The lowest manufactured dose of venlafaxine is 37.5 mg, available as an extended-release capsule. This is the smallest single pill you can get, and it serves as either a starting dose or a brief introductory step before moving to higher therapeutic doses, depending on the condition being treated.
How the 37.5 mg Dose Is Used
The 37.5 mg dose plays different roles depending on why it’s prescribed. For depression and generalized anxiety disorder, the standard starting dose is actually 75 mg per day, but some patients begin at 37.5 mg for four to seven days to ease into the medication before stepping up. For panic disorder, 37.5 mg per day for a full seven days is the recommended starting point before any increase.
For social anxiety disorder, treatment typically begins at 75 mg per day without a lower introductory step.
The immediate-release tablet form of venlafaxine (taken two or three times daily rather than once) also starts at a total of 75 mg per day for depression, split across multiple doses. The extended-release version is more commonly prescribed today because of the convenience of once-daily dosing.
Is 37.5 mg a Therapeutic Dose?
For most approved uses, 37.5 mg is considered a transitional dose rather than a target. The goal is usually to reach at least 75 mg per day within the first week. That said, 37.5 mg is used as a standalone treatment dose in some situations. For menopausal hot flashes, an off-label but well-established use, 37.5 mg once daily is the starting dose and often the only dose needed. Higher amounts haven’t been shown to work better for hot flashes and tend to cause more side effects.
Venlafaxine reaches steady levels in your bloodstream within about three days of consistent dosing, so even at 37.5 mg, the medication is actively working relatively quickly.
Available Pill Strengths
Extended-release venlafaxine capsules come in three strengths: 37.5 mg, 75 mg, and 150 mg. There is no commercially manufactured pill smaller than 37.5 mg. If a prescriber wants a patient on a dose below that, it requires special approaches like compounding pharmacies or, in some cases, opening the capsule and counting out individual beads (each extended-release capsule contains tiny beads that can technically be separated).
Side Effects at the Lowest Dose
Side effects with venlafaxine are dose-dependent, meaning lower doses generally produce fewer problems. In a clinical trial comparing 75 mg, 225 mg, and 375 mg daily against a placebo, the 75 mg group (the lowest dose tested in the trial) already showed notably higher rates of certain side effects compared to placebo. Nausea affected about 33% of people at 75 mg versus 14% on placebo. Dizziness hit 19% versus 4%. Insomnia occurred in roughly 23% compared to 10% on placebo, and drowsiness affected about 17% versus 4%.
Some side effects climbed steeply with higher doses. Nausea jumped from 33% at 75 mg to 58% at 375 mg. Sweating went from 7% to 19%. Tremor, which barely registered at 75 mg (1%), affected 10% of people at the highest dose. At 37.5 mg, which is half the lowest dose studied in that trial, side effects would be expected to fall somewhere between placebo and the 75 mg numbers, though this wasn’t formally tested in the same way.
Reduced appetite, nervousness, and fatigue are also common early side effects that tend to settle over the first few weeks.
Why Tapering Involves Even Smaller Doses
When stopping venlafaxine, the lowest manufactured dose of 37.5 mg is often not small enough. Venlafaxine is well known for causing discontinuation symptoms like dizziness, brain zaps, irritability, and flu-like feelings, and these can occur even when stepping down from 37.5 mg to zero. The relationship between dose and brain activity isn’t linear: going from 37.5 mg to nothing represents a proportionally larger change in brain chemistry than going from 150 mg to 112.5 mg.
To manage this, some people taper by roughly 10% of their current dose per month as they get to lower levels. One practical method involves opening extended-release capsules and removing a small number of beads at a time, reducing by as little as one bead per day. This approach, sometimes called microtapering, helps smooth out the transition and minimize withdrawal discomfort. It requires coordination with a prescriber, since it goes below any commercially available dose.
