What Is the Back of the Brain Called? Names & Functions

The back of the brain is called the occipital lobe. This region sits at the very rear of the cerebrum and is almost entirely dedicated to processing vision. But depending on how broadly you define “back of the brain,” two other structures also live there: the cerebellum, tucked underneath the occipital lobe, and the brainstem, which connects the brain to the spinal cord at the lowest rear portion of the skull.

The Occipital Lobe: Your Visual Processing Center

The occipital lobe is the rearmost section of the cerebrum, the large wrinkled mass that makes up most of your brain. Its front border runs along a line roughly 5 centimeters from the back edge of the brain on both the top and bottom of each hemisphere. Everything behind that line belongs to the occipital lobe.

Nearly everything the occipital lobe does relates to sight. It contains the primary visual cortex, which receives raw signals from your eyes and begins assembling them into the images you perceive. Different areas within the lobe handle different aspects of vision: some process color, others detect motion, and others help you recognize shapes and spatial relationships. This specialization is why damage to one part of the occipital lobe can knock out a very specific visual ability while leaving others intact.

What Happens When the Occipital Lobe Is Injured

Because the occipital lobe is so tightly focused on vision, injuries here produce distinctly visual symptoms. Depending on where the damage occurs, a person might experience partial or total vision loss, difficulty identifying colors, an inability to recognize that an object is moving, or trouble reading and writing. Some people develop visual hallucinations or illusions, seeing things that aren’t there or misperceiving objects that are. Others lose the ability to recognize drawn objects or to locate items in their environment, even when their eyes are physically healthy.

These deficits can be confusing because the eyes themselves still work. The problem is upstream, in the brain tissue that interprets what the eyes send. A standard eye exam may come back normal while the person genuinely cannot process what they’re seeing.

The Cerebellum: Coordination and Balance

Sitting just below and behind the occipital lobe, the cerebellum is a densely packed structure sometimes called the “little brain.” It’s positioned at the bottom of the brain near the back of your head, right above where the spinal cord connects. Despite being much smaller than the cerebrum, the cerebellum contains an enormous number of neurons, reflecting just how much computational work it handles.

The cerebellum’s primary job is coordinating movement. It doesn’t initiate actions the way the motor cortex does. Instead, it fine-tunes them, adjusting the timing and force of different muscle groups so that your movements come out smooth rather than jerky. It also plays a central role in balance. Using input from your inner ear and from sensors in your muscles and joints, the cerebellum constantly makes tiny postural corrections to keep you upright.

When the cerebellum is damaged, people develop a condition called ataxia. This looks like clumsiness: difficulty walking in a straight line, trouble reaching for objects without overshooting, and problems with tasks that require fine hand coordination. Early researchers studying cerebellar injuries noticed that patients could still move their limbs, but those movements were imprecise, as if the internal calibration system had been thrown off.

The Brainstem: Vital Functions at the Base

The brainstem occupies the lowest portion of the brain, connecting it to the spinal cord. It includes three sections stacked on top of one another: the midbrain at the top, the pons in the middle, and the medulla at the bottom. While technically more “bottom” than “back,” the brainstem sits at the rear base of the skull and is often grouped with posterior brain structures.

This small region controls functions you never have to think about. It regulates your heart rate, blood pressure, and breathing. It manages reflexes like swallowing, gagging, coughing, and sneezing. It adjusts your pupil size when lighting changes and stabilizes your eyes when you move your head. Ten of the brain’s twelve cranial nerves originate here, carrying signals that control movement, sensation, taste, and hearing across your head and face. Damage to the brainstem is life-threatening precisely because it governs so many automatic survival functions.

Anatomical Terms for “Back of the Brain”

In medical and scientific contexts, the back of the brain is described using directional terms. “Posterior” means toward the back, so a doctor might refer to the “posterior fossa” (the bony compartment at the back of the skull that houses the cerebellum and brainstem) or “posterior cortex” when discussing the occipital region. “Caudal” is another term meaning toward the back of the brain or the bottom of the spinal cord. You’ll see both terms in imaging reports or neurology notes, and they simply describe location relative to the front of the brain, which is called “anterior” or “rostral.”

Occipital Neuralgia: Pain at the Back of the Head

One common reason people search about the back of the brain is pain in that area. Occipital neuralgia is a condition involving the occipital nerves, which run from the upper spine through the scalp at the back of the head. It causes sharp, electric-shock-like pain, burning, or throbbing that typically starts at the base of the skull and can radiate behind one eye.

The most common trigger is pinched nerves or tight muscles in the neck. Head and neck injuries can also set it off. People with degenerative disk disease, osteoarthritis of the upper spine, diabetes, or gout may be more susceptible. The pain can mimic a migraine, but it’s distinct in how it follows the path of the occipital nerves from the back of the skull upward. If you’re experiencing recurring sharp pain at the back of your head, this is one of the more likely explanations.
