Kratom tea is used primarily for pain relief, mood enhancement, energy, and easing opioid withdrawal symptoms. Made from the leaves of a Southeast Asian tree (Mitragyna speciosa), the tea contains alkaloids that bind to the same receptors as opioid painkillers, producing effects that range from stimulating at low doses to sedating at higher ones. None of these uses are FDA-approved, and kratom carries real risks, but millions of people use it for these purposes.
How Kratom Tea Works in the Body
Kratom’s effects come from two key alkaloids: mitragynine, which makes up the bulk of the leaf’s active compounds, and 7-hydroxymitragynine, a much more potent but less abundant compound. Both interact with the same brain receptors that prescription opioids target. Mitragynine acts as a partial activator of these receptors, meaning it produces milder effects than full opioids like morphine. 7-Hydroxymitragynine, on the other hand, is a full activator with roughly 22 times stronger binding affinity than mitragynine, which is why even small amounts contribute significantly to kratom’s pain-relieving and sedating effects.
What makes kratom unusual is its dose-dependent split personality. At lower amounts, it acts more like a stimulant, boosting energy and alertness. At higher amounts, it shifts toward sedation, pain relief, and euphoria. This dual nature comes from how its alkaloids interact with different receptor systems at different concentrations, including opioid receptors and adrenergic receptors involved in energy and focus.
Pain Relief
Pain management is the single most common reason people brew kratom tea. In a U.S. study of 395 kratom users, nearly half met clinical criteria for chronic pain, and pain relief was the most frequently cited motivation for daily use, even among participants who weren’t recruited specifically for pain. The study, which tracked participants over 15 days using real-time reporting, found that recent kratom use was associated with lower current pain levels. People with chronic pain showed a particularly strong link between the subjective strength of kratom’s effects and the degree of pain reduction they experienced.
The anti-inflammatory properties of kratom may contribute to this. Lab research has shown that kratom extracts suppress the production of COX-2, an enzyme central to inflammation (the same enzyme targeted by ibuprofen and similar drugs). In animal studies, kratom extract reduced swelling in a dose-dependent way and inhibited the formation of inflammatory tissue. Mitragynine specifically blocks the production of prostaglandins, chemical messengers that amplify pain and inflammation at injury sites.
Mood and Anxiety
Many kratom tea drinkers report improved mood, reduced anxiety, and a greater sense of calm. Systematic reviews of kratom research confirm that it enhances mood and relieves anxiety for a significant portion of users. These effects are consistent with how partial opioid receptor activation works: it can produce a mild sense of well-being without the full sedation or respiratory depression associated with stronger opioids.
The specific strain matters here. White vein kratom varieties are most associated with mood enhancement, improved concentration, and alertness. Green vein strains offer a middle ground, with users describing them as mildly energizing and socially lubricating. Red vein strains lean more toward relaxation and calm, which some people find helpful for anxiety that manifests as restlessness or tension.
Energy and Focus
At lower doses, kratom tea functions as a stimulant. Users report reduced fatigue, sharper focus, and a mild boost in motivation. This is the traditional use pattern in Southeast Asia, where laborers have chewed kratom leaves or brewed tea for centuries to sustain energy during physical work. Low doses generally don’t interfere with ordinary activities and are more comparable to a strong cup of coffee than to the sedating effects people associate with opioids.
White vein kratom is the variety most commonly chosen for this purpose, as it tends to produce the strongest stimulating and focus-enhancing effects. Green vein kratom also provides mild energy without the jitteriness that some users experience with white strains.
Opioid Withdrawal Support
Kratom tea has gained significant attention as a self-treatment for opioid withdrawal. Because its alkaloids activate the same receptors as prescription opioids, it can ease withdrawal symptoms like muscle aches, nausea, anxiety, and insomnia. People often turn to kratom because it’s more accessible than medications prescribed for withdrawal management.
This is also where the picture gets complicated. The Mayo Clinic notes that while people use kratom to ease withdrawal, evidence suggests that kratom itself can lead to dependence and its own withdrawal symptoms. The same receptor activity that makes it helpful for managing opioid cravings means regular use can create a new pattern of physical dependence. Red vein varieties are most commonly used for this purpose due to their stronger sedating and pain-relieving profile.
How to Brew for Maximum Effect
Kratom’s main alkaloid, mitragynine, dissolves much more readily in slightly acidic water. Adding lemon juice to your brew before steeping can increase alkaloid extraction by 30 to 40% compared to plain water. The citric acid converts the alkaloids into a more water-soluble form, so this isn’t just about flavor.
The ideal water temperature is between 175°F and 195°F (80 to 90°C), which is hot enough to pull alkaloids from the powder efficiently without degrading them. A full rolling boil works but isn’t necessary and tends to make the tea taste harsher. Simmering just below boiling for 15 to 20 minutes is the preferred method. For best results, stir in lemon juice before adding hot water to pre-acidify the mixture, targeting a pH between 4 and 5.
Red, White, and Green Strains
- Red vein: The most calming and sedating option. Typically chosen for pain relief, relaxation, sleep support, and opioid withdrawal. The strongest analgesic profile of the three.
- White vein: The most stimulating. Users choose it for energy, focus, mood elevation, and euphoria. The least sedating option and the closest to a traditional stimulant experience.
- Green vein: A middle ground between red and white. Mildly energizing with a balanced effect profile. Often blended with red or white strains to smooth out extremes in either direction.
Risks and Side Effects
The FDA has warned consumers against using kratom, citing risks of liver toxicity, seizures, and substance use disorder. Kratom is not approved as a drug, dietary supplement, or food additive in the United States. The agency has also flagged contamination issues, with some kratom products testing positive for Salmonella or concerning levels of heavy metals.
Regular use can lead to physical dependence. The alkaloid 7-hydroxymitragynine, in particular, produces tolerance and withdrawal patterns similar to those seen with morphine. Withdrawal symptoms from kratom itself can include muscle aches, irritability, insomnia, and nausea. Higher doses (above 20 grams of raw leaf) amplify both the euphoric effects and the risk of adverse reactions.
Kratom’s legal status varies by state. It remains legal in most of the U.S. but is banned or restricted in several states and municipalities. The FDA has partnered with U.S. Customs and Border Protection to limit imports of unlawful kratom products, and some shipments enter the country mislabeled as potpourri or incense.
