What Not to Take With Claritin-D: Drugs to Avoid

Claritin-D contains two active ingredients: loratadine (an antihistamine) and pseudoephedrine (a decongestant). While loratadine is relatively mild, pseudoephedrine is a stimulant that narrows blood vessels and opens nasal passages. That stimulant component is responsible for most of the serious interactions. Here’s what you need to avoid or be careful with.

MAO Inhibitors: The Most Dangerous Combination

If you take or recently took a monoamine oxidase inhibitor (MAOI) for depression or other conditions, Claritin-D is off the table. You need a full 14-day gap after stopping an MAOI before taking Claritin-D. Pseudoephedrine combined with MAO inhibition can trigger a hypertensive crisis, a sudden and dangerous spike in blood pressure caused by overstimulation of the receptors that control blood vessel constriction. This is a hard contraindication, not a “use with caution” situation.

ADHD Medications and Other Stimulants

Prescription stimulants like those used for ADHD increase heart rate and blood pressure on their own. Pseudoephedrine does the same thing. Stacking the two amplifies both effects, and over time this combination can damage the heart. The same logic applies to any stimulant medication used for weight loss or wakefulness. If you’re on a prescription stimulant, talk to your prescriber before reaching for Claritin-D.

Blood Pressure and Heart Medications

Pseudoephedrine raises blood pressure by constricting blood vessels. If you take medication to lower your blood pressure, Claritin-D can work against it, making your treatment less effective. People with high blood pressure, heart disease, or diabetes should use Claritin-D with caution or avoid it entirely. Regular Claritin (loratadine alone, without the “D”) is typically a safer choice in these cases because it doesn’t contain pseudoephedrine.

Other Antihistamines and Sleep Aids

Taking Claritin-D alongside another antihistamine, especially older ones like diphenhydramine (Benadryl), chlorpheniramine, or hydroxyzine, increases the risk of sedation. These first-generation antihistamines cross into the brain and cause drowsiness on their own. Adding loratadine on top can compound that effect. The same goes for sleep aids that contain diphenhydramine, which is the active ingredient in many over-the-counter sleep products like ZzzQuil and Tylenol PM.

CNS depressants more broadly, including opioid pain medications and benzodiazepines (commonly prescribed for anxiety), can also increase sedation when combined with loratadine.

Other Cold and Allergy Products

Many cold, flu, and sinus products already contain pseudoephedrine or another decongestant. If you take Claritin-D and then add a multi-symptom cold medicine, you could end up doubling your decongestant dose without realizing it. Check the active ingredients on any over-the-counter product before combining it with Claritin-D. Look for pseudoephedrine and phenylephrine in particular.

Caffeine

Caffeine and pseudoephedrine both raise blood pressure and heart rate. Combining them enhances these effects. You don’t necessarily need to quit coffee entirely, but heavy caffeine intake (multiple cups, energy drinks, pre-workout supplements) while taking Claritin-D can leave you jittery, anxious, or with a noticeably elevated heart rate. If you’re sensitive to stimulants, scale back while you’re on this medication.

Alcohol

Alcohol interacts with both ingredients in Claritin-D, though in different ways. It can make the antihistamine component less effective at relieving allergy symptoms. It also increases the likelihood of drowsiness, dizziness, and feeling faint, side effects that loratadine rarely causes on its own. Your liver processes both alcohol and loratadine, so combining them slows the breakdown of each. You may feel more intoxicated than expected from the same amount of alcohol you’d normally drink.

Claritin-D and Breastfeeding

Pseudoephedrine passes into breast milk in small amounts and can reduce milk production. In one study, a single 60 mg dose caused a 24% drop in milk output over the following day. About 20% of exposed infants showed irritability. If your milk supply isn’t well established yet or you’re already struggling with production, pseudoephedrine can make the problem worse. Regular Claritin without the decongestant is a better option during breastfeeding.

Health Conditions That Don’t Mix Well

Beyond specific medications, certain conditions make Claritin-D riskier. The pseudoephedrine component can worsen:

  • High blood pressure, even if currently managed with medication
  • Heart disease, due to increased heart rate and blood pressure
  • Thyroid problems, particularly overactive thyroid
  • Diabetes, as pseudoephedrine can affect blood sugar
  • Enlarged prostate, which can make urination more difficult
  • Kidney or liver disease, which affects how your body processes the drug
  • Glaucoma, particularly narrow-angle glaucoma

If any of these apply to you, plain loratadine (Claritin without the “D”) gives you the antihistamine benefit without the stimulant risks. It’s the pseudoephedrine that causes the vast majority of interactions and complications, so removing it from the equation solves most of these concerns.

12-Hour vs. 24-Hour: Does the Formulation Matter?

Claritin-D comes in two versions. The 12-hour tablet contains 120 mg of pseudoephedrine and is taken twice daily. The 24-hour tablet contains 240 mg in an extended-release core and is taken once daily. Both carry the same interaction risks, but the 24-hour version delivers twice the total pseudoephedrine per tablet. The 12-hour formulation caused insomnia in 15% of users in clinical trials, compared to just 4% with the 24-hour version, likely because of the twice-daily dosing pattern. Both versions caused dry mouth in about 13% of users. Regardless of which formulation you use, the same interaction warnings apply.
