What Is Keppra XR? Uses, Side Effects & Dosing

Keppra XR is an extended-release anti-seizure medication containing the active ingredient levetiracetam. It’s taken once daily to help prevent seizures in people with epilepsy, and it comes in 500 mg and 750 mg tablets. The “XR” stands for extended release, meaning the drug is absorbed gradually over several hours rather than all at once.

How Keppra XR Works

Levetiracetam works differently from most other anti-seizure drugs. It binds to a specific protein found on the surface of tiny sacs (called synaptic vesicles) inside nerve cells. These sacs store chemical messengers that neurons use to communicate with each other. By attaching to this protein, known as SV2A, the medication appears to regulate how those chemical messengers are released, reducing the abnormal bursts of electrical activity that cause seizures.

This distinct mechanism is part of why levetiracetam is often well tolerated by people who haven’t responded to, or have had trouble with, other seizure medications.

How It Differs From Regular Keppra

Standard Keppra (the immediate-release version) is typically taken twice a day. Keppra XR delivers the same active ingredient but is designed to release it slowly, so you only need to take it once a day. The drug reaches its peak level in your blood about 4 hours after you swallow the tablet, compared to roughly 45 minutes for the immediate-release form. However, both versions have a similar half-life of about 7 to 9 hours and deliver the same total amount of medication over 24 hours.

For people switching from the twice-daily version, the transition is straightforward: the total daily dose stays the same. If you were taking 500 mg twice a day (1,000 mg total), you would take 1,000 mg of Keppra XR once daily.

Dosing and How to Take It

The typical starting dose for adults is 1,000 mg once daily. From there, the dose can be increased by 1,000 mg every two weeks if needed, up to a maximum of 3,000 mg per day. The tablets must be swallowed whole. Crushing or chewing them would break the extended-release coating and release the full dose at once, defeating the purpose of the formulation.

Because Keppra XR is taken only once a day, missing a dose can leave you without adequate protection against seizures. If you realize you’ve missed a dose, take it as soon as you remember. Keeping a consistent daily routine, like pairing it with a meal or setting a phone alarm, helps prevent gaps in coverage.

Common Side Effects

The most frequently reported side effects include drowsiness, dizziness, loss of coordination, and fatigue. These tend to be most noticeable when you first start the medication or after a dose increase, and they often improve over the first few weeks.

Behavioral and mood changes are the side effects that get the most attention. Some people experience unusual irritability, aggression, anxiety, or mood swings. Others report feeling emotionally flat or experiencing symptoms of depression. These changes can appear gradually, so they’re sometimes hard to recognize in yourself. People close to you may notice them first. Less commonly, the medication has been linked to thoughts of self-harm, which is a warning that applies broadly to anti-seizure medications as a class.

The informal term “Keppra rage” circulates widely in epilepsy communities. While not a medical diagnosis, it reflects a real phenomenon: a subset of people on levetiracetam develop notable irritability or angry outbursts that resolve when they stop the drug or switch to something else.

Pregnancy Considerations

Data from published studies and pregnancy registries have not established a link between levetiracetam use during pregnancy and major birth defects or miscarriage. That said, no study can completely rule out all risk. A North American pregnancy registry actively tracks outcomes in women taking anti-seizure medications, including Keppra XR, to build a more complete picture over time. For most women with epilepsy, uncontrolled seizures during pregnancy pose a greater danger than the medication itself.

Generic Availability

Generic versions of levetiracetam extended-release tablets are available, which can significantly reduce the cost compared to brand-name Keppra XR. Another branded version, Roweepra XR, also exists. If your pharmacy substitutes a generic, the active ingredient and dose are the same, though inactive ingredients like fillers and coatings may differ slightly.
