Dozens of commonly prescribed medications can cause hair loss in women, ranging from blood thinners and antidepressants to birth control pills and blood pressure drugs. The good news: most drug-related hair loss is temporary and reverses within six to eight months once the medication is stopped or changed. Understanding which drugs carry this risk, and how they affect your hair, can help you have a more productive conversation with your prescriber if you notice thinning.
How Medications Cause Hair Loss
Your hair follicles cycle through phases of active growth and rest. Medications interfere with this cycle in two distinct ways, and knowing the difference helps explain why some drugs cause sudden dramatic shedding while others produce a slow, gradual thinning.
The more common type is called telogen effluvium. A medication pushes a large number of hair follicles into their resting phase all at once. You won’t notice it right away because the resting hairs don’t actually fall out until new hairs start growing underneath and push them out. That’s why shedding typically begins two to four months after starting the medication, which can make it hard to connect the hair loss to the drug that caused it.
The more severe type, anagen effluvium, happens when a drug directly damages the hair follicle during its active growth phase. This disrupts cell division in the hair root, creating a weakened point in the shaft that breaks easily or causing the hair to fall out at the root entirely. Hair loss from anagen effluvium starts within two to three weeks of treatment. Chemotherapy drugs are the most common cause.
Chemotherapy Drugs
Cancer treatments are the most well-known cause of drug-related hair loss, and they tend to cause the most extensive shedding. Because these drugs target rapidly dividing cells throughout the body, hair follicles (which are among the fastest-dividing cells you have) are especially vulnerable. The result is often near-complete hair loss on the scalp, eyebrows, and body.
Most chemotherapy-related hair loss is temporary, with regrowth beginning after treatment ends. However, certain drugs can cause permanent thinning. Taxane-based chemotherapy agents, commonly used in breast cancer treatment, have been linked to cases of permanent hair loss. High-dose regimens involving busulfan (used before bone marrow transplants) have also been implicated. For the majority of women undergoing standard chemotherapy, though, hair does grow back, often with a slightly different texture or color initially.
Antidepressants
SSRIs list hair loss as an infrequent side effect, occurring in roughly 1 in 100 to 1 in 1,000 patients. Among the SSRIs, fluoxetine (Prozac) accounts for the largest share of reported cases at 38%, followed by sertraline (Zoloft) at 28% and citalopram (Celexa) at about 16%. Paroxetine (Paxil) appears to carry the lowest risk. SNRIs have a comparable overall risk.
If you notice increased shedding a few months after starting an antidepressant, it’s worth discussing with your prescriber. Switching to a different medication in the same class may help, since risk varies between individual drugs. Stopping an antidepressant abruptly on your own is not safe and can cause withdrawal symptoms.
Hormonal Birth Control
Birth control can either help or hurt your hair depending on the type of progestin it contains. Progestins with higher androgenic (male-hormone-like) activity can shorten the hair growth phase and damage follicles, leading to thinning. Pills with higher androgen activity include brands like Loestrin, Lo/Ovral, and Nordette. Non-pill options like the hormonal implant, the injection (Depo-Provera), the patch, and the ring (NuvaRing) can also raise androgen levels.
On the other hand, pills containing drospirenone or those with a low androgen index (like Desogen and Ortho-Cyclen) may actually improve hair growth by counteracting androgen effects. If you’re genetically prone to hormone-sensitive hair thinning, the choice of contraceptive matters. Hair loss can also occur when stopping birth control, as hormone levels shift and follicles readjust.
Blood Pressure and Heart Medications
Beta-blockers, used for high blood pressure, anxiety, and certain heart conditions, can trigger telogen effluvium. Propranolol, atenolol, and metoprolol are the most frequently reported. The effect is uncommon but can be distressing when it occurs. The mechanism may involve reduced blood flow to hair follicles or disruption of normal growth cycle signaling.
Blood thinners are another cardiac drug class linked to hair loss. Both warfarin and heparin can cause shedding, typically a few months into treatment.
Acne Medication (Isotretinoin)
Isotretinoin (formerly sold as Accutane) causes hair thinning in about 3% to 6% of users, and the effect is dose-dependent. At lower doses (under 0.5 mg/kg/day), the frequency is around 3.2%. At higher doses, it rises to about 5.7%. The hair loss is usually temporary and resolves after the course of treatment ends, but it can be alarming for women already self-conscious about a skin condition.
Seizure and Mood-Stabilizing Drugs
Valproic acid, prescribed for epilepsy and bipolar disorder, is one of the more commonly reported causes of drug-induced hair loss. Carbamazepine, another seizure medication, carries a similar risk. Lithium, widely used for bipolar disorder, is also a known trigger. For women taking these medications long-term, thinning may develop gradually and can sometimes be managed with dose adjustments or supplementation (particularly zinc or selenium, which valproic acid can deplete).
Thyroid Medications
This one is tricky because both the disease and the treatment can cause hair loss, making it hard to tell what’s responsible. Untreated hypothyroidism causes hair loss all over the body, not just the scalp. Hyperthyroidism causes fine, thinning hair across the entire head. Levothyroxine, the standard treatment for an underactive thyroid, can itself cause hair shedding, particularly during the first month or two of use. This is especially common in children but happens in adult women too.
The shedding is usually temporary. Once your thyroid levels stabilize on the medication, hair growth typically returns to normal. If hair loss persists well beyond the first few months of treatment, it may signal that your dose needs adjusting rather than that the medication itself is the problem.
Other Common Culprits
- Arthritis medications like indomethacin and methotrexate
- Gout medications such as allopurinol and colchicine
- High-dose vitamin A supplements or retinoid-based medications
- Amphetamines prescribed for ADHD, including dextroamphetamine
How Drug-Related Hair Loss Is Identified
There’s no single blood test that confirms a medication is causing your hair loss. Doctors rely primarily on timing. If hair loss begins within days to months of starting a new medication, changing a dose, or stopping a drug, the medication is a likely suspect. A dermatologist can examine the pattern and type of shedding to help distinguish drug-related loss from other causes like genetic thinning or autoimmune conditions.
The tapered, broken shafts of anagen effluvium look different under magnification than the club-shaped roots seen in telogen effluvium, so the type of shedding can point toward the category of drug involved.
What Recovery Looks Like
For telogen effluvium, which covers the vast majority of medication-related hair loss, the shedding phase lasts three to six months. After the trigger is removed (by stopping or switching the medication), most women see full recovery within six to eight months without any additional treatment. New growth appears first as short, fine hairs in the thinning areas.
Anagen effluvium from chemotherapy typically reverses faster once treatment ends, since the follicles weren’t in a prolonged resting phase. Regrowth often begins within weeks. The exception is permanent chemotherapy-induced alopecia, which affects a small subset of patients and may require different management strategies like topical treatments or hair restoration options.
If you suspect a medication is causing hair loss, the most useful step is keeping a timeline: when you started the drug, when you first noticed shedding, and how much hair you’re losing. That information helps your doctor assess whether the medication is likely responsible and weigh the benefits of the drug against this side effect.
