Hydroxyzine pamoate 25 mg capsules are primarily used to treat anxiety and itching (pruritus). The medication is also used as a sedative before or after surgery. It belongs to the first-generation antihistamine family, meaning it blocks histamine receptors throughout the body and brain, which is what gives it such a wide range of effects.
Anxiety and Tension Relief
Hydroxyzine pamoate is FDA-approved for the symptomatic relief of anxiety and tension. It works differently from more commonly known anti-anxiety medications like benzodiazepines. Instead of targeting the brain’s calming system directly, hydroxyzine blocks histamine receptors and certain serotonin and dopamine receptors in the brain. This produces a calming, sedative effect that can take the edge off anxiety without the addiction risk that comes with benzodiazepines.
For anxiety, the typical adult dose ranges from 50 to 100 mg taken four times a day, so a 25 mg capsule represents the lower end of the dosing range. Your prescriber may start you at a lower dose and adjust based on how you respond. Effects kick in quickly, usually within 15 to 30 minutes of taking a dose, which makes it useful for situational anxiety as well as ongoing symptoms.
Itching From Allergic Skin Conditions
The second major use is managing itching caused by allergic reactions. This includes chronic hives, eczema, contact dermatitis, and other conditions where histamine drives the itch. By blocking histamine at the receptor level, hydroxyzine interrupts the itch signal before it reaches your brain. For itching, the standard adult dose is 25 mg taken three or four times daily, making the 25 mg capsule a convenient single-dose unit for this purpose.
Hydroxyzine is sometimes preferred over newer, non-drowsy antihistamines for severe itching precisely because the sedation helps. If itching keeps you awake at night, the drowsiness becomes a benefit rather than a drawback.
Sedation Before and After Surgery
Hydroxyzine pamoate is also approved as a sedative for use before and after general anesthesia. Its combined calming and anti-nausea properties make it useful in surgical settings, where pre-procedure anxiety and post-procedure nausea are both common concerns.
Pamoate vs. Hydrochloride: Does the Form Matter?
You may have noticed that hydroxyzine comes in two forms: pamoate (sold as capsules, originally branded as Vistaril) and hydrochloride (sold as tablets and syrup, originally branded as Atarax). The difference is purely chemical. Both forms break down in your body and release the same active molecule, hydroxyzine, into your bloodstream. There is no clinical evidence that one form works better than the other when equivalent doses are used. Once absorbed, hydroxyzine is also rapidly converted into cetirizine, a well-known allergy medication, which contributes to its antihistamine effects.
In practice, doctors choose between the two based on what’s available at the pharmacy, what formulation works best for the patient, and what’s on the hospital or insurance formulary.
Common Side Effects
The most frequently reported side effect is drowsiness, which can range from mild sleepiness to significant sedation depending on the dose and your sensitivity. Dry mouth is also common, along with dizziness and headache. These side effects are driven by the drug’s anticholinergic properties, the same mechanism that blocks certain nerve signals throughout the body. Most people find that drowsiness lessens after a few days of regular use, though it can persist at higher doses.
Heart Rhythm Concerns
Hydroxyzine can affect the electrical activity of the heart, specifically by prolonging what’s called the QT interval. This is the time it takes your heart to reset between beats. When the QT interval stretches too long, it raises the risk of dangerous irregular heart rhythms. The European Medicines Agency issued updated safety guidance capping the maximum adult dose at 100 mg per day and recommending no more than 50 mg per day for older adults.
This risk is higher if you already have heart disease, an electrolyte imbalance (particularly low potassium or magnesium), a very slow heart rate, or a family history of sudden cardiac death. It also increases if you take other medications that affect heart rhythm. If any of these apply to you, hydroxyzine may not be the right choice.
Why It’s Not Recommended for Older Adults
The American Geriatrics Society lists hydroxyzine on its Beers Criteria, a widely used guide to medications that older adults should generally avoid. The recommendation is to avoid it entirely, and the reasoning is straightforward: older adults clear the drug from their bodies more slowly, and they’re more vulnerable to its anticholinergic effects. These include confusion, constipation, dry mouth, urinary retention, and blurred vision.
More concerning is the cumulative picture. Ongoing exposure to anticholinergic medications is associated with increased risk of falls, delirium, and dementia. This applies even to “younger-old” adults in their 60s and 70s, not just those in advanced age. If you’re over 65 and currently taking hydroxyzine, it’s worth discussing alternatives during a medication review.
Interactions With Alcohol and Other Sedatives
Hydroxyzine amplifies the effects of anything else that depresses your central nervous system. Alcohol is the most common offender. Combining the two can lead to severe drowsiness, dizziness, slurred speech, confusion, and dangerously slowed breathing.
The same risk applies to opioid painkillers, benzodiazepines, barbiturates, prescription sleep aids, and certain antidepressants. Cannabis and CBD products can also worsen sedation and confusion when used alongside hydroxyzine. If you take any of these, the combined sedative load can become dangerous, particularly the pairing with opioids, where slowed breathing is the primary concern.
Other anticholinergic medications, such as those used for overactive bladder or Parkinson’s disease, can stack with hydroxyzine’s anticholinergic effects, increasing the likelihood of dry mouth, constipation, confusion, and urinary problems.
What to Expect When Taking It
Hydroxyzine pamoate works fast. You’ll likely feel its effects within 15 to 30 minutes of swallowing a capsule, with drowsiness and calming hitting around the same time. It’s taken multiple times per day for ongoing conditions, typically three or four doses spread throughout the day. Because drowsiness is so common, many people take a larger portion of their daily dose at bedtime and smaller amounts during the day, though your prescriber will guide that timing.
Unlike benzodiazepines and opioids, hydroxyzine does not carry a significant risk of physical dependence or addiction. This makes it a useful option for people with a history of substance use or for situations where a prescriber wants to avoid controlled substances. It is not, however, as potent an anti-anxiety medication as benzodiazepines, so it works best for mild to moderate anxiety or as one piece of a broader treatment plan.
