What Is Non-HDL Cholesterol in a Blood Test?

Non-HDL cholesterol is a single number on your blood test that captures all the cholesterol types that can build up in your artery walls. You get it by subtracting your HDL (“good”) cholesterol from your total cholesterol. What’s left is everything that contributes to plaque: LDL cholesterol, triglyceride-rich particles, and other harmful lipoproteins bundled into one figure. For most adults, a non-HDL level below 130 mg/dL is considered desirable, though targets drop lower if you’re at higher risk for heart disease.

How Non-HDL Cholesterol Is Calculated

The math is simple. Take your total cholesterol number and subtract your HDL cholesterol. If your total cholesterol is 210 mg/dL and your HDL is 55 mg/dL, your non-HDL cholesterol is 155 mg/dL. No special test is needed. Your lab or doctor calculates it from the standard lipid panel you already get.

What makes this number useful is what it captures. LDL cholesterol, the one most people focus on, only reflects one type of harmful particle. Non-HDL also includes cholesterol carried by triglyceride-rich lipoproteins (sometimes called “remnant” cholesterol) and a particle called lipoprotein(a). All of these carry a protein called apolipoprotein B on their surface, which is essentially the ticket that lets them burrow into artery walls and start forming plaque. By measuring non-HDL, you’re getting a broader count of every cholesterol-carrying particle that can damage your arteries.

Why It Can Matter More Than LDL

LDL cholesterol has been the primary target in cholesterol management for decades, and it still matters. But non-HDL tells a more complete story. A large study published in Circulation tracked people from childhood into adulthood and found that non-HDL cholesterol was a better predictor of heart attacks and cardiovascular deaths than LDL alone. The difference was most striking in people whose LDL looked normal but whose non-HDL was elevated. That group had roughly double the risk of a cardiovascular event compared to people whose both numbers were normal.

This happens because some people carry a large amount of cholesterol in triglyceride-rich particles that LDL testing doesn’t fully account for. Their LDL number looks fine, but the total burden of artery-damaging cholesterol is actually high. Non-HDL catches this discrepancy. Research in the Journal of the American College of Cardiology confirmed that even among people already taking statins, those with high non-HDL or apolipoprotein B levels remained at elevated risk for heart events, even when their LDL was well controlled. In other words, LDL alone can give you a false sense of security.

This is also why the American Heart Association and American College of Cardiology now recommend non-HDL as a reasonable screening target alongside LDL.

Non-HDL Targets by Risk Level

Your target depends on your overall cardiovascular risk. The 2026 ACC/AHA cholesterol guidelines lay out three tiers:

  • General population: A non-HDL below 130 mg/dL is typically considered desirable.
  • High risk (10% or greater 10-year risk of cardiovascular disease): Non-HDL below 100 mg/dL (2.6 mmol/L).
  • Very high risk (existing heart disease at very high risk): Non-HDL below 85 mg/dL (2.2 mmol/L).

To put that in perspective, if you’ve already had a heart attack or have significant plaque buildup, your doctor is aiming for a non-HDL that’s roughly 30 mg/dL above your LDL target. That 30-point gap accounts for the cholesterol sitting in those triglyceride-rich particles that LDL testing misses.

Do You Need to Fast for This Test?

One practical advantage of non-HDL cholesterol is that it’s reliable even without fasting. LDL cholesterol is usually calculated using a formula that depends on an accurate triglyceride measurement, and triglycerides spike after eating. That’s why many labs ask you to fast 8 to 12 hours before a lipid panel. Non-HDL, however, only requires total cholesterol and HDL, both of which stay relatively stable after meals. If your doctor orders a non-fasting lipid panel, your non-HDL number is still trustworthy.

That said, the CDC still notes that some providers request fasting for a complete lipid panel. If you’re unsure, ask before your appointment.

What Drives Non-HDL Cholesterol Up

The same factors that raise LDL tend to raise non-HDL: diets high in saturated fat, excess body weight, lack of physical activity, smoking, and genetic predisposition. But non-HDL can also climb when triglycerides are elevated, which is common in people with insulin resistance, type 2 diabetes, or metabolic syndrome. In these cases, the liver produces more triglyceride-rich particles, and each one carries cholesterol that contributes to the non-HDL total. This is exactly the scenario where LDL looks deceptively normal but non-HDL reveals the true risk.

How to Lower Non-HDL Cholesterol

Because non-HDL reflects both LDL and triglyceride-rich particles, lowering it often requires tackling both. Lifestyle changes are the first line of defense, and they work on multiple fronts at once.

Exercise has a consistent effect. Working up to at least 30 minutes of moderate activity like brisk walking five days a week, or 25 minutes of vigorous activity like running three days a week, lowers triglycerides and can improve the overall balance of cholesterol particles. Dietary changes matter too: replacing saturated fats with unsaturated fats, increasing soluble fiber from foods like oats, beans, and fruits, and cutting back on refined carbohydrates and alcohol all help bring triglycerides and LDL down together.

Losing even a modest amount of weight, if you’re carrying extra, tends to lower triglyceride-rich particles disproportionately, which can improve non-HDL even when LDL doesn’t change much. For people who need medication, statins lower both LDL and non-HDL. In very high-risk patients whose numbers remain elevated on statins, additional medications may be added to push non-HDL below 85 mg/dL.

Reading Your Results in Context

When you get your lipid panel back, look at non-HDL alongside LDL rather than as a replacement. If the two numbers tell the same story (both normal or both high), your treatment path is straightforward. The more interesting scenario is when they disagree. If your LDL is under 100 mg/dL but your non-HDL is 145 mg/dL, that gap signals a significant amount of cholesterol riding in triglyceride-rich particles. This pattern is common in people with high triglycerides, and it carries real cardiovascular risk that LDL alone would miss.

Non-HDL cholesterol isn’t a new or exotic test. It’s sitting on your standard lipid panel right now, either reported directly or easy to calculate yourself. It just happens to be one of the most useful numbers on the page.
