Lyrica (pregabalin) is an anticonvulsant, also called an anti-epileptic or antiseizure drug. It’s classified by the DEA as a Schedule V controlled substance, the lowest category of controlled drugs, meaning it has a recognized medical use and a low potential for abuse compared to most other scheduled medications. While it belongs to the anticonvulsant family, Lyrica is prescribed far more often for nerve pain and fibromyalgia than for seizures.
How Lyrica Works in the Body
Pregabalin binds to a specific part of calcium channels on nerve cells called the alpha-2-delta subunit. Calcium channels are gateways that allow nerve cells to release chemical signals to neighboring cells. By attaching to these channels, Lyrica reduces the amount of calcium that flows in, which dials down the release of several signaling chemicals in the brain and spinal cord. The net effect is less excitatory nerve activity, which translates to reduced pain signaling, fewer seizures, or lower anxiety depending on where in the nervous system the drug is acting.
This mechanism is why a single drug can treat conditions that seem unrelated. Overactive nerve signaling is a common thread connecting neuropathic pain, certain seizure types, and fibromyalgia. Lyrica quiets that signaling across multiple circuits rather than targeting just one.
What Lyrica Is Approved to Treat
The FDA has approved Lyrica for four conditions:
- Diabetic nerve pain: the burning or tingling pain in the hands and feet caused by nerve damage from diabetes.
- Postherpetic neuralgia: lingering nerve pain that persists after a shingles outbreak has healed.
- Fibromyalgia: widespread musculoskeletal pain accompanied by fatigue and sleep problems.
- Partial onset seizures in adults: used alongside other seizure medications, not as a standalone treatment.
In Europe, pregabalin also carries an approved indication for generalized anxiety disorder, though the FDA has not granted that approval in the United States. Some prescribers in the U.S. do use it off-label for anxiety and other conditions.
Why It’s a Controlled Substance
In 2005, the DEA placed pregabalin into Schedule V of the Controlled Substances Act. The agency determined that pregabalin has a low potential for abuse relative to Schedule IV drugs (a category that includes benzodiazepines like lorazepam), and that misuse may lead to limited physical or psychological dependence. Schedule V is the same tier as certain cough medicines containing small amounts of codeine.
In practice, this means your pharmacy tracks Lyrica prescriptions more closely than a standard non-controlled medication, and refills may require a new prescription depending on your state’s rules. Some states have moved pregabalin to a stricter schedule based on local patterns of misuse.
How Lyrica Compares to Gabapentin
Lyrica is often compared to gabapentin (Neurontin) because both drugs target the same alpha-2-delta subunit on calcium channels. The key differences come down to how efficiently your body absorbs them and how much you need to take.
Pregabalin has a bioavailability of 90% or higher regardless of the dose, meaning your body absorbs nearly all of what you swallow. Gabapentin’s absorption drops significantly as doses go up, falling from about 60% at lower doses to just 33% at higher ones. Pregabalin is also roughly 2.5 times more potent than gabapentin based on blood concentrations, so effective doses are lower. Lyrica’s absorption is more predictable, which makes it easier to find and maintain the right dose.
Gabapentin is not federally scheduled as a controlled substance, though several states have added their own scheduling requirements in recent years.
Common Side Effects
Dizziness and drowsiness are by far the most frequent side effects. In clinical trials, 31% of people taking Lyrica reported dizziness compared to 9% on placebo, and 22% experienced drowsiness compared to 7% on placebo. These two side effects were also the most common reasons people stopped taking the drug during studies, with about 4% of participants dropping out because of each one.
For a notable portion of people, these effects don’t fade quickly. Among those who experienced dizziness, it persisted through the end of the study in about 30% of cases. Drowsiness was even more persistent, lasting until the final dose in 42% of affected patients. Other commonly reported effects include weight gain, swelling in the hands or feet, dry mouth, and blurred vision.
Serious Risks to Be Aware Of
Lyrica can cause severe allergic reactions including angioedema, a rapid swelling of the face, tongue, throat, or extremities that requires emergency treatment. It also carries a warning about suicidal thoughts and behavioral changes, a risk shared by all anticonvulsant drugs. If you notice new or worsening depression, agitation, or unusual mood shifts after starting pregabalin, that warrants immediate attention.
Combining Lyrica with opioid pain medications increases the risk of life-threatening respiratory depression, where breathing slows dangerously. People with heart failure should also be cautious, as the drug can cause fluid retention and swelling that strains the heart.
Stopping Lyrica Safely
Pregabalin should not be stopped abruptly. Sudden discontinuation can trigger withdrawal symptoms including anxiety, headache, insomnia, and nausea. In people taking it for seizures, stopping cold turkey can cause seizures to return or worsen.
The standard recommendation is to taper over at least one week. If withdrawal symptoms are severe during that period, the taper can be extended to four weeks with a more gradual dose reduction. Pregabalin has an elimination half-life of about six hours and is cleared almost entirely by the kidneys as unchanged drug, so once you stop, it leaves your system relatively quickly. The tapering process is about giving your nervous system time to readjust, not about waiting for the drug to clear.
