A fasting blood sugar of 100 mg/dL or higher is the first sign that levels are creeping too high. At 126 mg/dL or above on two separate fasting tests, you’re in the diabetes range. But “too high” depends on when you last ate, whether you already have diabetes, and how long your levels have been elevated. Here’s how to make sense of the numbers.
Fasting Blood Sugar Thresholds
Fasting blood sugar is measured after at least eight hours without food, typically first thing in the morning. The ranges break down clearly:
- Normal: below 100 mg/dL
- Prediabetes: 100 to 125 mg/dL
- Diabetes: 126 mg/dL or higher on two separate tests
A single high reading doesn’t automatically mean diabetes. Illness, stress, poor sleep, and certain medications can all push fasting glucose above normal temporarily. That’s why a diabetes diagnosis requires confirmation with a second test. But if your fasting levels consistently land between 100 and 125, your body is already struggling to regulate glucose, and the risk of progressing to type 2 diabetes is real.
Blood Sugar After Eating
Your blood sugar naturally rises after a meal and typically peaks about one to two hours later. For someone without diabetes, a reading below 140 mg/dL at the two-hour mark is normal. If you have diabetes, the goal is to stay below 180 mg/dL after meals.
Post-meal spikes can be surprisingly high even in healthy people. A Stanford Medicine study found that more than half of participants with normal prior blood sugar tests spiked into prediabetic or diabetic ranges after certain meals. Eighty percent of participants spiked after eating a bowl of cornflakes with milk. The size of those spikes varied widely between individuals, driven by genetics, gut microbiome composition, and the specific type of carbohydrate consumed. So a single high reading after a carb-heavy meal isn’t necessarily a sign of disease, but repeated spikes in that range deserve attention.
A1C: The Bigger Picture
While a finger-stick glucose test captures a single moment, the A1C test reflects your average blood sugar over the past two to three months. It measures the percentage of your red blood cells that have glucose attached to them, giving a much broader view of how well your body is managing sugar over time.
- Normal: below 5.7%
- Prediabetes: 5.7% to 6.4%
- Diabetes: 6.5% or higher
If you’re already managing diabetes, the American Diabetes Association recommends keeping your A1C below 7%, which corresponds to pre-meal glucose between 80 and 130 mg/dL and post-meal glucose under 180 mg/dL. Your provider may adjust those targets based on your age, health conditions, and risk of low blood sugar episodes.
Early Symptoms of High Blood Sugar
Mildly elevated blood sugar often produces no symptoms at all, which is why so many people with prediabetes don’t know they have it. As levels climb higher, the earliest warning signs tend to be:
- Increased thirst and hunger
- Frequent urination
- Headaches
- Blurred vision
These happen because your kidneys work overtime to filter excess glucose out of your blood, pulling water with it. You urinate more, get dehydrated, and feel thirsty. The blurred vision comes from fluid shifts in the lens of your eye caused by high glucose concentrations.
When blood sugar stays elevated for weeks or months, a different set of symptoms appears: persistent fatigue, unexplained weight loss, slow-healing cuts, recurring skin or yeast infections. These signal that chronically high glucose is starting to interfere with your body’s ability to fight infection and repair tissue.
When High Blood Sugar Becomes Dangerous
Blood sugar above 250 mg/dL moves into territory that requires close monitoring. At that level, the CDC recommends checking your blood sugar every four to six hours and testing your urine for ketones. Ketones are acids your body produces when it starts burning fat for energy instead of glucose, a sign that insulin isn’t doing its job. Elevated ketones combined with high blood sugar can lead to diabetic ketoacidosis (DKA), a medical emergency most common in type 1 diabetes but possible in type 2 as well.
Symptoms of DKA include nausea and vomiting, abdominal pain, rapid deep breathing, fruity-smelling breath, and confusion. If your blood sugar stays at 300 mg/dL or above, your breath smells fruity, or you can’t keep food or drinks down, that’s an emergency room situation.
An even more extreme condition called hyperosmolar hyperglycemic syndrome can develop in type 2 diabetes, with blood sugar sometimes climbing above 600 mg/dL. At these levels, severe dehydration causes the blood to become dangerously concentrated with glucose and salts. This condition develops more gradually than DKA, often over days or weeks, and typically affects older adults. It carries a high mortality rate without treatment.
What Prolonged High Blood Sugar Does to Your Body
The real danger of high blood sugar isn’t a single spike. It’s the cumulative damage from months and years of elevated glucose circulating through your bloodstream. That excess glucose triggers three interconnected processes: chronic inflammation, damage to the inner lining of blood vessels, and oxidative stress, where harmful molecules overwhelm your cells’ ability to neutralize them.
In large blood vessels, this accelerates atherosclerosis, the hardening and narrowing of arteries. That raises the risk of heart attack, stroke, and peripheral artery disease. In small blood vessels like the capillaries, the damage shows up as kidney disease, vision loss from retinal damage, and nerve damage. Roughly half of people with diabetes develop some degree of peripheral neuropathy, which causes numbness, tingling, or pain in the hands and feet. The excess glucose damages the protective coating around nerve fibers and injures the specialized cells that maintain them, leading to progressive, irreversible nerve loss that typically starts in the toes and works its way up.
Common Causes of Temporary Spikes
Not every high blood sugar reading means something is wrong with your metabolism. Several everyday factors can push glucose above normal ranges temporarily:
- Stress: Physical or emotional stress triggers your body to release hormones that raise blood sugar as part of the fight-or-flight response.
- Illness or infection: Even a common cold causes your body to release glucose into the bloodstream to fuel your immune system.
- High-carbohydrate meals: Refined carbs like white bread, cereal, and sugary drinks cause rapid, steep spikes compared to foods with fiber, fat, or protein.
- Poor sleep: Even a single night of inadequate sleep reduces your body’s sensitivity to insulin the next day.
- Medications: Steroids, certain blood pressure medications, and some antidepressants can raise blood sugar.
If you’re seeing occasional spikes tied to specific meals or stressful days, that’s a normal physiological response. What matters is where your levels settle the rest of the time and what your A1C looks like over months. A pattern of consistently elevated fasting readings, even if each one is only slightly above 100, is more concerning than one dramatic post-meal spike after a plate of pancakes.
