The body’s normal temperature is generally accepted as 98.6°F (37°C), but that number is outdated. It comes from a massive 19th-century study, and modern research shows the average human body temperature has been slowly dropping ever since. Today, a healthy adult’s oral temperature typically falls somewhere between 97.0°F and 99.0°F, with significant variation based on the time of day, where you measure, your age, and your biology.
Where 98.6°F Came From
The 98.6°F standard traces back to 1868, when a German physician named Carl Wunderlich published an analysis of several million temperature readings taken from roughly 25,000 patients. He concluded that 37°C (98.6°F) was the mean temperature of his enormous dataset and declared it the “physiologic point” of the healthy human body. That number stuck for over 150 years.
But Wunderlich’s thermometers were less precise than modern instruments, and the populations he studied lived in very different conditions. A landmark study published in eLife, using data spanning nearly two centuries in the United States, found that body temperature has been declining at a rate of about 0.03°C (roughly 0.05°F) per decade. Men born in the early 1800s ran temperatures about 1°F higher than men today. Women showed a similar decline of about 0.6°F since the 1890s. The likely reasons include lower rates of chronic infection and inflammation, better nutrition, and climate-controlled environments.
What’s Actually Normal Today
For most healthy adults, a normal oral temperature sits closer to 97.5°F to 98.9°F (36.4°C to 37.2°C). There’s no single “correct” number. Your personal baseline depends on your age, sex, time of day, and activity level. Getting familiar with your own typical reading matters more than comparing yourself to a universal standard.
A fever is medically defined as a body temperature of 100.4°F (38°C) or higher. On the other end, hypothermia begins when core temperature drops below 95°F (35°C). Moderate hypothermia sets in around 90°F (32.2°C), and severe hypothermia occurs below 82.4°F (28°C).
How Temperature Shifts Throughout the Day
Your body temperature isn’t static. It follows a predictable daily rhythm tied to your circadian clock. Temperature is lowest in the early morning hours, typically between 4 a.m. and 6 a.m., then rises steadily through the morning. Most people hit their daily peak in the late afternoon or early evening. There’s also a subtle dip between 2 p.m. and 4 p.m. for many people, which partly explains that mid-afternoon slump.
This daily swing can span about 1°F from your lowest to highest reading. So if you take your temperature at 6 a.m. and get 97.4°F, then check again at 5 p.m. and see 98.4°F, both readings are perfectly normal. This is why a slightly elevated evening reading doesn’t necessarily mean you have a fever.
Where You Measure Matters
Different parts of the body give different readings. The standard reference point is an oral (under the tongue) measurement, but here’s how other methods compare:
- Rectal: Reads 0.5°F to 1°F higher than oral. This is considered the most accurate measurement of core temperature, especially in infants.
- Ear (tympanic): Reads 0.5°F to 1°F higher than oral.
- Armpit (axillary): Reads 0.5°F to 1°F lower than oral.
- Forehead (temporal): Reads 0.5°F to 1°F lower than oral.
These offsets matter when you’re checking for a fever. An armpit reading of 99°F is roughly equivalent to an oral reading of 99.5°F to 100°F. For children, the Mayo Clinic considers a fever to be a rectal, ear, or forehead temperature of 100.4°F or higher, an oral temperature of 100°F or higher, or an armpit temperature of 99°F or higher.
Age, Sex, and Hormonal Influences
Infants and young children tend to run warmer than adults, partly because their metabolisms are higher relative to body size. Older adults often run cooler, which can make fevers harder to detect. An elderly person with a serious infection may register a temperature that looks unremarkable on paper but actually represents a significant spike from their personal baseline.
Hormonal cycles also play a clear role. In people who menstruate, basal body temperature rises after ovulation by anywhere from 0.4°F to 1°F and stays elevated through the second half of the cycle (the luteal phase). This shift is reliable enough that some people use daily temperature tracking as a fertility awareness method. Pregnancy keeps temperatures elevated for similar hormonal reasons.
Exercise, Weather, and Other Temporary Shifts
Physical exertion raises core temperature substantially. Your brain normally keeps your body within a degree or two of its baseline, but during intense exercise, especially in hot or humid conditions, internal temperature can climb to 101°F to 104°F (38.3°C to 40°C). This is the territory where heat exhaustion becomes a risk, and it’s why cooling down and hydrating during hard workouts is essential rather than optional.
Other temporary influences include eating a meal (digestion generates heat), drinking hot or cold liquids shortly before measuring (which can skew oral readings), stress and strong emotions, and even the clothing you’re wearing. If you want an accurate baseline reading, take your temperature first thing in the morning before getting out of bed, eating, or drinking anything.
What Your Reading Actually Tells You
A single temperature reading is a snapshot, not a diagnosis. What matters more is the pattern: how your current reading compares to your personal norm, whether it’s rising or falling, and what symptoms accompany it. A reading of 99.5°F in someone who normally runs at 97.8°F represents a bigger shift than the same reading in someone whose baseline is 98.8°F.
If you’re checking because you feel unwell, take your temperature a few times over several hours rather than relying on one measurement. Use the same method and the same thermometer each time for consistency. And keep in mind that the old benchmark of 98.6°F is an average from a different era. Your healthy temperature is yours, and it’s almost certainly not that exact number.
