Researchers are now studying doses as low as 50 mg per day for brain protection, and over-the-counter lithium orotate supplements deliver as little as 5 mg of elemental lithium.
Standard Starting Doses for Bipolar Disorder
When lithium is prescribed for bipolar disorder, most guidelines recommend starting at 200 mg to 600 mg taken at night. The dose is then gradually increased based on blood levels, which need to fall within a therapeutic window of 0.5 to 1.2 mmol/L (measured as a trough level, meaning right before the next dose). For long-term maintenance, most people end up taking 900 to 1,200 mg per day, split into two or three doses with regular-release tablets or two doses with extended-release formulations.

A common starting point is 300 mg twice daily, adjusted upward based on blood tests.
Research suggests that this lower-range approach, keeping blood levels around 0.5 to 0.8 mmol/L, works about as well as higher levels while producing fewer side effects. For people with early kidney problems, clinicians target even lower levels of 0.5 to 0.6 mmol/L, which translates to lower daily doses. The exact milligram amount varies from person to person because factors like body weight, age, kidney function, and hydration all influence how much lithium stays in your bloodstream.
Microdose Lithium for Brain Protection
A growing area of research involves lithium doses far below what’s used for mood disorders. Clinical trials are testing doses as low as 50 mg per day (sometimes called “trace doses”) for their potential to protect against cognitive decline and conditions like Alzheimer’s disease. At this level, lithium is thought to activate protective pathways in the brain without causing the side effects associated with standard psychiatric doses.
One active clinical trial is giving healthy participants and people with mild cognitive impairment just 50 mg of lithium daily, roughly one-tenth to one-twentieth of a typical bipolar maintenance dose, to see whether it slows or prevents cognitive decline. The research is still in early stages, and these microdoses are not an approved treatment for any condition. But the concept has attracted serious scientific interest because even tiny amounts of lithium appear to reach the brain. An MRI study that gave healthy men 5 mg of elemental lithium daily (through lithium orotate supplements) for up to 28 days detected lithium in their brains, at concentrations roughly 2 to 7 percent of what a standard therapeutic dose produces.
Lithium Orotate Supplements
Lithium orotate is sold over the counter as a dietary supplement, and its doses are dramatically lower than prescription lithium carbonate. A typical supplement provides about 5 mg of elemental lithium per tablet. To put that in perspective, a standard 300 mg lithium carbonate pill contains roughly 56 mg of elemental lithium, so a 5 mg lithium orotate supplement delivers less than one-tenth of that amount.
Some proponents claim that lithium orotate crosses into the brain more efficiently than lithium carbonate, but this hasn’t been established in rigorous human trials. What the MRI research does confirm is that even at these very low supplement doses, measurable lithium reaches the brain. These supplements are not regulated the same way prescription lithium is, and they aren’t approved for treating bipolar disorder, depression, or any psychiatric condition.
Why the Dose Matters for Side Effects
Lithium’s side effects are closely tied to how much is circulating in your blood. At levels below 1.0 mmol/L, side effects are relatively uncommon. The most persistent ones at therapeutic doses include a fine hand tremor, increased thirst, and more frequent urination. These tend to be mild and manageable for most people.
Toxicity becomes a concern when blood levels climb above 1.5 mmol/L, though elderly patients can experience toxic effects even within the normal therapeutic range. Signs of toxicity include a coarse, obvious tremor (as opposed to the fine tremor seen at lower levels), confusion, nausea, and coordination problems. This is why lithium requires regular blood monitoring regardless of the dose you’re taking. Small changes in hydration, salt intake, or kidney function can shift blood levels significantly, even if your dose stays the same.
Kidney Function Changes the Equation
Your kidneys are responsible for clearing lithium from your body, so any reduction in kidney function means lithium accumulates faster and reaches higher blood levels at the same dose. The FDA recommends that people with mild to moderate kidney impairment start at lower doses than usual and increase more slowly, with more frequent blood monitoring. For severe kidney impairment, lithium use is generally avoided altogether.
This is one of the main reasons why the “lowest dose” question doesn’t have a single universal answer. Two people taking the same 300 mg tablet can end up with very different blood levels depending on their kidney function, age, and overall health. The dose is always calibrated to the individual, with blood levels as the true guide rather than the number of milligrams on the pill.
