There isn’t one single “pneumonia vaccine.” Four pneumococcal vaccines are currently available in the United States, each sold under a different brand name: Prevnar 20, Vaxneuvance, Capvaxive, and Pneumovax 23. Which one you’d receive depends on your age, health history, and whether you’ve had a pneumococcal vaccine before.
The Four Pneumonia Vaccines by Name
Each vaccine targets a different number of bacterial strains (called serotypes) that cause pneumococcal disease. Here’s the lineup:
- Prevnar 20 (PCV20) protects against 20 types of pneumococcal bacteria.
- Vaxneuvance (PCV15) protects against 15 types.
- Capvaxive (PCV21) protects against 21 types. This is the newest option, approved for adults 18 and older.
- Pneumovax 23 (PPSV23) protects against 23 types.
The first three are conjugate vaccines (the “PCV” in their abbreviations). Pneumovax 23 is a polysaccharide vaccine, which works differently. That distinction matters more than the number of strains each one covers.
Conjugate vs. Polysaccharide: Why It Matters
Conjugate vaccines like Prevnar 20, Vaxneuvance, and Capvaxive attach a piece of the bacterial coating to a protein. This triggers a stronger, longer-lasting immune response because it activates two branches of your immune system. Your body creates memory cells that recognize the bacteria if you encounter it later, and it builds protection in the mucous membranes of your nose and lungs, where pneumonia-causing bacteria typically enter.
Pneumovax 23 skips the protein attachment. It presents the bacterial coating on its own, which activates only one branch of the immune system. The result is a shorter-lived response that prevents bacteria from spreading through the bloodstream but doesn’t protect the airways as effectively. This is why Pneumovax 23 is now used mainly as a follow-up dose rather than a standalone vaccine.
Who Needs a Pneumonia Vaccine
The CDC recommends a pneumococcal conjugate vaccine (Prevnar 20, Vaxneuvance, or Capvaxive) for all adults 50 and older who have never received one. Adults aged 19 through 49 also qualify if they have certain health conditions that raise their risk.
Those risk conditions include:
- Immune-related conditions: HIV, sickle cell disease, a missing or dysfunctional spleen, conditions treated with immunosuppressive drugs or radiation
- Chronic organ disease: chronic lung disease (including COPD, emphysema, and asthma), chronic heart disease (heart failure, cardiomyopathies), chronic liver disease, chronic kidney failure
- Other factors: diabetes, cochlear implants, cerebrospinal fluid leaks, alcoholism, cigarette smoking
If your doctor uses Vaxneuvance (PCV15), you’ll need a follow-up dose of Pneumovax 23 about one year later. If you receive Prevnar 20 or Capvaxive instead, no follow-up dose is needed. For people with immunocompromising conditions, cochlear implants, or cerebrospinal fluid leaks, the minimum wait between PCV15 and the Pneumovax 23 follow-up can be shortened to 8 weeks.
What If You’ve Already Been Vaccinated
Older adults who previously received the older Prevnar 13 (PCV13) along with Pneumovax 23 at age 65 or later have the option to get Prevnar 20 or Capvaxive for broader coverage. This isn’t a firm recommendation. It’s a shared decision you can make with your doctor based on your individual risk.
If you received Prevnar 20 or Capvaxive at any point, you’re considered up to date and don’t need additional pneumococcal doses.
Children Get a Different Schedule
Infants and young children receive pneumococcal conjugate vaccines as part of their routine immunization series, typically given across multiple doses in the first two years of life. Children with conditions like sickle cell disease, HIV, or cochlear implants may need additional doses or a different schedule than healthy children.
Side Effects Are Typically Mild
The most common reactions to any of the pneumococcal vaccines are soreness or swelling at the injection site, fatigue, headache, muscle pain, joint pain, fever, chills, and reduced appetite. In large studies comparing conjugate and polysaccharide vaccines in adults 50 and older, both types produced similar side effects. These reactions are mild and typically resolve within one to two days. Severe allergic reactions are rare.
Some people notice limited movement in the arm that received the shot. This is normal and clears up quickly. You don’t need to avoid activity afterward, though the arm may feel tender.
