Joint effusion in the knee is a buildup of excess fluid inside the knee joint capsule. A healthy knee contains only about 3 to 4 milliliters of synovial fluid, a slippery liquid that cushions and lubricates the joint. When the knee is injured or inflamed, the tissue lining the joint (called the synovial membrane) ramps up fluid production, and that small amount can increase dramatically, causing visible swelling, stiffness, and pain.
What Causes Fluid to Build Up
The causes fall into two broad categories: injuries and underlying diseases. On the injury side, a torn meniscus, a ligament sprain (like an ACL tear), or a fracture that extends into the joint can trigger rapid swelling, sometimes within hours. Repetitive stress from running or jumping can also irritate the joint enough to produce extra fluid over days or weeks.
Chronic conditions are equally common culprits. Osteoarthritis gradually wears down cartilage, and the resulting inflammation keeps the synovial membrane producing fluid long after any initial injury has healed. Rheumatoid arthritis and other autoimmune conditions cause the immune system to attack joint tissue directly, leading to persistent effusion. Gout and pseudogout deposit crystals inside the joint, which provoke intense, sometimes sudden swelling. Infections can also reach the joint through the bloodstream or an open wound, creating what’s known as septic arthritis, which is the most urgent cause.
How It Feels
The hallmark sign is a knee that looks puffy or swollen compared to the other side. You may notice the normal contours around the kneecap disappear, especially the small hollows on either side of the kneecap tendon. Common symptoms include:
- Aching or deep pain that may range from a mild tenderness to a constant throb
- Stiffness, particularly when trying to fully bend or straighten the knee
- A feeling of heaviness or tightness in the joint
- Warmth or redness over the skin
- Difficulty bearing weight or walking normally
Small effusions can be surprisingly easy to miss. You might only notice that your knee feels “off” or that bending it past a certain point is uncomfortable. Larger effusions are hard to ignore: the knee looks visibly swollen, and the pressure inside the joint can make it nearly impossible to fully extend or flex the leg.
How Doctors Detect It
A physical exam is usually the first step, and clinicians use a few specific hands-on tests to confirm fluid is present. The choice of test depends on how much fluid they suspect.
For small amounts of fluid, the bulge sign (also called the stroke test) is the go-to. With you lying flat and your knee straight, the examiner firmly strokes upward along the inner side of the knee to push fluid to the outer compartment, then strokes down the outer side. If a visible wave or bulge appears on the inner side within a few seconds, fluid is present. This test has the highest reliability among bedside exams, with a reliability coefficient of 0.97 and strong specificity when compared against ultrasound.
For moderate to large effusions, the patellar tap test works well. The examiner pushes fluid downward from above the kneecap, then presses the kneecap sharply toward the thighbone. If the kneecap “bounces” or rebounds off a cushion of fluid rather than sitting firmly against the bone, the test is positive. Its accuracy improves with larger volumes of fluid.
When the exam suggests effusion, imaging and lab work help pin down the cause. An X-ray can reveal arthritis or fractures. An MRI provides a detailed view of soft tissue, showing meniscus tears, ligament damage, or the extent of cartilage loss. Ultrasound is quick and effective at confirming fluid and measuring how much is there.
Joint Aspiration
If the cause isn’t obvious, or if infection is a concern, the doctor may use a needle to withdraw fluid directly from the joint. This procedure, called arthrocentesis, serves double duty: it relieves pressure immediately, and the fluid sample goes to a lab for analysis. The appearance and contents of the fluid tell a clear story. Normal or mildly arthritic fluid is clear and yellowish with a low white blood cell count (under 2,000 cells per microliter). Infected fluid looks cloudy or opaque and contains very high white blood cell counts, often above 50,000 cells per microliter. Gout crystals are visible under a microscope. This analysis is one of the fastest ways to distinguish a routine flare from something dangerous.
Treatment Options
Treating knee effusion means addressing both the fluid itself and whatever is causing it. For mild cases tied to overuse or a minor injury, rest, ice, compression, and elevation often reduce swelling within a few days. Anti-inflammatory medications help control pain and slow fluid production.
When the effusion is large enough to limit your movement, joint aspiration provides immediate relief. Removing the fluid drops the pressure inside the capsule, which reduces pain and restores range of motion. In many cases, after draining the fluid, a corticosteroid medication is injected into the joint to suppress inflammation and slow reaccumulation.
For effusions driven by an underlying condition, longer-term management is essential. Osteoarthritis-related effusions often respond to a combination of physical therapy, weight management, and periodic injections. Rheumatoid arthritis and other autoimmune causes typically require disease-modifying medications that calm the immune system. Gout-related effusions improve once uric acid levels are controlled. Septic arthritis demands urgent treatment with antibiotics, and the joint usually needs to be drained repeatedly or surgically washed out to prevent permanent damage.
What Happens if It’s Not Treated
Chronic or recurring effusion isn’t just uncomfortable. It can set off a chain of problems. Persistent swelling inhibits the quadriceps muscle on the front of the thigh. This happens through a reflex mechanism: the swollen joint sends signals that reduce the muscle’s ability to fully activate. Over weeks, the quadriceps weakens, the knee becomes less stable, and the cycle of irritation and fluid buildup gets harder to break.
Another common complication is a Baker’s cyst (also called a popliteal cyst). Excess fluid produced by the knee can travel through a one-way valve-like opening into the space behind the knee, pooling there and forming a fluid-filled sac. Baker’s cysts result from conditions like osteoarthritis or meniscus tears that keep the knee producing too much fluid. A small cyst may cause only mild tightness behind the knee, but a large one can interfere with blood flow in the leg, cause swelling in the calf, or compress nearby nerves, leading to numbness or weakness. If a Baker’s cyst ruptures, it can mimic the symptoms of a blood clot, with sudden calf pain and swelling.
Signs That Need Urgent Attention
Most knee effusions are not emergencies, but certain combinations of symptoms point to septic arthritis, which can destroy joint cartilage within days. Intense pain that comes on rapidly, a joint that’s hot and very swollen, fever, and an inability to use the joint at all are the key warning signs. People with artificial knee joints should be alert to new pain, swelling, or looseness even months or years after surgery, because prosthetic joint infections can develop on a slower timeline. Fast evaluation and treatment in these situations make a significant difference in preserving long-term joint function.
