What Is the Most Common Cause of Lightheadedness?

The most common cause of lightheadedness is a temporary drop in blood flow to the brain, usually triggered by standing up too quickly. This is called orthostatic hypotension, and it happens when gravity pulls blood toward your legs faster than your body can compensate. But lightheadedness is a broad symptom with many possible triggers, from skipping meals to anxiety to medication side effects. Dizziness accounts for roughly 5 percent of all primary care visits, making it one of the most frequent complaints doctors hear.

Why Standing Up Triggers It

Every time you stand, gravity pulls blood downward into your legs and abdomen. Your body has a built-in fix for this: specialized cells near your heart and neck arteries detect the pressure change and signal your brain within seconds. Your heart rate ticks up, blood vessels tighten, and pressure stabilizes. The whole process takes about one to three seconds in a healthy person.

Orthostatic hypotension happens when that process is too slow or too weak. Your brain briefly gets less blood than it needs, and you feel lightheaded, woozy, or like you might faint. A blood pressure drop of 20 points or more (systolic) or 10 points or more (diastolic) within three minutes of standing is considered abnormal. Dehydration is the single most frequent reason this happens in otherwise healthy people. Not drinking enough water, sweating heavily, or recovering from illness reduces your blood volume, making the pressure drop more pronounced.

Older adults are especially vulnerable because the pressure-sensing cells near the heart become less responsive with age. Hot weather, large meals, and alcohol all make it worse by redirecting blood away from your core.

Medications That Cause Lightheadedness

If you started a new medication and began feeling lightheaded, the drug is a likely culprit. Five classes of medication are the most common offenders:

  • Blood pressure drugs, including diuretics (water pills), calcium channel blockers, and ACE inhibitors. These lower blood pressure by design, which sometimes overshoots.
  • Antidepressants, particularly SSRIs and SNRIs, which can affect blood pressure regulation.
  • Anti-anxiety medications, especially benzodiazepines, which suppress central nervous system activity.
  • Antihistamines, the kind found in allergy and sleep medications.
  • Antispasmodic drugs used for stomach cramps, which can relax blood vessels as a side effect.

These medications don’t always cause lightheadedness on their own, but combining two or more of them dramatically increases the risk. If you take blood pressure medication and an antihistamine, for example, the effects can stack.

Low Blood Sugar

Your brain runs almost entirely on glucose. When blood sugar drops below about 70 mg/dL, lightheadedness is one of the first warning signs. You might also notice shakiness, sweating, irritability, or a sudden feeling of hunger.

This doesn’t only affect people with diabetes. Skipping meals, exercising on an empty stomach, or drinking alcohol without eating can push blood sugar low enough to cause symptoms in anyone. The fix is straightforward: eating something with fast-acting carbohydrates (juice, crackers, fruit) typically resolves the lightheadedness within 10 to 15 minutes.

Anxiety and Hyperventilation

Anxiety is an underrecognized cause of lightheadedness, and it creates a frustrating loop: feeling lightheaded makes you anxious, and anxiety makes you more lightheaded. The mechanism is surprisingly physical. When stress activates your fight-or-flight response, your breathing rate increases. If there’s no physical exertion to match that faster breathing, you start exhaling too much carbon dioxide.

Low carbon dioxide causes blood vessels throughout your body to narrow, including the ones supplying your brain. Less blood reaches your brain, and you feel dizzy, weak, and disconnected. Your heart may pound. You might feel short of breath even though you’re actually breathing too much, not too little. Slowing your breathing, particularly by extending your exhale, reverses the process within a few minutes.

Inner Ear Problems

Lightheadedness and vertigo are related but different. Lightheadedness feels like you might faint. Vertigo feels like the room is spinning. The distinction matters because they point to different causes, though the two can overlap.

The most common inner ear cause of dizziness is benign paroxysmal positional vertigo (BPPV), which affects about 2.4 percent of people at some point in their lives, making it the most common vestibular disorder in adults. It happens when tiny calcium crystals inside your ear detach from their normal position and drift into the semicircular canals, the fluid-filled tubes your brain uses to detect motion. When these crystals shift, they send false movement signals to your brain, creating an intense spinning sensation triggered by head movements like rolling over in bed or looking up.

BPPV is more likely to cause true vertigo than lightheadedness, but many people describe the sensation as “lightheaded” because the two feelings can be hard to distinguish. A simple repositioning maneuver (the Epley maneuver) moves the crystals back where they belong and resolves symptoms in most cases, often in a single session.

How to Tell What’s Causing Yours

The pattern of your lightheadedness is the strongest clue to its cause. Pay attention to when it happens and what you were doing at the time.

If it strikes when you stand up and passes within a few seconds, orthostatic hypotension or dehydration is the most likely explanation. If it comes on during periods of stress or rapid breathing, hyperventilation is probable. If it’s tied to head position changes and feels more like spinning, an inner ear issue is worth investigating. If it shows up a few hours after your last meal or after exercise, low blood sugar is the likely trigger.

You can check for orthostatic hypotension at home if you have a blood pressure cuff. Lie down for five minutes and take a reading. Then stand up and measure again after one minute and again after three minutes. A systolic drop of 20 points or more, a diastolic drop of 10 points or more, or lightheadedness during the test all count as a positive result.

Warning Signs That Need Urgent Attention

Most lightheadedness is harmless and short-lived. But certain accompanying symptoms signal something more serious, like a stroke or heart problem. Seek emergency care if lightheadedness comes with a sudden severe headache, chest pain, difficulty breathing, numbness or weakness on one side of your body, a rapid or irregular heartbeat, confusion, slurred speech, blurred or double vision, or fainting. Lightheadedness following a head injury also warrants immediate evaluation.
