The difference is almost entirely about packaging and marketing: standard Allegra (often labeled “Allegra Allergy”) is sold for seasonal allergy symptoms like sneezing, runny nose, and itchy eyes, while Allegra Hives is sold specifically for treating itchy, raised skin welts known as hives.

Same Drug, Different Label

Fexofenadine is a second-generation antihistamine that blocks histamine receptors throughout the body. It works by preventing histamine from triggering inflammation, whether that shows up as a stuffy nose from pollen or as itchy welts on your skin. The FDA has approved fexofenadine for both seasonal allergic rhinitis (hay fever) and chronic idiopathic urticaria (recurring hives without a known cause), so the same molecule treats both problems.

The inactive ingredients in Allegra Hives are standard tablet fillers and coatings: silicon dioxide, cellulose, corn starch, magnesium stearate, and a few others used to hold the tablet together and give it its color. For children ages 6 to 11, the recommended dose of fexofenadine drops to 30 mg twice daily regardless of whether it’s being used for allergies or hives. The adult 180 mg tablets sold as Allegra Hives are not sized for this age group, so parents of younger children typically need the lower-dose tablets or oral suspension instead.

How Fexofenadine Works on Hives

Hives form when certain immune cells in the skin release histamine, causing small blood vessels to leak fluid into surrounding tissue. That produces the characteristic raised, itchy welts. Fexofenadine blocks the receptors that histamine latches onto, which reduces both the swelling and the itch. It also appears to inhibit the release of histamine from mast cells and other inflammatory cells, adding a second layer of effect beyond simple receptor blocking.

If both boxes contain 180 mg fexofenadine tablets, buy whichever is cheaper or more readily available. You are getting the same medication. Some stores price the Hives version slightly differently, so it’s worth comparing. Generic fexofenadine 180 mg is also widely available and identical in effect.

One practical note: standard Allegra sometimes comes in 60 mg tablets meant for twice-daily dosing. Allegra Hives is typically sold only as the 180 mg once-daily tablet. If you prefer the convenience of taking one pill per day, either the 180 mg Allegra Allergy or Allegra Hives will work the same way. The total daily dose of fexofenadine is 180 mg for adults in both cases, whether that comes as one tablet or split across two.

What Fexofenadine Won’t Do

Fexofenadine is a non-drowsy antihistamine, meaning it doesn’t cross into the brain the way older antihistamines like diphenhydramine do. That’s a real advantage for daytime use, but it also means it’s not as sedating, which some people with severe nighttime itching notice. It treats the symptoms of hives (itch, swelling, redness) but does not address whatever is triggering them.
