After thyroid cancer surgery, levothyroxine serves a dual purpose: it replaces the missing thyroid hormone and suppresses TSH, a pituitary hormone that can stimulate any remaining cancer cells to grow. Suppression therapy historically required doses higher than typical replacement, enough to push TSH well below the normal range.

Guidelines from the American Thyroid Association have shifted on this point. The 2015 recommendations called for complete TSH suppression (to nearly undetectable levels) in high-risk patients and moderate suppression in intermediate-risk patients. The updated 2025 guidelines no longer recommend driving TSH to undetectable levels for anyone. Studies found no benefit to pushing TSH below 2 mU/L, and one study linked complete suppression to an increased risk of death from heart problems. Doses above 500 mcg are associated with serious cardiac complications, particularly in older adults. This protocol exists only for a medical emergency and bears no resemblance to how the medication is used day to day.

Signs Your Dose Is Too High

Because levothyroxine is identical to the hormone your thyroid naturally produces, the line between a therapeutic dose and an excessive one is essentially the line between normal thyroid levels and hyperthyroidism. Too much medication pushes you into an overactive state, and the symptoms reflect that: rapid or irregular heartbeat, chest pain, tremors, nervousness, difficulty sleeping, diarrhea, excessive sweating, heat intolerance, and unexplained weight loss. Rare cases of people intentionally taking excessive doses, sometimes to lose weight or sometimes due to underlying mental health conditions, represent a recognized clinical phenomenon called factitious hyperthyroidism.

Someone who has had their entire thyroid removed will generally need a higher replacement dose than someone whose thyroid is underactive but still producing some hormone.
