage implants, small tubes surgically placed in the eye, can also create a new pathway for fluid to escape. These procedures are typically performed by veterinary ophthalmologists and can preserve sight for months to years, though many dogs still need some ongoing medication.
For eyes that are already blind and painful, the priority shifts to comfort. Removing the eye entirely (enucleation) is the most definitive option and eliminates the source of pain permanently. While the idea sounds drastic, dogs adapt remarkably well to life with one eye or even no eyes, relying on their other senses. A cosmetic prosthetic can be placed if appearance is a concern. Another option involves injecting medication into the eye to stop fluid production altogether, which shrinks the eye over time but avoids surgery.
Living With a Glaucoma Diagnosis
Glaucoma in dogs is a lifelong management challenge rather than a condition with a simple cure. If your dog is diagnosed with primary glaucoma in one eye, there’s a high likelihood the other eye will eventually be affected. Many veterinary ophthalmologists will start preventive drops in the second eye to delay onset, and regular pressure checks (every few months) become part of your routine.
Daily eye drops are the reality for most glaucoma dogs. Depending on the medications prescribed, you may need to administer drops two to four times a day. Consistency matters because missed doses allow pressure to spike, and each spike causes cumulative damage. If you struggle with the schedule, talk to your vet about combination drops or longer-acting options.
Dogs who lose vision to glaucoma still live full, happy lives. They memorize furniture layouts, navigate familiar routes with confidence, and rely heavily on smell and hearing, senses that were already dominant for them. Keeping furniture in consistent places, using verbal cues, and blocking off hazards like pools or staircases makes the transition smoother for both of you.
