Nail lifting, known medically as onycholysis, is the separation of your nail plate from the nail bed underneath. It can affect fingernails or toenails, and it usually starts at the tip of the nail and works its way back toward the cuticle. The separated portion typically looks white, yellow, or opaque compared to the healthy pink nail still attached. It’s painless in most cases, but the gap it creates can trap moisture, debris, and bacteria, turning a cosmetic nuisance into something that needs real attention.
Why Nails Lift in the First Place
The causes of nail lifting differ quite a bit depending on whether it’s happening on your fingers or your toes. On fingernails, the most common culprit is physical trauma. That includes obvious injuries like slamming your finger in a door, but also repetitive, low-grade damage: aggressive manicuring, picking or cleaning under your nails with sharp tools, or daily work that puts pressure on the nail tips. People who frequently expose their hands to water, detergents, or chemical solvents are also at higher risk because moisture weakens the bond between the nail plate and the bed.
Toenail lifting has a very different story. It’s primarily mechanical, caused by pressure from shoes pushing against the toes during walking. People with flat feet or an uneven gait tend to get more pressure on certain toes, making them especially vulnerable. Tight or narrow shoes accelerate the problem.
Beyond trauma and pressure, several medical conditions cause nail lifting as a secondary symptom. Psoriasis is the most common disease behind fingernail onycholysis. In a study of psoriasis patients, over 71% had some form of nail involvement, and onycholysis specifically showed up in nearly 63% of those with affected nails. Fungal nail infections are another frequent cause, and sometimes fungus moves in after lifting has already started from another trigger. Certain medications that increase sun sensitivity can also cause nail separation through a phototoxic reaction.
The Thyroid Connection
Nail lifting can sometimes signal a problem happening elsewhere in your body. Thyroid disease, particularly hyperthyroidism, has a well-documented link to onycholysis. When it appears in this context, it’s sometimes called Plummer’s nails. The exact mechanism isn’t fully understood, but researchers have noted that unexplained nail lifting, especially when there’s no obvious trauma or skin condition, can be an early indicator of thyroid dysfunction. Both overactive and underactive thyroid conditions have been associated with it, though hyperthyroidism is the stronger link.
If your nails are lifting and you can’t point to an obvious cause like an injury, tight shoes, or a skin condition, it’s worth having your thyroid levels checked. Nail changes can appear before other thyroid symptoms become noticeable.
What It Looks and Feels Like
The first thing most people notice is a change in color. The detached part of the nail loses its normal pink hue because it’s no longer in contact with the blood-rich nail bed. It may appear white, yellowish, or greenish depending on what’s growing in the gap. The nail might feel slightly raised or hollow when you press on it. In many cases there’s no pain at all, which is why some people don’t notice it until the separation has progressed.
Underneath the lifted nail, a buildup of dead skin cells (called subungual hyperkeratosis) can develop, thickening the space between the nail and the bed. This creates a visible ridge and makes the nail sit higher than normal. If the lifted area gets wet frequently or stays damp, you may also notice an unpleasant odor.
Infections That Move In
The space under a lifted nail is warm, dark, and easily traps moisture, making it an ideal environment for bacteria and fungi. The most common bacterial infection in this scenario is green nail syndrome, caused by Pseudomonas aeruginosa. It’s exactly what it sounds like: the nail turns a distinct green or greenish-black color. Working in wet conditions, having diabetes, or already having a fungal infection all raise the risk.
Fungal and bacterial infections frequently overlap. In documented cases of green nail syndrome, patients often had a yeast coinfection alongside the Pseudomonas bacteria. This means treatment sometimes needs to address both organisms, not just one. A green, yellow-green, or dark discoloration under a lifted nail is a strong signal that infection has set in and home care alone won’t resolve it.
How to Care for a Lifted Nail at Home
The most important principle is protecting the exposed nail bed from further damage and keeping the area dry. Here’s what that looks like in practice:
- Keep it short. Clip the detached nail back as far as you comfortably can so it doesn’t catch on things and pull further away. Repeat this every two weeks as the nail grows out.
- Minimize moisture. Dry the exposed nail bed thoroughly after every hand wash. Wear cotton gloves under vinyl gloves when doing dishes or wet work. Avoid soaking your hands for prolonged periods.
- Clean gently. Wash under the nail with a clean washcloth rather than sharp tools like cuticle sticks, which can break the skin underneath and introduce infection.
- Use antimicrobial soaks. Diluted vinegar, hydrogen peroxide, or tea tree oil soaks can help prevent bacteria and fungi from colonizing the gap.
- Skip the cosmetics. Avoid nail polish, artificial nails, and gel manicures on the affected nail. Experts recommend staying away from nail cosmetics until at least three months after the onycholysis has fully resolved.
- Protect your hands. Wear gloves during sports, yard work, or any activity that could further traumatize the nail.
Avoid the temptation to aggressively clean or scrape underneath the lifted portion. This is one of the most common mistakes people make, and it often worsens the separation or causes a secondary infection.
How Long Regrowth Takes
Once the underlying cause is addressed, the nail won’t reattach to the bed. Instead, you’ll need to wait for entirely new nail growth to replace the separated portion. How long that takes depends on which nail is affected.
Fingernails grow at an average rate of about 3.5 millimeters per month. A fully separated fingernail takes roughly four to six months to grow out completely. Toenails are much slower, averaging only about 1.6 millimeters per month, which means a toenail can take 12 to 18 months to fully replace itself. The little fingernail grows the slowest of all fingernails, while the big toenail grows the fastest among toenails. Younger people and men tend to have slightly faster growth rates, though the differences aren’t dramatic.
This timeline assumes you’ve stopped whatever was causing the problem. If the trigger persists, whether it’s ill-fitting shoes, ongoing chemical exposure, or an untreated condition like psoriasis, the new nail may lift again as it grows in.
When There’s No Obvious Cause
Sometimes nail lifting shows up without any clear trigger: no injury, no skin disease, no chemical exposure. This is called idiopathic onycholysis, and it can be stubborn. In these cases, the same home care principles apply, but treatment may also include topical medications prescribed by a dermatologist to help the new nail grow in attached.
General measures for idiopathic cases include minimizing all nail trauma, avoiding contact irritants and allergens, and using finger cots or small protective coverings on the affected fingertips during daily tasks like cooking. The combination of meticulous nail care and removing even subtle sources of irritation resolves most cases over time, though patience is essential given how slowly nails grow.
