Stomatitis in cats is a severe, chronic inflammation of the mouth that goes far beyond ordinary gum disease. While regular gingivitis affects the gum line near the teeth, stomatitis causes deep, painful inflammation across the back of the mouth, the throat, and sometimes the inner cheeks and tongue. It affects roughly 0.7% of cats seen in general veterinary practice, though some estimates suggest 3 to 5% of cats show signs of the disease at some point in their lives. It’s one of the most painful oral conditions cats can develop, and it often requires aggressive treatment.
How Stomatitis Differs From Gingivitis
Most cats develop some degree of gum inflammation as they age, especially along the tooth line. Stomatitis is a different condition entirely. The inflammation extends well beyond the gums into the tissues at the back of the mouth (called the caudal oral mucosa), the area around the base of the tongue, and sometimes the roof of the mouth. The tissue becomes raw, ulcerated, and intensely red. In many cats, the inflammation is so widespread that simply opening the mouth during a veterinary exam reveals angry, bleeding tissue across large areas of the oral cavity.
Veterinarians often refer to the full condition as feline chronic gingivostomatitis (FCGS) to distinguish it from milder gum disease. Dental X-rays play an essential role in diagnosis because many affected cats also have significant bone loss and tooth root damage that isn’t visible on the surface.
What Causes It
Stomatitis appears to be the result of a cat’s immune system overreacting to bacteria and other irritants in the mouth. Instead of mounting a normal, proportional response to dental plaque, the immune system launches an intense, sustained attack that damages the cat’s own oral tissues. Biopsies of affected tissue show dense infiltration of immune cells, particularly a type that targets cells infected by viruses. This suggests the immune system is responding as though it’s fighting a chronic viral infection in the mouth.
Feline calicivirus (FCV) is strongly linked to the condition. Studies have found that nearly 100% of cats with chronic stomatitis test positive for calicivirus, compared to much lower rates in the general cat population. The virus appears to act as a persistent trigger, keeping the immune system in a constant state of high alert. Feline immunodeficiency virus (FIV) also shows up more frequently in affected cats, with about 13.5% testing positive in one study of 104 cases, though FIV alone doesn’t cause stomatitis.
The exact reason some cats develop this exaggerated immune response while others don’t remains unclear. Genetics likely plays a role, and cats with underlying immune suppression or chronic viral infections seem more vulnerable.
Signs Your Cat May Have Stomatitis
Pain dominates the picture. Cats with stomatitis paw at their faces, drool excessively (sometimes with blood in the saliva), and refuse food they previously loved. Some cats scream or cry out when they yawn because stretching the mouth triggers intense pain. You may notice your cat approaching the food bowl with obvious hunger, then backing away after attempting to eat.
Other common signs include:
- Weight loss from reduced eating over weeks or months
- Matted or unkempt fur because grooming becomes too painful
- Bad breath that’s noticeably worse than typical “cat breath”
- Difficulty swallowing, with food dropping from the mouth
- Hiding or withdrawing from interaction
Because cats are experts at masking pain, stomatitis is sometimes quite advanced before owners notice anything wrong. A cat that gradually eats less, loses weight, and becomes quieter may have been suffering for months.
Why Tooth Extraction Is the Primary Treatment
This is the part that surprises most cat owners: the single most effective treatment for stomatitis is removing most or all of the teeth. The logic is straightforward. If the immune system is overreacting to bacteria that colonize tooth surfaces, removing the teeth eliminates the surface where those bacteria thrive. It sounds drastic, but it works.
In a study of 95 cats that underwent tooth extractions, 28.4% experienced complete resolution of their stomatitis and 39% had substantial improvement. That means about two-thirds of cats got significantly better or were cured. Of those cats that improved, roughly 69% still needed medication for a limited period after surgery before reaching their best outcome, but they were eventually able to stop treatment.
Whether the vet removes only the premolars and molars (the teeth closest to the inflamed tissue) or performs a full-mouth extraction doesn’t appear to make a significant difference in outcomes. Many vets start by extracting the teeth in the areas of worst inflammation and monitor the response before deciding whether additional teeth need to come out.
Cats without teeth adapt remarkably well. After a recovery period of soft food for 7 to 10 days, most cats return to eating enthusiastically. Many can even eat moistened kibble once fully healed. The relief from chronic oral pain typically outweighs any difficulty with chewing.
When Extractions Don’t Work
About one-third of cats don’t respond well to tooth extraction. In the same 95-cat study, 26.3% showed little improvement and continued to need ongoing pain medication, anti-inflammatory drugs, or antibiotics. Another 6.3% had no improvement at all, with their condition considered refractory.
For these cats, immune-suppressing medication becomes the next line of defense. Cyclosporine, a drug that dials down the overactive immune response, has shown promising results in cats that didn’t fully respond to extractions. In a controlled study, about 78% of cats receiving cyclosporine showed more than 40% improvement in their oral inflammation scores over six weeks, compared to just 14% of cats receiving a placebo. The drug needs to reach adequate levels in the bloodstream to be effective, and cats with higher blood levels showed significantly better results (72% improvement vs. 28%).
Corticosteroids can also reduce inflammation, but long-term use carries serious side effects including diabetes and immune suppression, so most vets try to avoid relying on them indefinitely.
Stem Cell Therapy as a Newer Option
For cats that fail both surgery and standard medications, stem cell therapy has emerged as an option at some specialty practices. The treatment involves intravenous infusion of stem cells derived from fat tissue, given in two sessions about 30 days apart. In a long-term study, 65.5% of treated cats showed a positive response, and 58.6% experienced permanent improvement or cure. About 24% of cats showed no improvement at all, and 17% improved temporarily before relapsing. This therapy isn’t widely available and tends to be expensive, but it offers another path for cats with otherwise unmanageable disease.
Living With a Cat That Has Stomatitis
If your cat has been diagnosed, the priority is pain control. Cats in chronic oral pain eat less, lose weight, and become withdrawn. Even while waiting for surgery or during the weeks of post-operative healing, your vet can prescribe pain relief to keep your cat comfortable and eating.
After tooth extraction, offer small, frequent meals of canned or moistened food during the first week to 10 days. Most cats start eating with visible enthusiasm within a few days of surgery, simply because the source of their pain is gone. Long-term, cats without teeth do well on soft canned food, and many owners report that their cats seem years younger once the chronic inflammation resolves.
Regular follow-up exams are important even after successful treatment. Some cats experience flare-ups months or years later, and catching recurrence early makes it easier to manage. Cats that required extended medical management after surgery should be monitored for signs of returning inflammation, such as renewed drooling, reluctance to eat, or facial pawing.
