There is no single “best” medicine for varicose veins, but the most studied and widely recommended options fall into three categories: oral supplements that strengthen vein walls, topical gels that relieve local symptoms, and injectable treatments that close off damaged veins permanently. The right choice depends on whether you’re managing early symptoms like heaviness and swelling or dealing with bulging, painful veins that need more aggressive treatment.
How Varicose Vein Medicines Work
Varicose veins develop when the one-way valves inside your leg veins stop closing properly, allowing blood to pool and stretch the vein walls. Over time, the stretched walls leak fluid into surrounding tissue, triggering inflammation, swelling, and that heavy, aching feeling at the end of the day.
Medicines classified as venotonics or venoactive compounds target this process at several points. They seal the inner lining of blood vessels to reduce fluid leakage, improve the ability of vein walls to contract and push blood upward, reduce inflammation by preventing white blood cells from sticking to damaged vein walls, and promote lymphatic drainage so excess fluid clears faster. No pill will make a bulging varicose vein disappear, but these medicines can significantly reduce the symptoms that come with venous insufficiency.
Oral Medicines With the Strongest Evidence
Horse Chestnut Seed Extract
Horse chestnut seed extract is the most studied natural treatment for varicose vein symptoms. Its active compound, aescin, increases the sensitivity of vein walls to calcium, which helps the muscle fibers in those walls contract more effectively. It also reduces the permeability of small blood vessels, creating what researchers describe as a “sealing effect” that limits fluid leakage into surrounding tissue. This directly addresses the swelling, heaviness, and ankle puffiness that many people with varicose veins experience daily.
The dosage used in most clinical studies is 100 to 150 mg of aescin per day, taken orally. When shopping for supplements, check that the label lists the aescin content specifically, not just the total extract weight. Horse chestnut is also available as a topical cream, typically standardized to two percent aescin and applied three to four times daily for localized relief.
Flavonoid Supplements
Flavonoid-based supplements, particularly those combining the plant compounds diosmin and hesperidin, are widely prescribed in Europe for chronic venous disease. They work through similar pathways as horse chestnut: reducing inflammation, improving vein wall tone, and limiting fluid leakage. These are generally well tolerated, with hesperidin studied at doses up to 3 grams daily for up to three months without major issues.
There are a few important caveats. Hesperidin can slow blood clotting, so if you have a bleeding disorder or take blood-thinning medications, this combination could increase your risk of bruising and bleeding. It may also cause drowsiness, especially if you’re taking sedatives or sleep aids. If you have surgery planned, stop taking hesperidin-based supplements at least two weeks beforehand.
Topical Gels for Symptom Relief
If your main complaints are localized pain, swelling, or skin redness along a surface vein, topical heparin gel can provide meaningful relief without the need for oral medication. A study of 69 patients using a high-strength heparin gel (2,400 IU/g) for 14 days found that 95.7% experienced improvement in pain, 92.8% saw reduced swelling, and 88.4% had less redness. Pain scores dropped from an average of 4.65 out of 10 down to 1.71.
A broader review of 20 clinical trials covering over 1,000 participants confirmed that topical heparin gels were significantly more effective than placebo for reducing varicose vein symptoms. Notably, the high-strength liposomal formulations performed comparably to injected blood thinners for treating superficial vein inflammation, making them a practical at-home option for flare-ups. Standard sodium heparin gel (1,000 IU/g) also outperformed heparinoid creams for relieving leg pain, swelling, and heaviness.
Anti-inflammatory gels containing diclofenac have also shown significant improvement in local symptoms, and combination gels containing plant-derived escin offer another topical alternative.
Injection Treatments That Close Veins
When varicose veins are large enough to cause persistent pain, skin changes, or cosmetic concern, oral supplements and gels won’t resolve the underlying problem. Sclerotherapy uses injectable medicines called sclerosing agents that irritate the inner lining of the damaged vein, causing it to collapse, seal shut, and eventually be reabsorbed by the body. Blood reroutes through healthier veins nearby.
The two most common sclerosing agents are polidocanol and sodium tetradecyl sulfate. Both belong to the same drug class and work through the same basic mechanism, though they differ slightly in how they’re formulated. Your doctor chooses between them based on the size and location of the veins being treated. For larger varicose veins, these agents are often mixed into a foam that covers more surface area inside the vein, improving the success rate compared to liquid injection alone.
Sclerotherapy sessions typically take 15 to 30 minutes. You’ll wear compression stockings afterward, and most people return to normal activities within a day or two. Multiple sessions are often needed, spaced a few weeks apart, because not every vein responds fully on the first treatment.
Compression: The Foundation of Treatment
Regardless of which medicine you use, graduated compression stockings remain the baseline treatment for varicose veins. They apply the most pressure at the ankle and gradually decrease up the leg, physically assisting blood flow back toward the heart. Compression doesn’t fix damaged valves, but it reduces the pooling that causes symptoms. Most vein specialists recommend wearing them daily and consider them essential alongside any oral or topical treatment.
Matching Treatment to Severity
For early-stage symptoms like leg heaviness, mild swelling, and achiness after standing, oral venotonics such as horse chestnut extract or diosmin-hesperidin combined with compression stockings are a reasonable starting point. Adding a topical heparin gel can help during flare-ups when a specific area feels tender or swollen.
If you have visible, bulging veins that are progressively worsening, causing skin discoloration near the ankles, or leading to episodes of superficial vein inflammation, oral medicines alone won’t be sufficient. Sclerotherapy or other procedural treatments (thermal ablation, for example) become the primary approach, with oral supplements playing a supporting role in managing residual symptoms and slowing progression in other veins.
The size of the affected veins matters too. Spider veins and small reticular veins respond well to sclerotherapy. Larger, ropey varicose veins often require heat-based closure procedures first, with sclerotherapy used afterward to clean up smaller branches.
