What Is Omeprazole Used for in Dogs: Uses & Side Effects

Omeprazole is a stomach acid reducer used in dogs primarily to treat and prevent gastric ulcers. It’s the most widely used proton pump inhibitor (PPI) in veterinary medicine, and it works by shutting down the acid-producing pumps in the stomach lining. Vets also prescribe it for acid reflux, erosive stomach conditions, and as part of combination therapy against a specific stomach bacterium called Helicobacter.

Primary Uses in Dogs

The core job of omeprazole is reducing stomach acid, which makes it useful across several conditions. The most common reason a vet prescribes it is gastric or duodenal ulcers, which are open sores in the stomach or upper intestine. These ulcers can develop from prolonged stress (which raises the body’s natural cortisol levels), from certain medications like NSAIDs or corticosteroids, or from dietary issues.

Beyond ulcers, omeprazole is used for:

  • Acid reflux (GERD): Dogs with chronic acid reflux benefit from the sustained acid suppression omeprazole provides.
  • Helicobacter infection: Omeprazole is commonly part of a “triple therapy” regimen, combined with two antibiotics, to eliminate this ulcer-causing stomach bacterium.
  • Erosive gastritis: Inflammation and erosion of the stomach lining, often triggered by medications or illness, responds well to acid suppression.
  • Reducing cerebrospinal fluid pressure: In dogs with hydrocephalus or syringomyelia, omeprazole may be used specifically because one of its effects is lowering fluid pressure in and around the brain.

Why Vets Choose It Over Other Acid Reducers

Older acid-reducing drugs like famotidine work by blocking histamine receptors in the stomach. Omeprazole takes a different approach: it permanently disables the proton pumps that actually produce acid, which means it suppresses acid output more completely and for a longer duration. Clinical evidence shows that these older antihistamine-type acid blockers are inferior to proton pump inhibitors like omeprazole for treating stomach ulcers and erosions. This stronger suppression is why omeprazole has become the standard choice for serious gastric conditions in dogs.

How It’s Given

The standard oral dose for dogs is 0.5 to 1 mg/kg, given once daily. Most dogs receive human over-the-counter formulations, since dedicated veterinary versions for dogs aren’t widely available. The capsules or tablets are enteric-coated, meaning they’re designed to survive stomach acid and dissolve in the intestine. This coating matters: crushing the medication or opening capsules can destroy the drug before it reaches where it needs to go.

Omeprazole works best when given on an empty stomach, ideally 30 minutes or so before a meal. It takes a few days of consistent dosing to reach its full acid-suppressing effect, which is why vets sometimes start with a higher initial dose for urgent cases.

Potential Side Effects

Most dogs tolerate omeprazole well, but side effects do occur. In a study from the Estonian University of Life Sciences analyzing adverse reactions in 30 dogs, vomiting was by far the most common issue, affecting two-thirds of the patients who experienced side effects. Other reported reactions included diarrhea, reduced appetite or complete loss of appetite, abdominal pain, lethargy, and restlessness.

It’s also worth knowing that drug sensitivities can develop over time. Your dog may tolerate omeprazole fine for weeks and then begin showing signs of a reaction. Watch for changes in appetite, energy level, or stool consistency throughout treatment, not just at the beginning.

Longer-term use carries additional concerns. Sustained acid suppression can lead to elevated levels of gastrin (a hormone that stimulates acid production), changes in the bacterial balance of the small intestine, and potentially the formation of certain harmful compounds in the stomach. These risks become more relevant when a dog has been on omeprazole for months rather than weeks.

Drug Interactions to Know About

Omeprazole can interfere with other medications in two ways. First, it inhibits certain liver enzymes that break down drugs, which can cause some medications to build up to higher-than-expected levels. This is particularly relevant for sedatives like diazepam and midazolam, as well as the blood thinner warfarin.

Second, because omeprazole reduces stomach acid, it can prevent certain drugs from being absorbed properly. Antifungal medications like ketoconazole and itraconazole need an acidic stomach environment to dissolve, so taking them alongside omeprazole can make them less effective. The same applies to iron supplements. If your dog takes any of these medications, your vet will need to adjust timing or choose alternatives.

Stopping Omeprazole Safely

One of the more important things to understand about omeprazole is that you shouldn’t stop it abruptly after extended use. When the stomach’s acid pumps have been suppressed for four weeks or longer, suddenly removing the drug can trigger rebound acid hypersecretion, where the stomach produces even more acid than it did before treatment. This can worsen the very symptoms you were treating.

The recommended approach is a gradual taper: reducing the dose by about 50% per week. If your dog was on twice-daily dosing, the first step is typically dropping the evening dose. This stepdown process gives the stomach time to readjust its acid production to normal levels without overcorrecting.
