What Is the Difference Between Pepcid and Pepcid Complete?

Pepcid (sold over the counter as Pepcid AC) contains only famotidine, a single acid-reducing ingredient. Pepcid Complete combines that same famotidine with two antacids, calcium carbonate and magnesium hydroxide, in a chewable tablet. The result is a product that starts working faster than Pepcid AC alone but shares the same long-lasting acid control.

What’s in Each Product

Pepcid AC is a film-coated tablet containing 10 mg or 20 mg of famotidine and nothing else. You swallow it with water, and it works by blocking the signals that tell your stomach to produce acid.

Pepcid Complete is a chewable tablet containing 10 mg of famotidine plus 800 mg of calcium carbonate and 165 mg of magnesium hydroxide. Those two extra ingredients are classic antacids, the same kind found in products like Tums and Milk of Magnesia. They don’t stop acid production. Instead, they chemically neutralize the acid already sitting in your stomach.

How They Work Differently

Famotidine belongs to a class called H2 blockers. After you eat, your body releases a chemical messenger called histamine that binds to receptors on your stomach lining and triggers acid production. Famotidine blocks those receptors, so the signal never gets through and your stomach produces less acid. This effect takes roughly an hour to kick in after you swallow the tablet, but once it does, it suppresses acid for 10 to 12 hours.

Antacids work on a completely different timeline. They don’t prevent acid from being made. They neutralize whatever acid is already there, which provides relief within minutes. About 70% of episodes stayed relieved for at least 7 hours with the combination, compared to 58.5% with the antacid alone. Patients on the combination formula also needed rescue medication less often than any other group.

In short, Pepcid AC gives you the long game but takes a while to start. The antacid gives you fast relief that fades. Pepcid Complete gives you both.

When to Use Each One

Pepcid AC works best as a preventive measure. The label recommends swallowing a tablet 10 to 60 minutes before eating foods or drinking beverages that trigger your heartburn. If you know a spicy dinner is coming and you plan ahead, Pepcid AC covers you for the rest of the evening.

Pepcid Complete is better suited for heartburn that’s already happening or about to happen with little warning. Because you chew the tablet and the antacids go to work immediately, you don’t have to wait an hour for relief. It’s also a reasonable choice if you want both fast and extended coverage in a single dose.

Side Effects

The famotidine in both products can cause headache, dizziness, or constipation in roughly 1% or more of users. Less commonly, some people experience nausea, diarrhea, or dry mouth.

Pepcid Complete’s antacid ingredients add their own considerations. Calcium carbonate can contribute to constipation, while magnesium hydroxide can have a mild laxative effect. The two tend to balance each other out in combination, which is one reason they’re paired together. However, people with kidney problems should be cautious with both the famotidine and the magnesium component, since the kidneys are responsible for clearing famotidine from the body and for regulating magnesium levels. Higher famotidine levels in people with reduced kidney function have been linked to confusion, dizziness, and heart rhythm changes.

Which One to Choose

If your heartburn is predictable and you can take something before meals, Pepcid AC is straightforward and effective. If you tend to get caught off guard by heartburn or want the fastest possible relief without sacrificing long-lasting protection, Pepcid Complete covers both needs in one chewable tablet. The clinical evidence supports that the combination genuinely outperforms either ingredient on its own, so you’re not just paying for two things duct-taped together. The combination works better as a unit than you’d expect from its parts.
