What Medications Become Toxic After Expiration?

Very few medications actually become toxic after their expiration date. The real risk with most expired drugs is that they lose potency and stop working when you need them most. That distinction matters: a heart medication that has quietly degraded could fail you during a cardiac emergency, which is a different kind of dangerous than a pill turning into poison.

The one well-documented case of a medication becoming chemically toxic after expiration involves an old formulation of tetracycline, an antibiotic. Degraded tetracycline was linked to a form of kidney damage in the 1960s. Modern formulations have largely eliminated this risk, but the case cemented the idea that expired drugs are poisonous. In reality, the bigger concern for nearly every medication in your cabinet is reduced effectiveness, and for certain drugs, that reduced effectiveness can be life-threatening.

Medications Where Lost Potency Is Dangerous

Some drugs don’t need to become toxic to hurt you. If they lose enough strength, the consequences of a failed dose are severe. These are the medications worth paying closest attention to.

Nitroglycerin is one of the most fragile. Used to treat chest pain during a cardiac event, sublingual nitroglycerin tablets degrade faster than most people expect. In a study simulating real-world conditions, a 100-count bottle carried in a pants pocket fell below acceptable potency within 12 months, while a smaller 25-count bottle held up for about two years. Heat, moisture, and repeated opening of the container all accelerate the breakdown. If you rely on nitroglycerin, replacing it on schedule is not optional.

Insulin is similarly sensitive. The FDA notes that insulin vials and cartridges (opened or unopened) can remain effective at room temperature between 59°F and 86°F for up to 28 days. Beyond that window, or after exposure to extreme heat or cold, insulin gradually loses its ability to control blood sugar. The decline isn’t dramatic enough that you’d notice a single bad dose, but over time it leads to dangerously uncontrolled glucose levels. Cloudy insulin that used to be clear, or insulin with visible clumps or particles, has degraded and should not be used.

Epinephrine autoinjectors (EpiPens) are interesting because the data is more reassuring than people assume. Research published through the American Academy of Allergy, Asthma & Immunology found that autoinjectors retained 100% of their epinephrine up to six months past expiration, 95% or better at one year past, and at least 90% out to 30 months beyond the labeled date. That said, during a severe allergic reaction, you want full-strength epinephrine. If your autoinjector is expired and it’s all you have, use it. But don’t make a habit of keeping expired ones as your primary supply.

Birth control pills won’t make you sick after expiration, but they may not prevent pregnancy as reliably. The hormones in oral contraceptives degrade over time, and because the margin for error with hormonal birth control is already narrow (missed doses of even a few hours can reduce effectiveness for some formulations), using expired pills adds unnecessary risk.

Liquid Antibiotics Degrade Fastest

Reconstituted liquid antibiotics, the kind pharmacists mix with water for children’s ear infections or strep throat, are in a category of their own. Once mixed, these suspensions have a very short shelf life. Research on amoxicillin-clavulanic acid suspensions found that at warm room temperatures (around 104°F), the drug remained stable for only about two days. Even under ideal conditions, most reconstituted antibiotics are meant to be used within 7 to 14 days and then discarded.

Taking a weakened antibiotic isn’t just ineffective. It can allow bacteria to survive at sub-lethal doses, which is exactly the condition that promotes antibiotic resistance. If your child’s leftover liquid antibiotic has been sitting in the fridge for weeks, it belongs in the trash.

Aspirin Tells You When It’s Gone Bad

Aspirin is one of the few medications that gives you a clear physical warning of degradation. Over time, the active ingredient (acetylsalicylic acid) breaks down into salicylic acid and acetic acid. Acetic acid is the compound that gives vinegar its smell. If you open an old bottle of aspirin and catch a vinegar-like odor, the tablets have significantly degraded. They’re unlikely to harm you, but they won’t deliver the expected dose, which matters if you’re taking aspirin for heart protection or pain relief during an emergency.

Most Medications Last Longer Than Labels Suggest

Expiration dates are conservative by design. Pharmaceutical companies test their products for a set period (usually one to three years), confirm stability through that window, and print the date. They have no financial incentive to test longer.

The most comprehensive data on this comes from the FDA’s Shelf Life Extension Program (SLEP), which tested thousands of medication lots stockpiled by the federal government. Of 3,005 lots tested, 88% were extended past their original expiration date, with an average extension of 66 months, more than five years. These were solid-form medications like tablets and capsules stored under controlled conditions. The takeaway: most pills sitting in a cool, dry medicine cabinet are likely fine well past the printed date.

That said, “most” is doing a lot of work in that sentence. The drugs discussed above (nitroglycerin, insulin, liquid antibiotics, hormonal contraceptives) are notable exceptions. So are any medications stored in humid bathrooms, hot cars, or direct sunlight, because heat and moisture accelerate chemical breakdown regardless of the expiration date.

How to Handle Expired Medications Safely

For most expired over-the-counter and prescription drugs, the FDA recommends using a drug take-back program. Many pharmacies and police stations host collection events or have permanent drop-off bins. You can also use prepaid mail-back envelopes if no local option exists.

The FDA maintains a specific “flush list” of medications that should be flushed down the toilet rather than thrown away if no take-back option is available. These are drugs with high potential for misuse that could cause death from a single accidental dose. The list is almost entirely opioids: medications containing fentanyl, hydrocodone, oxycodone, morphine, methadone, hydromorphone, and others. A small number of non-opioid drugs also appear on the list, including certain formulations of diazepam and methylphenidate.

Everything else should go in the household trash, ideally mixed with coffee grounds, dirt, or cat litter in a sealed bag to discourage accidental ingestion. The priority with disposal is preventing someone else, especially children or pets, from accessing medications that could harm them. An expired medication that has lost potency is still fully potent enough to poison a toddler.
