The best medications for restless legs syndrome (RLS) are gabapentin, gabapentin enacarbil, and pregabalin. These are the only drugs that received a “strong for” recommendation in the most recent clinical guidelines from the American Academy of Sleep Medicine, alongside intravenous iron infusion for people with low iron stores. This marks a significant shift from earlier years, when dopamine-boosting drugs like ropinirole and pramipexole were the go-to prescriptions. Those older medications now carry a “conditional against” recommendation due to serious long-term risks.
Why Gabapentin Is Now the Top Choice
Gabapentin and its relatives (pregabalin and gabapentin enacarbil) work by calming overactive nerve signals rather than targeting dopamine. They reduce the crawling, pulling, and tingling sensations in the legs that make it impossible to sit still or fall asleep. Gabapentin enacarbil, a formulation designed specifically for RLS, is typically taken as a single 600 mg dose around 5 PM so that it’s active during the evening and nighttime hours when symptoms peak.
Standard gabapentin and pregabalin are also effective but may require dose adjustments to find the right level. Common side effects include drowsiness, dizziness, and some weight gain. For many people, the drowsiness actually helps because RLS primarily disrupts sleep. These medications don’t carry the tolerance and dependency risks that plagued earlier treatments, which is the main reason guidelines now favor them.
Why Dopamine Drugs Fell Out of Favor
For years, dopamine agonists like ropinirole, pramipexole, and rotigotine patches were prescribed as first-line RLS treatment. They do reduce symptoms initially, but long-term use creates a problem called augmentation: the medication itself makes RLS worse over time. Symptoms start appearing earlier in the day, spread to the arms, and become more intense. Studies show augmentation rates between 4% and 42% with pramipexole and up to 40% with ropinirole, depending on the study. Levodopa is even worse, with augmentation rates as high as 73%.
Beyond augmentation, dopamine agonists can cause compulsive behaviors, including gambling, binge eating, and compulsive shopping. Patients build tolerance and need higher doses, which amplifies these side effects. Coming off these drugs often requires medical supervision because of extreme withdrawal symptoms. The brain becomes dependent on the external dopamine source, and mental health can deteriorate significantly during the transition. For all these reasons, the current guidelines recommend against starting dopamine agonists for RLS.
If you’re currently taking a dopamine agonist and doing well, that doesn’t necessarily mean you need to switch immediately. But it’s worth discussing a transition plan with your doctor, especially if your symptoms have gradually worsened or shifted to earlier in the day.
Iron: The Treatment Many People Miss
Low iron in the brain plays a central role in RLS, and correcting it can reduce or even eliminate symptoms without any other medication. The key number is your serum ferritin level. If it’s 75 or below, oral iron supplements are recommended as a starting point. This threshold is much higher than what’s considered “anemic,” so your iron levels can look normal on a standard blood test and still be too low for your brain’s dopamine system to function properly.
Oral iron (ferrous sulfate) is the simplest option, but it comes with downsides: stomach upset, constipation, and slow absorption that can take weeks or months to raise your levels. Taking it every other day on an empty stomach with vitamin C improves absorption. If you can’t tolerate oral iron, have absorption issues, or need faster relief, intravenous iron is an alternative. IV iron replenishes stores in one or two infusions rather than months of pills. One specific IV formulation, ferric carboxymaltose, earned a “strong for” recommendation in the latest guidelines based on its demonstrated improvement in both symptom severity and quality of life.
For children with RLS, iron supplementation is the only treatment that currently has a guideline recommendation. No other medication has enough evidence in pediatric patients to be formally recommended.
Options for Severe or Treatment-Resistant RLS
RLS is considered refractory when it doesn’t respond to adequate trials of both gabapentin-type drugs and dopamine agonists, whether due to poor effectiveness, intolerable side effects, or augmentation. Before moving to stronger medications, doctors should first check iron levels and consider combining treatments.
When those steps fail, low-dose opioids become an option. Oxycodone and methadone are the most commonly used, with effective doses considerably lower than those used for chronic pain: typically 10 to 30 mg daily for oxycodone and 5 to 20 mg daily for methadone. Extended-release formulations are preferred for evening dosing, with short-acting versions available if daytime symptoms break through. Opioids carry well-known risks of dependence and require careful monitoring, but for people with truly refractory RLS, the guidelines give them a conditional recommendation. The condition is debilitating enough that the benefits can outweigh the risks when other options have failed.
Non-Drug Approaches That Help
Several non-medication strategies have clinical evidence behind them, though they work best as complements to treatment rather than replacements for severe RLS. Regular aerobic exercise significantly reduces symptom severity in studies. Pneumatic compression devices, which use inflatable sleeves to massage the legs, have also shown measurable improvement. Peroneal nerve stimulation, a device worn below the knee, received a conditional recommendation in the latest guidelines as a drug-free option.
Other approaches with some supporting evidence include yoga, acupuncture, and light therapy. These are unlikely to resolve moderate-to-severe RLS on their own, but they can complement medication and help with sleep quality. Practical habits matter too: reducing caffeine and alcohol, keeping a consistent sleep schedule, and avoiding long periods of sitting in the evening all help reduce the frequency and intensity of episodes.
Choosing the Right Treatment
The best starting point depends on your iron status and symptom severity. If you haven’t had your ferritin level checked, that’s the first step, since iron correction alone resolves symptoms for some people. For moderate-to-severe RLS that needs medication, gabapentin, pregabalin, or gabapentin enacarbil are the current standard. The choice between them often comes down to insurance coverage, dosing convenience, and individual side effect tolerance.
If you’ve been on a dopamine agonist for years and your symptoms are worsening, augmentation is the likely culprit. Transitioning to a gabapentin-class drug, sometimes with temporary opioid bridging during the switch, is the standard approach. The transition can be rough for a few weeks, but most people end up with better symptom control once they’re off dopamine agonists and stabilized on the newer recommended treatments.
