Zarxio is a medication that boosts your body’s production of white blood cells, specifically neutrophils. It’s used primarily to prevent dangerous infections in people whose immune systems have been weakened by chemotherapy, bone marrow transplants, or chronic blood disorders. Approved in 2015 as the first biosimilar in the United States, Zarxio is a near-identical version of the brand-name drug Neupogen (filgrastim).
How Zarxio Works
Zarxio belongs to a class of drugs called granulocyte colony-stimulating factors, or G-CSFs. It works by signaling your bone marrow to produce more neutrophils, the white blood cells that serve as your body’s front-line defense against bacterial and fungal infections. Beyond just increasing production, Zarxio also speeds up the maturation of developing neutrophils and triggers their release into the bloodstream, where they can fight off pathogens.
This matters because many cancer treatments, radiation exposure, and certain genetic conditions can cause neutrophil counts to drop dangerously low, a condition called neutropenia. Without enough neutrophils, even a minor infection can become life-threatening. Zarxio essentially tells the bone marrow to work harder and faster to rebuild that defense.
FDA-Approved Uses
Zarxio is approved for six distinct medical situations, all centered on either preventing or shortening periods of dangerously low white blood cell counts.
Chemotherapy-Induced Neutropenia
The most common use is in cancer patients receiving chemotherapy. Many chemotherapy drugs destroy fast-dividing cells indiscriminately, wiping out neutrophils along with cancer cells. Zarxio is given to reduce the risk of febrile neutropenia, which is when a patient develops a fever while their neutrophil count is critically low. This is one of the most dangerous complications of chemotherapy and a leading cause of treatment delays and hospitalizations.
Acute Myeloid Leukemia Treatment
Patients undergoing intensive chemotherapy for acute myeloid leukemia (AML) face especially prolonged periods of neutropenia. Zarxio shortens the time it takes for neutrophils to recover after induction or consolidation chemotherapy cycles, which also reduces the duration of fever during that vulnerable window.
Bone Marrow Transplants
After a bone marrow transplant, the body needs time to rebuild its blood cell production from scratch. Zarxio helps shorten that dangerous gap by stimulating the new marrow to produce neutrophils faster, reducing the window where patients are most susceptible to infections.
Stem Cell Collection
Zarxio has a different role before certain transplant procedures. It pushes stem cells out of the bone marrow and into the circulating blood, where they can be collected through a filtering process. These collected stem cells are then transplanted back into the patient (or into a donor recipient) after high-dose chemotherapy.
Severe Chronic Neutropenia
Some people are born with or develop persistently low neutrophil counts unrelated to cancer treatment. This includes congenital neutropenia (present from birth), cyclic neutropenia (counts that rise and fall in regular patterns), and idiopathic neutropenia (low counts with no identifiable cause). These patients may take Zarxio on an ongoing basis to keep neutrophil levels high enough to prevent recurring fevers, infections, and mouth ulcers.
Acute Radiation Exposure
In the rare event of exposure to high doses of radiation, such as a nuclear accident, Zarxio is approved to increase survival. Radiation can devastate the bone marrow, and Zarxio helps the body regenerate its ability to produce white blood cells.
Zarxio vs. Neupogen
Zarxio (filgrastim-sndz) is a biosimilar to Neupogen (filgrastim), meaning it was developed to be highly similar in structure, function, and clinical effect. It shares five of the same FDA-approved indications. A biosimilar isn’t a generic in the traditional sense, since biologic drugs are made from living cells and can’t be exactly replicated. But clinical studies confirmed that Zarxio performs comparably to Neupogen, with no meaningful differences in safety or effectiveness. For most patients, the practical difference comes down to cost, as biosimilars typically offer a lower price point.
Common Side Effects
In combined clinical trials, about 21% of patients experienced side effects considered related to the drug. The most frequent complaints were musculoskeletal: bone pain (7.2%), muscle pain (7.2%), general musculoskeletal pain (1.8%), and joint pain (1.8%). These effects make sense given that the drug is essentially pushing your bone marrow into overdrive. The bone pain is often felt in the hips, legs, or lower back and tends to be manageable with over-the-counter pain relievers.
Other reported side effects included reactions at the injection site, digestive issues, skin reactions, and headaches, though these occurred less frequently.
Serious Risks to Watch For
While uncommon, Zarxio carries two serious warnings worth knowing about. The first is splenic rupture. The spleen can enlarge during treatment, and in rare cases it can rupture. Pain in the upper left abdomen or left shoulder tip is the key warning sign. The second is acute respiratory distress syndrome (ARDS), a severe lung condition. Fever combined with difficulty breathing or worsening lung symptoms during treatment should be evaluated immediately.
How It’s Given and Stored
Zarxio is given as an injection, either under the skin or into a vein, depending on the clinical situation. For chemotherapy patients, injections typically begin the day after a chemotherapy cycle and continue daily until neutrophil counts recover to a safe level. Patients with severe chronic neutropenia may inject it daily on an ongoing basis. Many patients learn to self-inject at home using prefilled syringes.
If you’re storing Zarxio at home, keep it refrigerated between 36°F and 46°F in its original packaging to protect it from light. Take it out 30 minutes before injection to let it reach room temperature. It can stay at room temperature for up to 4 days, but anything beyond that should be discarded. If it freezes accidentally, thaw it in the refrigerator before use, but throw it away if it has been frozen more than once.
