Omega-3 fish oil supports heart health, brain function, mood, and joint comfort, with the strongest evidence behind its ability to lower triglycerides and reduce cardiovascular risk. The two active components, EPA and DHA, each play slightly different roles in the body, which is why certain benefits depend on getting the right balance of the two.
Heart Health and Triglyceride Reduction
The cardiovascular benefits of fish oil are the most studied and most robust. At prescription-strength doses (4 grams per day), omega-3s reduce triglycerides by 20% to 30% in people with elevated levels. For people with very high triglycerides (above 500 mg/dL), reductions can exceed 30%. This matters because persistently high triglycerides are a major driver of heart disease and stroke.
The landmark REDUCE-IT trial showed that high-dose EPA reduced major cardiovascular events by 25% in high-risk patients already taking statins. That included a 20% reduction in cardiovascular death specifically. These weren’t marginal improvements; they represent meaningful protection on top of standard treatment.
For people who already have coronary heart disease, the American Heart Association recommends about 1 gram per day of combined EPA and DHA, preferably from fatty fish like salmon, mackerel, or sardines. The AHA does not recommend omega-3 supplements for people without elevated cardiovascular risk, though eating fish twice a week remains part of their general dietary guidance.
Brain Health and Cognitive Protection
DHA is a structural building block of brain cell membranes, and higher blood levels of both DHA and EPA are linked to larger total brain and hippocampal volumes over time. The hippocampus is the brain region most involved in memory, and it’s one of the first areas to shrink during cognitive decline and Alzheimer’s disease.
An interesting finding from clinical trials is that omega-3s and B vitamins appear to work as a team. In one controlled trial, omega-3 fatty acids slowed brain shrinkage only in people who also had adequate B vitamin levels, and B vitamins only slowed brain atrophy in people with high omega-3 levels. Neither worked as well alone. This suggests that if you’re supplementing with fish oil for cognitive reasons, making sure you’re getting enough B vitamins (from diet or a multivitamin) could be important.
Mood and Depression
Multiple meta-analyses suggest omega-3s can improve symptoms of major depression, though the size of the benefit varies across studies. The most effective formulations appear to contain at least 60% EPA relative to DHA. This is a useful detail if you’re comparing supplement labels, since many standard fish oil capsules contain more DHA than EPA, or a 50/50 split.
Clinical trials have tested doses ranging from 0.5 grams to as high as 10 grams per day. A practical range supported by clinical experience is 1 to 2 grams per day of combined EPA and DHA, with EPA as the dominant fatty acid. Fish oil is not a replacement for antidepressant therapy in moderate to severe cases, but it may offer a meaningful complement.
Joint Pain and Inflammation
Omega-3s are natural anti-inflammatory compounds. They reduce the production of molecules that drive swelling and pain, which is why fish oil has shown real benefits for people with rheumatoid arthritis. A 2021 analysis of 70 studies found that fish oil significantly reduced disease activity, pain, and morning stiffness in RA patients. At higher doses, some patients were able to stop taking NSAIDs entirely, with reductions in disease activity lasting nearly eight months.
For general joint stiffness or exercise-related soreness, the evidence is less dramatic but still promising. Omega-3s won’t replace pain medication for severe inflammatory conditions, but they can meaningfully reduce the amount of medication some people need.
Eye Health
DHA is highly concentrated in the retina, and adequate intake is important for maintaining retinal cell membranes. Fish oil has been widely promoted for dry eye relief, but the evidence is mixed. A systematic review of omega-3 eyedrops found no clear advantage over controls for improving actual tear production. Some people do report subjective improvements in comfort, but the clinical data on measurable tear film changes has been inconsistent.
Plant-Based Omega-3s vs. Fish Oil
Flaxseed, chia seeds, and walnuts contain ALA, a plant-based omega-3. Your body can convert ALA into EPA and DHA, but the conversion rate is poor: roughly 5% to 8% for EPA and as low as 0.5% to 5% for DHA. That means you’d need to eat a very large amount of flaxseed to match what a single fish oil capsule provides.
If you follow a plant-based diet and want the benefits described above, algae-based supplements are a more reliable option. Algae is where fish get their omega-3s in the first place, and algal oil supplements provide preformed DHA and EPA without the conversion bottleneck.
How Much to Take
There’s no single official recommendation for EPA and DHA intake in healthy adults. The FDA allows supplement labels to note that up to 2 grams per day of EPA and DHA may reduce coronary heart disease risk. Beyond that, dosing depends on what you’re trying to accomplish:
- General wellness: Two servings of fatty fish per week, or roughly 500 mg of combined EPA and DHA daily from a supplement.
- Existing heart disease: About 1 gram per day of EPA plus DHA, per American Heart Association guidance.
- High triglycerides: 4 grams per day of prescription omega-3s, under medical supervision.
- Mood support: 1 to 2 grams per day, with at least 60% EPA.
When reading supplement labels, check the EPA and DHA content per serving, not just the total “fish oil” amount. A 1,000 mg fish oil capsule often contains only 300 mg of actual EPA and DHA combined. The rest is other fats.
Safety and Blood Thinning
Fish oil is well tolerated by most people at standard doses. The most common side effects are fishy burps, mild nausea, or loose stools, all of which tend to improve if you take capsules with food or switch to enteric-coated versions.
The more important concern is bleeding risk. Omega-3s have a mild blood-thinning effect, which is part of how they protect the heart. But if you’re taking blood-thinning medications like warfarin, combining them with fish oil can, in rare cases, increase bleeding risk. The same caution applies to aspirin or other antiplatelet drugs. If you’re on any blood thinner, this is a conversation to have with your prescriber before starting high-dose fish oil.
