Vasculitis of the legs is inflammation of the blood vessels in the lower extremities, where the immune system mistakenly attacks vessel walls and damages them. This can restrict blood flow to the skin and surrounding tissue, causing visible changes like raised purple or red spots, skin ulcers, and pain. The legs are one of the most common sites for vasculitis to appear because gravity naturally increases pressure in the blood vessels below the knees.
What Happens Inside the Blood Vessels
In vasculitis, white blood cells flood into the walls of blood vessels and begin destroying them from within. Early on, a specific type of white blood cell called a neutrophil drives the attack. Over time, other immune cells take over the process. The inflammation can affect just a segment of a vessel or run along its entire length, and it can target one or more layers of the vessel wall.
As the vessel wall breaks down, blood leaks into the surrounding skin, producing the characteristic purplish spots. If the inflammation is severe enough, the vessel narrows from internal scarring and clot formation, choking off blood flow to the tissue it supplies. That restricted flow can cause tissue death, which is how vasculitic skin ulcers form.
Types That Commonly Affect the Legs
Most vasculitis in the legs involves the smallest blood vessels: capillaries and tiny veins in the skin. This is called cutaneous small vessel vasculitis, and it’s the type responsible for the classic rash of raised, reddish-purple bumps on the lower legs. Under a microscope, pathologists see fragments of broken-down immune cells scattered around the damaged vessels, a pattern known as leukocytoclastic vasculitis.
IgA vasculitis (previously called Henoch-Schönlein purpura) is another common form that targets small vessels and frequently shows up on the legs and buttocks. It involves a specific antibody, IgA, depositing in vessel walls and triggering inflammation. It’s more common in children, though adults can develop it too.
When medium-sized arteries deeper in the skin are involved, as in polyarteritis nodosa, the appearance shifts. Instead of small raised spots, you may see a lace-like purplish pattern on the skin (called livedo reticularis), painful nodules, or deeper ulcers. These arteries sit in the lower layers of the skin and subcutaneous fat, so the damage goes deeper.
What Leg Vasculitis Looks and Feels Like
The hallmark sign is palpable purpura: small, raised, bruise-like spots that you can feel with your fingertips. They typically cluster on the lower legs and ankles, and they don’t blanch (turn white) when you press on them. Fresh spots tend to be red or reddish-purple, while older ones darken to brown as the leaked blood breaks down.
Beyond the rash, you may notice burning, itching, or aching in the affected areas. Some people develop blisters or shallow ulcers, particularly when the inflammation is severe or ongoing. In medium vessel disease, deeper nodules can form under the skin, and the lace-like discoloration of livedo reticularis may spread across larger areas of the leg. Skin that loses its blood supply entirely can turn dark and form areas of dead tissue that require wound care.
Common Triggers and Causes
Vasculitis in the legs can be triggered by infections, medications, autoimmune diseases, or sometimes no identifiable cause at all. Infections like hepatitis B and C are well-established triggers. The immune system’s response to the infection produces clumps of antibodies and viral proteins that lodge in small blood vessels and spark inflammation.
A range of medications can also set it off. Antibiotics, NSAIDs (like ibuprofen and naproxen), and certain blood pressure medications are among the more common culprits. Hydralazine, propylthiouracil, and cocaine are particularly associated with a more serious form of drug-induced vasculitis that involves the production of specific autoantibodies. Minocycline, a common acne antibiotic, has been linked to polyarteritis nodosa affecting the skin.
Underlying autoimmune conditions like rheumatoid arthritis or lupus can also cause vasculitis in the legs as part of broader disease activity. In some cases, vasculitis signals a hidden malignancy, which is one reason thorough evaluation matters even when the skin findings seem mild.
How It’s Diagnosed
A skin biopsy is the most definitive way to confirm vasculitis. A small sample of affected skin is removed and examined under a microscope for the characteristic pattern of vessel wall inflammation and immune cell debris. Getting the biopsy depth right matters: if the suspected type involves medium-sized arteries (like polyarteritis nodosa), a shallow biopsy that only captures the surface layers of skin will miss the affected vessels entirely. The sample needs to reach the deep dermis and subcutaneous fat to be diagnostic.
Blood work typically accompanies the biopsy. Tests for specific autoantibodies help classify the type of vasculitis and determine whether internal organs are involved. A urinalysis checks for kidney involvement, since some forms of small vessel vasculitis can damage the kidneys alongside the skin. The goal is to distinguish isolated skin disease from vasculitis that’s part of a systemic condition affecting multiple organs.
Conditions That Can Look Similar
Several other conditions cause reddish or purplish discoloration on the lower legs and can be mistaken for vasculitis. Stasis dermatitis, caused by poor circulation and pooling of blood in the veins, produces bilateral brownish discoloration and swelling that overlaps visually with healing vasculitis. Pigmented purpuric dermatoses create tiny reddish-brown dots on the lower legs from red blood cells leaking out of very superficial vessels, but without the true vessel wall destruction seen in vasculitis.
Cellulitis, a bacterial skin infection, can also mimic vasculitis with redness and swelling, but it’s typically one-sided, tender, and accompanied by fever. These distinctions are one reason biopsy is so valuable: looking at the tissue under a microscope separates true vasculitis from its mimics with much greater certainty than visual inspection alone.
Treatment and What to Expect
Most cases of isolated cutaneous vasculitis in the legs are self-limited, meaning the inflammation resolves on its own within three to four weeks without systemic treatment. For mild cases, the primary approach is supportive: leg elevation, compression if appropriate, and good skin care to prevent secondary infection of any open sores.
When symptoms are more severe, with significant pain, widespread lesions, or ulceration, treatment escalates. Anti-inflammatory medications are often the first step. For IgA vasculitis, NSAIDs can manage joint pain and mild symptoms. When that’s not enough, corticosteroids are the standard next step, typically started at a high dose and gradually tapered over weeks to months.
For types of vasculitis that are chronic, recurring, or involve internal organs, stronger immune-suppressing medications become necessary. These aim to shut down the overactive immune response more completely. The specific approach depends on the type and severity: some patients do well with milder agents, while others with aggressive disease require more potent therapy. If a medication is identified as the trigger, stopping it is usually the most effective treatment.
Recovery and Long-Term Outlook
The prognosis for vasculitis limited to the skin of the legs is generally favorable. Skin lesions often regress spontaneously within weeks to months. However, “favorable” doesn’t always mean easy. Painful, itchy, or ulcerating lesions can significantly affect quality of life while they’re active, and some people experience chronic or recurring episodes that come and go over months or years.
Skin lesions that ulcerate carry a risk of secondary infection, so keeping wounds clean and monitored is important during active flares. Discoloration from healed lesions can linger for months after the inflammation itself has resolved, as the breakdown products of leaked blood slowly clear from the skin. For people with recurrent disease, working with a specialist to identify and address underlying triggers (whether that’s an infection, a medication, or an autoimmune condition) is the most effective way to reduce future flares.
