Vivaer is an in-office procedure that uses low-energy radiofrequency waves to reshape the nasal valve, the narrowest part of your nasal airway, so you can breathe more easily through your nose. The treatment takes roughly 15 to 20 minutes, requires only local anesthesia, and most people return to normal activities the same day. It was designed as a less invasive alternative to traditional nasal surgery for people whose breathing problems stem from a weak or collapsed nasal valve.
How the Procedure Works
The Vivaer device delivers controlled radiofrequency energy just beneath the surface of the tissue along the lateral nasal wall, right where the upper and lower cartilages of the nose meet. That energy heats the tissue to about 60°C, which causes existing collagen proteins in the cartilage to contract and tighten. At the same time, it stimulates your body to produce new collagen in the area. The combined effect gently stiffens and reshapes the nasal valve, widening the passage so air flows through more freely.
A small wand-shaped stylus is inserted into each nostril, and the doctor applies several brief pulses of energy (each lasting about 18 seconds) to different spots along the nasal wall. You’re awake the entire time. Before the procedure begins, your doctor numbs the inside of your nose with a topical anesthetic spray or gel, then injects a small amount of local anesthetic into the treatment area. Most patients feel mild pressure or warmth but not significant pain.
Who It’s Designed For
Vivaer is specifically for people whose nasal obstruction is caused by nasal valve collapse, not for every type of stuffy nose. The nasal valve is the small triangular opening inside each nostril formed by cartilage and soft tissue. In some people, these structures are weak or positioned in a way that causes them to cave inward during breathing, partially blocking airflow. You might notice this as a sensation of breathing through a narrow straw, or your nostrils visibly pinching shut when you inhale.
Your ENT doctor can identify nasal valve collapse during a physical exam, often using a simple test: if gently pulling the side of your nose outward with a finger or a cotton swab dramatically improves your breathing, the valve is likely the problem. In the key clinical trial (called AERWAY), patients enrolled had severe or extreme scores on the NOSE scale, a standardized questionnaire that rates nasal obstruction from 0 to 100, with higher numbers meaning worse symptoms. Nasal valve collapse had to be a primary or significant contributor to their breathing difficulty.
Vivaer is not the right fit if your congestion is mainly caused by enlarged turbinates (the spongy structures deeper inside the nose), a deviated septum, or chronic sinus inflammation. Those conditions have their own treatments. Some people have more than one issue contributing to their obstruction, and a doctor may recommend Vivaer alongside other procedures.
How Well It Works
Clinical trials show meaningful and lasting improvement for most patients. In a study published in JAMA Otolaryngology, average NOSE scores improved by about 54% at three months, 57% at six months, and 59% at twelve months. The fact that scores continued to improve slightly over time suggests the tissue remodeling process keeps working for months after treatment.
Longer follow-up data is encouraging. A multicenter trial tracking patients for two full years found that improvements held steady, with a 66.5% average improvement in NOSE scores at 24 months. At that point, 97% of participants still qualified as responders, meaning they had maintained at least a 15-point improvement in their scores. Before treatment, all participants had severe or extreme nasal obstruction. By the two-year mark, only 17% remained in those categories.
These results come from single-arm studies without a placebo comparison group, which is worth keeping in mind. But the consistency of improvement across multiple time points and study sites suggests the effect is real and durable for the majority of patients.
Recovery and Side Effects
Most people go back to their normal routine immediately or within a day. There’s no general anesthesia, no incisions, and no packing in the nose.
That said, your nose will need time to heal internally. Temporary congestion, increased sensitivity, and crusting inside the nose are common for about two weeks, though these symptoms can linger up to six weeks in some cases. Other possible side effects include mild bleeding, temporary numbness or tingling at the treatment site, soreness, swelling, and occasionally bruising around the eyes. These are generally mild and resolve on their own.
During recovery, you’ll want to blow your nose gently, avoid pinching or pressing on the treatment area, and use any saline rinses or nasal sprays your doctor recommends. Less common but more serious complications include infection, significant bleeding, scar tissue that could actually worsen obstruction, or changes to the external shape of the nose.
How It Compares to Other Options
Before Vivaer existed, the main surgical option for nasal valve collapse was functional rhinoplasty or septorhinoplasty, both of which are performed in an operating room under general anesthesia with longer recovery times. Vivaer offers a much less invasive path for appropriate candidates.
Another in-office option is LATERA, a small bioabsorbable implant that gets placed inside the lateral nasal wall to physically prop it open. While LATERA also targets nasal valve collapse, it works mechanically (like an internal nasal strip) rather than by remodeling tissue. The implant dissolves over about 18 months, but the structural support it provides is meant to persist through scar tissue formation.
If your obstruction comes from swollen turbinates rather than a collapsing valve, radiofrequency turbinate reduction is a different in-office procedure that shrinks that tissue. It uses similar energy technology but targets a completely different structure inside the nose. Your doctor may recommend turbinate reduction alone, Vivaer alone, or both together depending on what’s causing your symptoms.
Insurance Coverage and Cost
Vivaer has its own dedicated CPT billing code (30469), which is a positive sign for insurance coverage since it means insurers formally recognize the procedure. However, coverage varies significantly by plan. Some major insurers, including Anthem, have classified low-dose radiofrequency nasal tissue remodeling as investigational and not medically necessary, which means they may deny claims. Other insurers do cover it, particularly as more long-term data becomes available.
If your insurance denies coverage, out-of-pocket costs typically range from $1,500 to $3,000 depending on your provider and location. It’s worth having your doctor’s office submit a prior authorization request before scheduling, as coverage decisions often depend on your specific plan, your documented NOSE score, and whether you’ve tried conservative treatments like nasal dilators or steroid sprays first.
