The Swiss Cheese Model is a way of explaining how accidents happen in complex systems. Rather than blaming a single person or event, it shows that disasters occur when multiple safety barriers fail at the same time, allowing a hazard to pass through every layer of defense. The model was proposed by psychologist James Reason in his 1990 book Human Error and has since become one of the most widely used frameworks in safety science, from aviation to healthcare to nuclear power.
How the Model Works
Picture several slices of Swiss cheese lined up in a row. Each slice represents a different layer of defense in a system: training, equipment checks, safety protocols, supervision, alarms, and so on. In a perfect world, each slice would be solid and would stop any hazard from getting through. But in reality, every layer has weaknesses, represented by the holes in the cheese.
Most of the time, the holes in different slices don’t line up. A nurse might skip a step in a checklist (hole in one layer), but a pharmacist catches the error during review (the next layer holds). An accident happens only when the holes in every slice align at the same moment, creating a clear path for a hazard to travel all the way through every barrier and cause harm. This alignment is rare, which is why major accidents are rare, but when it happens the results can be catastrophic.
The key insight is that the holes aren’t static. They shift in size and position over time as conditions change: staff turnover, budget cuts, fatigue, new equipment, shifting workloads. This constant movement is what makes the occasional alignment so hard to predict.
Active Failures vs. Latent Conditions
Reason drew an important distinction between two types of errors that create those holes. Active failures are the immediate, visible mistakes made by the person at the sharp end of the system: the pilot who misreads an instrument, the surgeon who operates on the wrong side, the technician who skips a safety check. These errors are easy to spot because they happen right before the accident.
Latent conditions are harder to see. They’re flaws baked into the system itself: poor equipment design, understaffing, inadequate training programs, confusing communication protocols, or administrative decisions made months or years earlier. A faulty ventilator sitting in a hospital is a latent condition. It might never cause harm if someone catches the problem during a routine check. But if that check gets skipped (an active failure triggered by fatigue or distraction), the latent flaw suddenly matters.
The model’s central argument is that behind every catastrophic failure, there’s an underlying fault in the system that gets triggered by an act of omission or commission from the people directly involved. Blaming the individual who made the active error misses the bigger picture. The real question is: what conditions in the system made that error possible, likely, or invisible?
Types of Human Error
The model also breaks down how people make mistakes in the first place. Slips happen when you know the right plan but your execution goes wrong, often because of fatigue, distractions, or noise. You meant to push the correct button but hit the one next to it. Lapses are memory failures: you intended to do something but forgot a step. Mistakes are more fundamental. They happen when the plan itself is wrong, either because of a knowledge gap, insufficient training, or fixating on one explanation when the real problem is something else entirely.
Each type of error calls for a different kind of fix. Slips and lapses respond well to better working conditions: shorter shifts, fewer interruptions, clearer instrument layouts. Mistakes require better training, simulation practice, and decision-support tools like checklists and algorithms that reduce reliance on memory alone. Communication failures, which Reason classified as latent errors, need structured handoff protocols and team communication habits.
Where the Model Gets Used
Aviation was one of the first fields to adopt the Swiss Cheese Model. When investigators analyze a plane crash, they don’t just look at what the pilot did in the final seconds. They trace the chain backward through maintenance decisions, airline scheduling practices, regulatory oversight, and aircraft design. Each of those represents a defensive layer, and the investigation asks where and why the holes aligned.
Healthcare picked up the framework to understand medical errors, which are a leading cause of preventable death. A wrong-site surgery, for instance, involves failures at multiple levels: the consent process, the surgical briefing, the operating room checklist, and the physical marking of the correct site. If any one of those layers catches the mistake, the patient is safe. The model helped shift hospital safety culture away from punishing individual clinicians and toward redesigning systems so errors get caught before they reach patients.
During the COVID-19 pandemic, the model gained widespread public attention as a way to explain layered defenses against viral spread. No single measure (masks, vaccines, ventilation, hand hygiene, physical distancing, testing) is perfect on its own. Each one is a slice of cheese with holes. But stacking multiple imperfect layers together dramatically reduces the chance that the virus passes through all of them.
How It’s Used in Investigations
When something goes wrong, the Swiss Cheese Model gives investigators a structured way to work backward from the accident. Instead of stopping at the person who made the final mistake, they examine each defensive layer in the system and ask: did this barrier exist? Was it functioning? If not, why not? The goal is to identify not just what happened, but the organizational and systemic conditions that allowed it to happen.
This approach often reveals that the “root cause” isn’t a single failure but a combination of weaknesses across multiple layers. A hospital might discover that a medication error involved an unclear label design (latent condition), a pharmacist working a double shift (fatigue contributing to a slip), and a barcode scanning system that was offline for maintenance (a temporarily missing defense layer). Fixing only one of those leaves the system vulnerable the next time a different combination of holes aligns.
Criticisms and Limits
The Swiss Cheese Model isn’t without its critics. Some safety researchers argue that the model is too linear: it implies hazards travel in a straight line through neatly stacked barriers, which oversimplifies how real accidents unfold in complex, interconnected systems. In practice, failures often involve feedback loops, unexpected interactions between components, and emergent behaviors that don’t fit the simple “holes lining up” metaphor.
The model developed over roughly a decade and was shaped by intense intellectual debate among researchers in the 1980s, including figures like Jens Rasmussen, Charles Perrow, and Barry Turner, each of whom brought different perspectives on how systems fail. Some critics note that the model works better as a retrospective tool (explaining accidents after they happen) than as a predictive one (identifying which combination of weaknesses will cause the next accident).
Reason himself continued refining the ideas. His 2016 book Organizational Accidents Revisited extended the framework by analyzing accidents across multiple industries and advocating for integrating systemic safety factors with what he called “personal mindfulness,” the mental skills individuals use to catch and correct errors in real time. This update acknowledged that system design and individual awareness both matter, and neither alone is sufficient.
Why It Still Matters
Despite its simplicity, the Swiss Cheese Model endures because it communicates a powerful idea in an intuitive way: safety depends on multiple overlapping defenses, not on any single person doing their job perfectly. It shifted the conversation in high-risk industries from “who screwed up?” to “how did the system allow this to happen?” That reframing has saved lives by directing resources toward systemic fixes (better checklists, smarter equipment design, improved staffing policies) rather than simply disciplining the last person in the chain.
For anyone working in or thinking about safety, the core lesson is straightforward. No single barrier is reliable enough to stand alone. The more independent layers of defense you build into a system, the less likely it is that one bad moment, one tired worker, or one overlooked flaw will be enough to cause a disaster.
