What Is the Deadliest Disease in the World?

Chronic obstructive pulmonary disease (a group of lung conditions that make breathing progressively harder) killed about 3.4 million. Lung cancer took 1.9 million lives, and Alzheimer’s disease and other dementias killed 1.8 million. Diabetes and kidney disease also ranked in the global top ten.

COVID-19 disrupted these rankings dramatically. In 2021, it was directly responsible for 8.8 million deaths, temporarily making it the second leading cause of death worldwide. Among infectious diseases in a typical year, tuberculosis holds that distinction. TB killed 1.23 million people in 2024, making it the single deadliest infectious agent on Earth.

Diseases With Near-100% Fatality Rates

Some diseases kill almost everyone who develops symptoms, even though they affect far fewer people overall. These are the deadliest in terms of individual survival odds.

Rabies is nearly 100% fatal once symptoms appear. Fewer than 10 documented cases of unvaccinated humans surviving symptomatic rabies have ever been recorded, and the majority of those survivors had significant lifelong neurological damage. The key word is “symptomatic.” Rabies is almost entirely preventable if you receive post-exposure treatment promptly after a bite. But once the virus reaches the brain and causes symptoms like confusion, agitation, and fear of water, there is no effective treatment.

Prion diseases, such as Creutzfeldt-Jakob disease (CJD), are always fatal. These aren’t caused by a virus or bacterium but by misfolded proteins in the brain that trigger a chain reaction, causing other proteins to misfold and progressively destroying brain tissue. CJD moves fast: the median time from first symptoms to death is just four to five months. There is no treatment.

Primary amebic meningoencephalitis, the infection caused by the brain-eating amoeba found in warm freshwater, kills more than 97% of those infected. Between 1962 and 2024, 167 cases were reported in the United States. Only four people survived. Most patients die within five days of symptoms appearing, though the range extends from one to 18 days.

Viral Hemorrhagic Fevers

Ebola and Marburg virus cause similar diseases: sudden onset of fever, severe bleeding, and organ failure. Marburg virus disease has an average case fatality rate of about 50%, but that number varies wildly depending on the outbreak and the quality of care available. Some outbreaks have had fatality rates of 24%, while others have reached 88% or even 100% in small clusters. The 2005 Angola outbreak killed 88% of those infected. Several outbreaks in Uganda and Guinea between 2014 and 2021 had 100% fatality among confirmed cases, though these involved very small numbers of patients.

The 1918 influenza pandemic killed an estimated 50 to 100 million people worldwide. For context, World War I, which overlapped with the pandemic, killed about 20 million. The 1918 flu was unusually lethal because it triggered a severe immune overreaction, disproportionately killing healthy young adults rather than the very old and very young.

COVID-19 caused an estimated 27 million excess deaths between January 2020 and November 2023. While its case fatality rate was far lower than the Black Death or the 1918 flu, the sheer number of infections across a connected global population produced a massive total death toll.

A case fatality rate measures the proportion of people diagnosed with a disease who die from it. This is how rabies and prion diseases earn their reputation: they’re rare, but once you have them, survival is essentially zero.

A disease can be extremely deadly at the individual level while barely registering in global death statistics. Prion diseases kill a few hundred people per year worldwide. Heart disease kills nine million. Both facts are true, and both are important, but they answer different questions.
