Swollen ankles improve with a combination of elevation, movement, compression, and dietary changes. The swelling itself, called edema, happens when tiny blood vessels leak fluid into surrounding tissues. For most people, the cause is straightforward: sitting too long, eating too much salt, hot weather, or hormonal shifts during the menstrual cycle. But persistent or sudden swelling can signal something more serious, so understanding both the remedies and the warning signs matters.
Elevate Your Legs Above Your Heart
Elevation is the simplest and most immediately effective way to reduce ankle swelling. Gravity is working against you all day, pulling fluid downward into your feet and ankles. When you raise your legs, you reverse that pull and let fluid drain back toward your core, where your kidneys can process it.
The key detail most people miss: your ankles need to be higher than your heart for elevation to work well. Propping your feet on a low ottoman while you sit on the couch isn’t enough. Lie down and stack pillows under your calves so your feet sit above chest level. Do this whenever you’re resting, and aim for at least 20 to 30 minutes at a time. If your swelling is persistent, elevating several times throughout the day makes a noticeable difference.
Ankle Pumps and Calf Movement
Your calf muscles act as a pump for the veins in your lower legs. Every time you flex and contract them, they squeeze blood and fluid upward toward your heart. When you sit or stand still for hours, that pump stops working, and fluid pools around your ankles.
Ankle pumps are a simple exercise you can do anywhere. Sit or lie with your legs extended, then point your toes toward your knees as far as you can, hold briefly, and then point them away from you. Alternate back and forth for two to three minutes, and repeat this two to three times per hour when you’re sedentary. Walking is even better if you’re able. Even a short lap around the room every 30 minutes keeps the calf pump active and prevents fluid from settling.
Cut Back on Sodium
Salt causes your body to hold onto water, and that extra fluid often shows up as ankle swelling first. For people already dealing with edema, health guidelines from Georgetown University recommend keeping daily sodium intake between 1,375 and 1,800 mg. For context, a single fast food meal can easily contain 1,500 mg or more, and the average American consumes over 3,400 mg daily.
The biggest sources of hidden sodium aren’t the salt shaker on your table. They’re processed foods: canned soups, deli meats, frozen meals, bread, condiments, and restaurant dishes. Reading nutrition labels and cooking more meals at home are the most practical ways to bring your intake down. You won’t see results overnight, but within a few days of reducing sodium, your body starts releasing the extra fluid it was holding.
Drink More Water, Not Less
It sounds counterintuitive, but drinking more water actually helps reduce swelling. When you’re mildly dehydrated or consuming a lot of salt, your body holds onto every bit of fluid it can. Staying well hydrated signals your kidneys that it’s safe to flush out excess fluid and sodium. Think of it as giving your kidneys the raw material they need to do their job. There’s no magic number of glasses per day, but if your urine is dark yellow, you’re likely not drinking enough.
Compression Socks
Compression socks apply steady, graduated pressure to your lower legs, preventing fluid from accumulating in the tissues around your ankles. They’re tightest at the ankle and gradually loosen toward the knee, which helps push fluid upward.
Compression levels are measured in mmHg (millimeters of mercury). For mild, everyday swelling, 15 to 20 mmHg is usually sufficient. These are widely available without a prescription and are often marketed for travel, desk jobs, or general leg fatigue. If you have more significant swelling from chronic venous insufficiency or lymphedema, a healthcare provider may recommend 20 to 30 mmHg or higher. The important thing is to put them on in the morning before swelling builds up during the day. Pulling compression socks over already-swollen ankles is harder and less effective.
Magnesium and Diet
Some people find that getting enough magnesium helps with fluid retention, particularly around menstruation. While clinical research specifically linking magnesium supplements to reduced edema is limited, many adults don’t meet the recommended daily intake: 420 mg for men and 320 mg for women over 31. Foods rich in magnesium include spinach, pumpkin seeds, dark chocolate, almonds, and black beans. If you choose a supplement, keep the dose under 350 mg per day from supplements alone, as higher amounts commonly cause digestive issues like diarrhea.
Why Diuretics Often Don’t Help
Many people assume that “water pills” are the obvious fix for swollen ankles, but in most common scenarios, diuretics are ineffective and can cause real harm. They don’t work for swelling caused by prolonged sitting, medication side effects, or lymphedema. In older adults, diuretics are one of the most common causes of side effects, including weakness, confusion, dizziness, and falls. One study found that a fifth of adults over 65 were taking diuretics, often without benefit and with significant risk.
Diuretics are appropriate in specific medical conditions like heart failure, where a doctor prescribes and monitors them closely. But for the everyday ankle swelling most people search about, elevation, compression, movement, and sodium reduction are more effective and far safer.
When Ankle Swelling Is a Warning Sign
Most ankle swelling is harmless and responds to the strategies above. But certain patterns require prompt medical attention.
- Sudden swelling in one leg only, especially with calf pain, warmth, or redness, could indicate a deep vein thrombosis (a blood clot). This needs urgent evaluation.
- Swelling in both legs with shortness of breath, difficulty breathing when lying flat, or unusual fatigue may point to heart failure.
- Swelling that doesn’t improve with elevation and sodium reduction over a week or two could reflect kidney disease, liver problems, or chronic venous insufficiency, all of which need a proper diagnosis.
- Pitting edema, where pressing your thumb into the swollen area leaves a visible dent that takes several seconds to fill back in, often suggests a systemic cause worth investigating.
If your swelling started after beginning a new medication, that’s worth flagging with whoever prescribed it. Certain blood pressure medications, hormones, steroids, and anti-inflammatory drugs are well-known causes of ankle swelling, and the fix is adjusting the medication rather than adding a diuretic on top of it.
