What LDL Cholesterol Means on Your Blood Test

LDL cholesterol on a blood test measures the amount of cholesterol carried by low-density lipoproteins, the particles most responsible for clogging arteries. For most adults, a healthy LDL level is below 100 mg/dL, though your ideal target depends on your overall heart disease risk. This number appears on a standard lipid panel, the routine blood test your doctor orders to check your cholesterol.

What LDL Actually Is

Your body needs cholesterol to build cells and make hormones, but cholesterol can’t dissolve in blood on its own. It travels inside tiny round particles made of fat and protein called lipoproteins. LDL, or low-density lipoprotein, is one type of these carriers. Its job is to shuttle cholesterol from your liver out to cells throughout your body.

The problem is that LDL tends to deposit excess cholesterol inside artery walls. Once LDL particles slip into the lining of an artery, they get trapped and undergo chemical changes, including oxidation. This triggers an immune response: white blood cells rush in, swallow the modified cholesterol, and gradually form fatty streaks that harden into plaque. Over years, this plaque narrows arteries and can eventually rupture, causing a heart attack or stroke. That chain of events is why LDL is labeled “bad” cholesterol.

How LDL Is Measured

LDL cholesterol appears as part of a lipid panel, which also reports your total cholesterol, HDL (“good”) cholesterol, and triglycerides. In most cases, the lab doesn’t measure your LDL directly. Instead, it calculates LDL using a formula that subtracts your HDL and a fraction of your triglycerides from your total cholesterol. This calculated method works well for most people, but it becomes less accurate when triglycerides are very high.

When triglycerides are elevated, or in people with diabetes or known heart disease, guidelines recommend a direct LDL measurement. Direct assays can process large numbers of samples quickly and produce results close to the gold-standard laboratory method, which involves spinning blood in a centrifuge for 18 hours and is too slow for routine use.

Do You Need to Fast?

Traditionally, doctors required a 9 to 12 hour fast before a lipid panel, but that practice is changing worldwide. Eating a normal meal shifts LDL readings by only about 8 mg/dL on average, a difference that rarely changes clinical decisions. Guidelines in the United States, Europe, Canada, the United Kingdom, and several other countries now endorse nonfasting lipid panels as equally useful for assessing heart disease risk.

If your results come back borderline, your doctor may ask you to repeat the test while fasting to confirm the number. But for a routine screening, skipping breakfast is no longer considered necessary in most situations.

What Your LDL Number Means

For adults 20 and older, a healthy LDL level is below 100 mg/dL. For children and teenagers (19 and younger), the threshold is slightly higher at below 110 mg/dL. But “healthy” is relative to your personal risk profile. The latest guidelines from the American College of Cardiology and American Heart Association break it down further:

  • Borderline or intermediate risk: LDL goal below 100 mg/dL
  • High risk: LDL goal below 70 mg/dL
  • Very high risk (people who already have heart disease): LDL goal below 55 mg/dL

Your risk category depends on factors like age, blood pressure, smoking history, diabetes, and whether you’ve already had a heart attack or stroke. Someone with no risk factors and an LDL of 95 mg/dL is in good shape. The same number in someone who’s already had a cardiac event would be too high.

Not All LDL Particles Are Equal

A standard lipid panel tells you how much cholesterol your LDL particles are carrying, but it doesn’t tell you about the size of those particles. Research shows that small, dense LDL particles are more dangerous than large, buoyant ones. A large study from the Copenhagen General Population found that for every 39 mg/dL increase in small dense LDL cholesterol, the risk of heart attack rose by 85% and overall cardiovascular disease risk rose by 62%.

Small dense particles are more likely to penetrate artery walls and more susceptible to the oxidation that triggers plaque formation. Some advanced lipid tests can measure particle size and count, but these aren’t part of a standard panel. Your doctor might order one if your standard numbers look normal but your heart disease risk seems higher than expected.

What Drives LDL Higher

Several factors push LDL up. Diets high in saturated fat and trans fat increase LDL production. Carrying excess weight, particularly around the midsection, raises LDL while lowering HDL. Physical inactivity has a similar effect. Genetics play a major role too: some people inherit a tendency toward high LDL regardless of lifestyle, a condition called familial hypercholesterolemia that affects roughly 1 in 250 people.

Age and hormonal changes also matter. LDL tends to rise as you get older, and women often see a noticeable jump after menopause. Conditions like underactive thyroid, kidney disease, and type 2 diabetes can elevate LDL as well.

Lowering LDL

Lifestyle changes are the first line of defense. Replacing saturated fats with unsaturated fats (think olive oil instead of butter, nuts instead of cheese), increasing soluble fiber from foods like oats and beans, losing even a modest amount of weight, and exercising regularly can each lower LDL by 5 to 15%. Combined, these changes can be substantial.

When lifestyle changes aren’t enough, or when cardiovascular risk is high, medication becomes part of the plan. Statins are the most widely prescribed option and typically lower LDL by 30 to 50%. For people who need deeper reductions, additional medications can be added. The key takeaway from current guidelines is that lower is better when it comes to LDL, and the higher your risk, the more aggressively that number should be brought down.
