What Is TRE Therapy? Exercises, Uses, and Evidence

TRE stands for Tension and Trauma Releasing Exercises, a series of seven simple movements designed to trigger involuntary shaking in your body as a way to release deep muscle tension and reduce stress. Developed by David Berceli, PhD, the method is built on the idea that this shaking, called neurogenic tremoring, is a natural mechanism the nervous system uses to calm itself after stress or danger. You don’t need any special equipment, and once you learn the sequence, you can practice it on your own.

How TRE Works in the Body

The core idea behind TRE is that your body already knows how to discharge stress physically. Animals do this instinctively: after a predator chase, an animal’s heart rate, blood pressure, and nervous system activation spike to help it survive. Once the threat passes, the animal begins to tremor, and those tremors bring heart rate, blood pressure, and nervous system arousal back down to baseline. The animal returns to a calm state. Berceli’s argument is that humans have this same built-in mechanism but tend to suppress it, often because shaking feels socially awkward or is mistaken for weakness.

The exercises target the psoas muscle, a deep hip flexor that connects your spine to your legs and plays a central role in your body’s protective “fetal” response to danger (curling up, pulling the knees in, bending forward). When you feel threatened, this muscle contracts and tightens. TRE reverses that posture. You open the legs, lift the pelvis, and lie flat, essentially signaling to your nervous system that you’re safe. This open position appears to activate the tremor mechanism, and once it starts, the shaking tends to move on its own through the legs, pelvis, spine, and sometimes into the shoulders and arms.

Berceli describes this as a genetically encoded mechanism that reduces tightness in muscle tissue and connective tissue (fascia), and can even help realign the hips and spinal column. Because the nervous system connects to every other system in the body, the theory is that releasing tension through tremoring can also affect digestion, breathing, and organ function more broadly.

The Seven Exercises

A TRE session follows a specific sequence of seven exercises. The first several are designed to fatigue your leg and hip muscles, which makes the tremor response easier to activate. The final exercises put you in positions where tremoring can begin and spread naturally.

  • Standing adductor stretch: Feet wide apart, toes turned slightly inward, leaning forward. Hold for about 90 seconds.
  • Calf raise holds: Lift your heels off the ground, hold for 30 seconds, and repeat three times.
  • Deep squat hold: Sink into a deep squat with arms extended forward. Hold for 60 to 90 seconds.
  • Bridge lift pulses: Lying on your back with knees bent, pulse your hips upward 20 times, then hold a bridge position for 60 seconds.
  • Legs-up diamond: Bring the soles of your feet together with knees wide. This is often where tremoring begins.
  • 90-90 bent-knee hold: Knees stacked over hips, shins parallel to the floor. You observe the shaking as it develops.
  • Reverse butterfly: Feet flat on the mat, knees falling together. Tremors often migrate up the spine during this position.

The first four exercises do the physical work of tiring the muscles. The last three are passive, meaning you simply lie still and let the tremoring happen. Many people report that the shaking starts in their legs and gradually moves upward through the pelvis and along the spine.

What TRE Is Used For

TRE is classified as a mind-body therapy. People use it for both physical and psychological complaints, though the line between those categories blurs quickly with this type of approach. On the physical side, practitioners and some early research suggest it may help with chronic pain, muscle tightness, spasticity, fatigue, sleep disturbance, and general physical tension. On the psychological side, it’s used to address stress, anxiety, and depression, and it has been explored in populations dealing with PTSD symptoms.

A pilot study published in Global Advances in Health and Medicine looked at TRE’s effects on non-professional caregivers, a group that typically carries significant chronic stress. The study found that self-induced tremoring helped mitigate the experience of excess stress and improved quality of life measures. The researchers described the tremors as a “natural neurophysiological response to mitigate excess stress,” though they also noted the tremors remain formally unclassified in medical literature.

An exploratory pilot study on people with multiple sclerosis examined whether TRE could help with symptoms like spasticity, fatigue, walking difficulty, pain, sleep quality, bladder and bowel dysfunction, and overall stress levels. The rationale was that because TRE targets deep muscle tension and nervous system regulation, it might address the broad constellation of symptoms MS patients experience. Broader reviews of mind-body therapies for MS have noted potential benefits for depression, anxiety, fatigue, and quality of life, though the research base for TRE specifically remains small.

What the Evidence Looks Like

The honest picture is that TRE’s research base is still in its early stages. The published studies are mostly pilot studies with small sample sizes and no control groups, which means they can suggest benefit but can’t prove it. No large randomized controlled trials have been completed. The biological mechanism, while plausible and consistent with what we know about the nervous system’s stress response, hasn’t been rigorously tested in the way that would satisfy most clinical researchers.

That said, the anecdotal reports are widespread and consistent. People who practice TRE regularly tend to describe reduced muscle tension, better sleep, a sense of calm, and in some cases relief from chronic pain. These reports align with the proposed mechanism but aren’t a substitute for controlled evidence. If you’re considering TRE for a serious condition like PTSD, it’s worth understanding that more established therapies have stronger evidence behind them, and TRE might work best as a complement rather than a replacement.

What a Session Feels Like

If you’ve never experienced neurogenic tremoring before, the sensation can feel strange. Most people describe it as involuntary but not uncomfortable. Your legs may shake visibly, sometimes vigorously, and you might feel vibrations moving through your hips, abdomen, or back. The shaking is not something you consciously produce or control, though you can stop it at any time by straightening your legs or changing position.

Beginners are generally advised to keep their tremoring sessions short, around 15 minutes of actual tremoring time (not counting the warm-up exercises). The reason is that releasing too much tension at once can occasionally leave people feeling emotionally overwhelmed or physically sore. As you become more experienced, you develop a better sense of how much tremoring feels right for your body in a given session. Most practitioners recommend two to three sessions per week, with rest days in between to let your nervous system integrate the changes.

Learning TRE Safely

TRE for All, Inc., the nonprofit organization that oversees TRE globally, runs a Global Certification Training Program for providers. You must complete this program to teach TRE professionally. For people who just want to learn the technique for personal use, the typical path is attending a workshop or working one-on-one with a certified provider for a few sessions until you’re comfortable with the exercises and the tremoring process.

Once you’ve learned the sequence, TRE is designed to be a self-help tool. You can do it at home on a yoga mat with no equipment. This is one of its main selling points: unlike therapy sessions or bodywork appointments, the ongoing cost is zero, and you control the timing and intensity. People with a history of significant trauma or those taking psychiatric medication should work with a certified provider rather than learning from videos alone, because intense tremoring can sometimes surface strong emotional responses that benefit from professional support.
