There’s no single answer, because it depends on when someone became deaf and what language they grew up with. A person born deaf who learned sign language from an early age typically thinks in sign language, experiencing visual and spatial imagery of signs rather than an audible inner voice. Someone who became deaf later in life, after acquiring spoken language, generally continues to think in that spoken language, complete with an internal monologue that sounds much like it did before their hearing changed.
Thinking in Sign Language
For people who are deaf from birth or early childhood and grow up using sign language, their primary language of thought is sign language. This isn’t a rough translation of English (or any spoken language) into hand movements. Sign languages like American Sign Language are complete, independent languages with their own grammar and syntax. When a native signer thinks, they often experience something like watching or feeling themselves sign internally, similar to how a hearing person “hears” their own voice in their head.
Research on deaf signers confirms this goes beyond just quiet reflection. A 2013 study of 28 deaf signers found that they regularly engage in what researchers called “signed soliloquy,” the visible equivalent of talking to yourself out loud. Just as a hearing person might mutter while working through a problem, deaf signers move their hands through signs. The study also found that deaf signers reported using both this visible self-talk and silent inner signing more frequently than the hearing comparison group used their own forms of self-talk. Participants described it as an integral part of everyday functioning, not something unusual or occasional.
The Brain Processes Sign and Speech Similarly
Brain imaging studies show that deaf signers and hearing speakers use many of the same neural pathways for language. Broca’s area, the brain region most associated with language production and comprehension in hearing people, activates in deaf signers both when they produce signs and when they watch someone else sign. This region works as part of a larger network that also includes areas involved in motor planning and spatial processing.
What this means is that the brain doesn’t treat sign language as a lesser substitute for speech. It processes sign language through the same core language infrastructure. The internal experience of thinking in sign is, neurologically speaking, a full language process, not just a visual shortcut.
How Deaf People Experience Reading
One common assumption is that when deaf people read written text, they must “sound out” words internally the way hearing readers do. Research suggests otherwise. A study comparing deaf skilled readers and hearing readers found that deaf readers did not rely on phonological mediation (the mental process of converting written letters into sounds) when reading. Instead, they processed written words through visual and orthographic patterns, recognizing the shapes and letter arrangements of words directly. Their reading comprehension matched that of hearing readers despite taking a completely different mental route to get there.
This finding reinforces the idea that thought and reading don’t require an auditory component. Deaf readers access meaning through visual recognition rather than an internal voice sounding out syllables.
When Hearing Loss Comes Later
People who lose their hearing after acquiring spoken language, typically after age six or so, tend to retain their spoken inner monologue. The brain’s language foundations are largely established by that age, and the internal voice persists even without continued auditory input. A person who spoke English for 30 years before becoming deaf will still think in English, still “hear” their own internal voice when planning what to say or working through a decision.
Many people with later-onset deafness who go on to learn sign language describe a blended experience. Their thinking might shift between their original spoken language and sign language depending on the situation, who they’re communicating with, or the type of problem they’re working through. This bilingual inner life is similar to what hearing bilingual people report, where the language of thought shifts with context.
Thinking Beyond Words
Not all thought is linguistic, for anyone. Deaf and hearing people alike think in images, spatial relationships, emotions, and abstract patterns that don’t map neatly onto any language. But there’s evidence that growing up deaf and using a visual language shapes certain cognitive habits. Research has found that fluent signers tend to show heightened visual peripheral perception, stronger spatial processing skills, and a faster ability to shift visual attention.
A study comparing deaf students (both with and without cochlear implants) and hearing students found some interesting differences in cognitive style. Cochlear implant users leaned more toward visual thinking than verbal thinking, while deaf non-users and hearing participants were more balanced between the two. Deaf students were also more likely to retain pictorial mental representations of diagrams, holding onto the visual properties rather than converting them into verbal descriptions.
These patterns suggest that deaf individuals don’t just substitute sign language where hearing people use speech. Their entire relationship with visual information can be more central to how they process the world, plan actions, and solve problems. Language is a major component of thought, but it’s layered on top of a rich foundation of non-linguistic cognition that varies from person to person regardless of hearing status.
The Role of Language Access
The picture changes significantly for deaf individuals who grow up without consistent access to any language, whether signed or spoken. This can happen when deaf children are born to hearing families who don’t learn sign language and the child doesn’t receive effective intervention early on. Without a fully developed first language, the structure of internal thought looks different. These individuals, sometimes called “homesigners,” develop their own gestural systems and can reason abstractly, but they may struggle with certain language-dependent cognitive tasks like mental time travel or complex conditional reasoning.
This is why early language access matters so much in the deaf community. The question isn’t really whether a deaf person thinks in sign or speech. It’s whether they had full access to any language during the critical developmental window. A deaf child who learns sign language from birth develops the same richness of inner thought as any hearing child learning a spoken language. The modality, hands versus voice, is just the delivery system. The cognitive architecture underneath is the same.
