Juvederm is made primarily of hyaluronic acid, a sugar molecule that occurs naturally in your skin, joints, and eyes. The hyaluronic acid in Juvederm is suspended in a saltwater buffer at concentrations of 20 to 26 mg/mL, depending on the specific product. Most formulations in the Juvederm line also contain 0.3% lidocaine, a mild numbing agent that reduces pain during injection.
Hyaluronic Acid: The Main Ingredient
Hyaluronic acid (HA) is a long-chain sugar polymer made up of two repeating building blocks: glucuronic acid and N-acetyl-glucosamine. Your body produces it constantly. In skin, it acts like a sponge, binding water molecules and keeping tissue plump and hydrated. A single gram of hyaluronic acid can hold up to six liters of water, which is what makes it so effective as a filler material.
The HA in Juvederm is not extracted from animal tissue. Older methods relied on rooster combs as a source, but modern production uses bacterial fermentation. Specifically, Juvederm’s hyaluronic acid is produced by Streptococcus equi bacteria grown in controlled lab conditions. This eliminates the contamination risks and ethical concerns tied to animal-derived ingredients, and it allows manufacturers to produce HA with consistent quality and purity.
Why the Hyaluronic Acid Is Cross-Linked
If you injected plain hyaluronic acid under the skin, your body would break it down within days. To make it last months or years, the HA chains in Juvederm are chemically cross-linked, meaning they’re bonded together into a three-dimensional mesh. This mesh resists the enzymes, free radicals, and mechanical forces that would otherwise degrade it quickly.
The cross-linking process is what separates different Juvederm products from one another and from competitors. Higher degrees of cross-linking produce a firmer, more structured gel that holds its shape under pressure, ideal for adding volume to cheeks or the jawline. Lower degrees of cross-linking create a softer, more spreadable gel better suited for fine lines or lips. The concentration of hyaluronic acid also varies across the product line: Voluma (designed for cheeks) uses 20 mg/mL, while other formulations like the original Juvederm Ultra range from 22 to 26 mg/mL.
The Carrier Solution
The cross-linked hyaluronic acid doesn’t come as a dry substance. It’s suspended in a physiologic buffer, a sterile saltwater solution balanced to match your body’s natural pH of about 7.4. This buffer contains sodium chloride (table salt) and phosphate compounds that keep the gel at a stable acidity level. Because the solution matches your tissue’s chemistry, the filler integrates smoothly after injection without triggering irritation from pH imbalance.
Lidocaine for Comfort
Products labeled “XC” in the Juvederm line contain 0.3% lidocaine mixed directly into the gel. Lidocaine is a local anesthetic, the same numbing agent dentists use. By building it into the filler itself, the injection becomes progressively more comfortable as the treatment continues. Earlier Juvederm formulations didn’t include lidocaine, requiring separate numbing injections or topical creams before the procedure.
How Different Juvederm Products Compare
The Juvederm family includes several products that share the same core ingredients but differ in their physical properties. What changes between them is the ratio of high-weight to low-weight hyaluronic acid chains, the degree of cross-linking, and the final concentration. These adjustments give each product a distinct thickness and flexibility.
- Voluma XC is the firmest option, designed for deep injection in the cheeks and midface to restore volume. Its robust cross-linking helps it resist compression and maintain lift for up to two years.
- Vollure XC sits in the middle, with moderate firmness suited for nasolabial folds (the lines running from your nose to the corners of your mouth). It typically lasts up to 18 months.
- Volbella XC is the softest and lightest, formulated for lips and fine perioral lines. Its lower cross-linking creates a natural, subtle result rather than dramatic volume.
All three use the same bacterial-fermented hyaluronic acid, the same lidocaine concentration, and the same type of buffered carrier solution. The differences come down to engineering the gel’s texture for specific areas of the face.
How Your Body Breaks It Down
Despite cross-linking, Juvederm is not permanent. Your body gradually degrades the filler through the same process it uses to recycle its own hyaluronic acid. Enzymes called hyaluronidases, which are present in most tissues, clip the bonds between the sugar units in the HA chain. One type of hyaluronidase first chops the long chains into mid-sized fragments, which are then pulled into cells and broken down further into tiny sugar pieces that your body reabsorbs harmlessly.
Cross-linking slows this process considerably, which is why Juvederm lasts months to years rather than days. But mechanical forces (chewing, facial expressions) and exposure to free radicals from UV light and normal metabolism also chip away at the gel over time. The rate of breakdown varies by injection site: areas with more movement, like the lips, tend to metabolize filler faster than relatively static areas like the cheeks.
