The practice effect is the tendency for people to perform better on a test simply because they’ve taken it before, not because their actual ability has changed. This improvement comes from familiarity with the test format, the types of questions asked, and the strategies needed to answer them. It’s one of the most well-documented phenomena in psychological testing, and it has real consequences for everything from IQ scores to dementia diagnosis.
How the Brain Gets Faster With Repetition
When you perform any cognitive task repeatedly, your brain essentially builds shortcuts. Instead of working through the full problem-solving process each time, it stores the results of frequently used mental computations and retrieves them directly. Think of it like memorizing a route to work: the first time you drive it, you’re actively reading signs and making decisions at every turn. After a few weeks, you navigate almost without thinking.
This shortcut-building process produces three distinct changes in behavior. First, you get faster at selecting the right response. Second, the task requires less mental effort than it did initially. Third, your behavior becomes more automatic and habitual. These three changes are often bundled together under the label “automaticity,” but they each reflect a different practical consequence of the same underlying mechanism: your brain caching solutions it has already computed so it doesn’t have to compute them again.
The trade-off is flexibility. Because cached responses are based on past experience, they can become outdated if the task changes. Your brain may keep defaulting to old patterns even when a new approach would be more appropriate. This is why heavily practiced skills can feel rigid, and why breaking a well-rehearsed habit takes deliberate effort.
How Large the Effect Actually Is
The practice effect isn’t trivial. On a common nonverbal reasoning test, simply taking the same test a second time boosts scores by roughly eight IQ points. That’s a meaningful jump, enough to shift someone from the 50th percentile to around the 70th. Several rounds of repeated testing can produce similar or even larger gains, though scores eventually plateau.
The size of the effect also depends on the type of cognitive ability being tested. Research tracking over 1,600 adults found that practice effects varied across reasoning, spatial visualization, memory, processing speed, and vocabulary. They were consistently positive (meaning scores improved), but not equally so across all domains.
Who Benefits Most
Age is one of the strongest predictors of how much someone gains from prior test exposure. Younger adults tend to show larger practice effects than older adults. In that same large study, the correlation between age and the size of the practice effect was strongly negative for most abilities: -.89 for spatial skills, -.86 for memory, and -.80 for processing speed. In plain terms, the older you are, the less your scores tend to improve from simple re-exposure to the test.
This pattern likely reflects the same cognitive resources that decline with age. People with higher general cognitive ability, regardless of age, tend to show bigger short-term practice gains. The capacity to benefit from prior experience appears to be itself a cognitive skill, one that tracks closely with overall mental sharpness.
Why It Matters for Dementia Screening
The practice effect creates a serious blind spot in tracking cognitive decline. When someone at risk for Alzheimer’s disease or mild cognitive impairment takes the same battery of tests every six or twelve months, their familiarity with those tests can artificially inflate their scores. The result is that real, ongoing cognitive decline gets partially or fully hidden behind practice-related gains.
Research on serial cognitive assessments has found that practice effects produce artifactual improvements across a wide range of measures, including processing speed, memory, executive function, and working memory. In some studies, these gains were large enough to reduce or eliminate what should have been observable age-related decline over several years. Even when decline does show up in the data, it’s often an underestimate of the true loss in cognitive ability.
This problem is especially damaging in clinical drug trials. If both the treatment group and the placebo group improve over time due to practice effects, the actual benefit of the drug gets buried. The improvement within each group (driven partly by test familiarity) can dwarf the difference between groups (which is what matters for evaluating the treatment). Researchers have estimated that practice effects in these studies correspond to a standardized effect size of about 0.25 in healthier populations, enough to mask a small but genuine treatment benefit and lead to the false conclusion that a drug doesn’t work.
How Long Practice Effects Last
Practice effects operate on multiple timescales. A study of older adults at risk for Alzheimer’s found measurable improvement at every interval tested: within individual test sessions, across days within a testing week, and across visits spaced six months apart. The biggest gains tended to occur on the first day of testing, with diminishing returns after that.
Retention of those gains also varies by cognitive health. Participants with early signs of cognitive impairment lost more of their practice-related improvement between six-month visits than cognitively healthy participants did. This differential “fade” is itself a potentially useful signal. The rate at which someone retains or loses practice gains may tell clinicians something meaningful about the trajectory of their brain health.
Can Alternate Test Versions Solve the Problem?
The most common strategy for reducing practice effects is to use alternate forms of a test. Instead of giving someone the same word list or puzzle twice, you swap in a parallel version with different content but the same structure and difficulty level. In theory, this removes the benefit of remembering specific answers while keeping the measurement consistent.
In practice, this approach helps but doesn’t fully solve the problem. Alternate forms can reduce the advantage of remembering specific test items, but they can’t eliminate the broader benefits of test familiarity: knowing what to expect, feeling less anxious, understanding the pacing, having refined your strategy. These general testing factors contribute to practice effects independently of the specific content.
Worse, alternate forms sometimes introduce new problems. If the two versions aren’t perfectly equivalent in difficulty, which is hard to guarantee, then any change in score could reflect a difference between the tests rather than a change in the person. Studies using alternate word lists to track cognition over time have found that swapping forms can complicate the interpretation of results rather than clarifying it.
Statistical Corrections
Clinicians and researchers also use mathematical tools to adjust for practice effects after the fact. The most widely used is the Reliable Change Index, a formula that determines whether the difference between two test scores is large enough to reflect genuine change rather than normal variability and practice-related gains.
The basic version, introduced in 1991, compares an individual’s score change to the amount of variation you’d expect if nothing had actually changed. Later refinements added a correction specifically for practice effects: instead of just looking at the raw difference between the two scores, the formula subtracts the average practice effect observed in a comparison group. What remains, if anything, is the change that can’t be explained by simple retest improvement. The result is expressed as a standardized score that tells clinicians whether the change is statistically meaningful.
These corrections improve accuracy, but they rely on having good normative data about how much practice effects typically boost scores for someone of a given age and cognitive profile. Since practice effects vary by person, by test, and by the interval between tests, no single correction factor works perfectly in every case.
