Tylenol PM contains two active ingredients: 500 mg of acetaminophen (a pain reliever) and 25 mg of diphenhydramine hydrochloride (an antihistamine that causes drowsiness). The combination is designed to relieve minor aches and pains while helping you fall asleep. The recommended dose is two caplets at bedtime, and no more than two in a 24-hour period.
How the Two Ingredients Work Together
Acetaminophen is the same pain reliever found in regular Tylenol. It reduces pain and fever, though scientists still don’t fully understand its exact mechanism. At 500 mg per caplet (1,000 mg if you take the recommended two), it’s a standard dose for mild to moderate pain from headaches, muscle aches, or menstrual cramps.
Diphenhydramine is the drowsiness-inducing component. It’s a first-generation antihistamine, the same active ingredient in Benadryl. It crosses into the brain easily, where it blocks histamine receptors involved in keeping you awake. That’s what makes you sleepy. It also blocks acetylcholine receptors, which is responsible for many of its side effects like dry mouth and blurred vision.
The idea behind combining them is simple: if pain is keeping you up at night, one ingredient handles the pain while the other helps you drift off.
Side Effects Worth Knowing About
Most side effects come from the diphenhydramine. Common ones include dry mouth, dry eyes, constipation, and urinary retention. Because the drug lingers in your system (it has a relatively long half-life), many people experience next-day grogginess, poor concentration, reduced memory, and sluggish reaction times the morning after taking it. This “hangover” effect can compromise your performance at work or behind the wheel.
These effects tend to be worse in older adults. Diphenhydramine is on the Beers Criteria list of medications that are potentially inappropriate for people over 65, largely because of its anticholinergic effects and the risk of confusion and falls. In children, the drug can sometimes cause the opposite of drowsiness: agitation, confusion, and paradoxical stimulation.
The Liver Risk With Acetaminophen
Acetaminophen is safe at recommended doses for most people, but it has a narrow margin of safety when it comes to liver damage. Taking more than 4,000 mg in 24 hours can cause serious liver injury. Two Tylenol PM caplets contain 1,000 mg of acetaminophen, so you’re well within that limit if Tylenol PM is the only acetaminophen-containing product you’re taking. The danger comes from stacking: many cold medicines, flu remedies, and prescription painkillers also contain acetaminophen, and it’s easy to exceed the daily limit without realizing it.
Your liver processes acetaminophen by neutralizing a toxic byproduct with a compound called glutathione. If you overwhelm your liver’s glutathione supply, that toxic byproduct accumulates and damages liver cells. Acetaminophen toxicity accounts for nearly half of acute liver failure cases in North America and roughly a fifth of liver transplants in the United States.
Alcohol makes this worse. Both alcohol and acetaminophen rely on the same glutathione stores in the liver. Chronic heavy drinking depletes those stores, meaning even normal doses of acetaminophen can become dangerous. If you drink regularly, keeping acetaminophen use to rare occasions and staying under 2,000 mg per day is a safer approach.
Dosage Limits
The label is straightforward: take two caplets at bedtime, and do not take more than two caplets in any 24-hour period. That gives you 1,000 mg of acetaminophen and 50 mg of diphenhydramine per dose. Tylenol PM is not intended for daily or long-term use. It’s a short-term solution for occasional sleepless nights caused by minor pain. If you find yourself reaching for it regularly, that’s a sign to explore other options with a healthcare provider.
If You Only Need the Sleep Aid
Not everyone who takes Tylenol PM actually needs the pain reliever. If you’re looking for the sleep component alone, Simply Sleep (made by the same manufacturer) contains only 25 mg of diphenhydramine per caplet with no acetaminophen. This avoids unnecessary acetaminophen exposure and the liver-related risks that come with it. Generic diphenhydramine is also widely available and inexpensive.
Inactive Ingredients
Beyond the two active ingredients, Tylenol PM caplets contain a list of inactive ingredients used for coating, binding, and coloring. These include carnauba wax, microcrystalline cellulose, magnesium stearate, stearic acid, and several others that help the tablet hold together and dissolve properly. The caplet’s blue color comes from FD&C Blue No. 1 aluminum lake, and titanium dioxide provides the white base of the coating.
