A healthy adult heart beats 60 to 100 times per minute at rest, but heart rates well below that range are common and often completely normal. Trained endurance athletes can have resting rates in the 30s or even low 40s. The lowest heart rate ever reliably documented belongs to cyclist Miguel Indurain, whose resting rate was reportedly 28 bpm. Whether a low heart rate is impressive or dangerous depends entirely on why it’s low and whether it causes symptoms.
What Counts as a Low Heart Rate
Bradycardia, the medical term for a slow heart rate, is traditionally defined as anything below 60 bpm in adults. In practice, though, that cutoff is somewhat arbitrary. Population studies often use 50 bpm as the threshold, since plenty of healthy people sit comfortably in the 50s without any problems. The number on your wrist or chest strap matters far less than how you feel at that number.
For children and infants, the expected range is much higher. A newborn’s heart normally beats 100 to 160 times per minute, while toddlers aged one to three typically fall between 80 and 130 bpm. The range gradually narrows as children grow, reaching the adult standard of 60 to 100 bpm by around age 15.
Why Athletes Have Unusually Low Rates
Elite endurance athletes routinely have resting heart rates in the 30s and 40s. For decades, this was attributed to heightened activity in the vagus nerve, which acts as a brake on heart rate. More recent research tells a more interesting story. Studies using drugs that temporarily shut off both branches of the nervous system found that athletes still have a slower intrinsic heart rate, meaning the heart’s natural pacemaker itself has been physically remodeled by years of training.
One mechanism involves the heart’s right atrium, which stretches in response to the increased blood volume that comes with endurance training. That stretching appears to directly alter the electrical cells that set your heart’s rhythm. Exercise training also reduces the activity of a specific ion channel (sometimes called the “funny channel”) that controls pacing speed. On top of all this, genetics play a role. Some people are simply born with electrical wiring that predisposes them to lower heart rates, and those individuals may also be drawn to endurance sports where that trait becomes an advantage.
The practical result: a highly trained heart pumps more blood per beat, so it doesn’t need to beat as often to deliver the same amount of oxygen. A resting rate of 35 bpm in a competitive cyclist is a sign of cardiovascular efficiency, not disease.
Your Heart Rate Drops During Sleep
You don’t need to be an athlete to see surprisingly low numbers. During sleep, your heart rate typically runs 20% to 30% below your daytime resting rate. For someone whose waking rate is 70 bpm, that can mean dipping into the low 50s or even high 40s overnight. The lowest point usually occurs during deep, non-REM sleep stages, when blood pressure also drops. A sleeping heart rate of 50 to 75 bpm is considered normal for most healthy adults.
If you wear a fitness tracker to bed and notice occasional dips into the 40s, that’s usually nothing to worry about, especially if you’re physically active. Consistent readings in the 30s during sleep, or readings accompanied by long pauses between beats, are worth mentioning to a doctor.
When a Low Heart Rate Is a Problem
A slow heart rate becomes dangerous when it can’t deliver enough blood to your brain and organs. The heart rate number alone doesn’t determine this. Some people feel perfectly fine at 45 bpm, while others become symptomatic at 55. The warning signs to watch for include dizziness or lightheadedness, fainting or near-fainting episodes, unusual fatigue that doesn’t match your activity level, shortness of breath, confusion, and chest discomfort.
In emergency settings, clinicians look for specific red flags: low blood pressure, altered mental status, signs of shock, and chest pain. These indicate that the slow rate is actively compromising blood flow and needs immediate treatment. A heart rate that’s simply low on a monitor, with no symptoms and no distress, is treated very differently from one that’s causing someone to pass out.
Medical Causes of Abnormally Slow Rates
When a low heart rate isn’t explained by fitness or sleep, something is usually disrupting the heart’s electrical system. The two main culprits are sinus node dysfunction, where the heart’s natural pacemaker fires too slowly, and heart block, where electrical signals get delayed or lost on their way from the upper chambers to the lower chambers.
A wide range of conditions can trigger these problems:
- Aging: The heart’s conduction system naturally deteriorates, particularly after age 70.
- Thyroid disorders: An underactive thyroid slows metabolism broadly, including heart rate.
- Electrolyte imbalances: Too much or too little potassium disrupts the electrical signals that coordinate heartbeats.
- Heart conditions: Heart failure, prior heart attacks, cardiomyopathy, and congenital heart defects can all damage the conduction system.
- Infections: Lyme disease is a well-known cause, along with Chagas disease and several others that can inflame heart tissue.
- Inflammatory conditions: Lupus, sarcoidosis, rheumatoid arthritis, and scleroderma can affect the heart’s electrical pathways over time.
- Sleep apnea: Obstructive sleep apnea can trigger episodes of very slow heart rate during sleep, sometimes with prolonged pauses.
Medications That Lower Heart Rate
Several common drug classes reduce heart rate as either their primary effect or a side effect. Beta-blockers and certain calcium channel blockers are prescribed specifically to slow the heart in conditions like high blood pressure, heart failure, and irregular rhythms. Other medications that can push heart rate down include antiarrhythmic drugs, digoxin (used for heart failure and atrial fibrillation), opioids, certain antidepressants, and lithium.
If you’re taking one of these medications and notice your resting rate consistently dropping below 50, or you’re experiencing dizziness or fatigue, that’s worth a conversation about adjusting your dose. Medication-related bradycardia is one of the most straightforward causes to fix, since the solution is often simply changing or reducing the drug responsible.
How Low Is Too Low
There’s no single bpm number that universally separates safe from dangerous. Context is everything. A resting rate of 38 bpm in a 25-year-old marathon runner who feels great is perfectly healthy. That same rate in a 75-year-old who’s been feeling dizzy and exhausted is a medical concern that may require a pacemaker.
As a general framework: rates in the 50s are normal for many people, especially during sleep or if you exercise regularly. Rates in the 40s deserve attention if they come with symptoms but are often benign in fit individuals. Sustained rates in the 30s are unusual outside of elite athletes and deep sleep, and rates below 30 are extremely rare and almost always require medical evaluation. The key question is never just “how low is it?” but “how low is it, and how do you feel?”
