Ondansetron (brand name Zofran) interacts with several common medications, and one combination is dangerous enough to be completely off-limits. The most critical drug to avoid is apomorphine, a medication used for Parkinson’s disease, which can cause a severe drop in blood pressure and loss of consciousness when taken with ondansetron. Beyond that absolute contraindication, a number of widely prescribed medications can raise the risk of heart rhythm problems or a potentially life-threatening condition called serotonin syndrome.
Apomorphine: The One True Contraindication
The FDA lists apomorphine as the only drug that is flatly contraindicated with ondansetron. Apomorphine is an injectable medication sometimes used to treat “off” episodes in Parkinson’s disease. When combined with ondansetron, it can cause profound hypotension (a dangerous plunge in blood pressure) and loss of consciousness. This isn’t a theoretical risk or a minor caution. It’s a hard rule: if you take apomorphine, ondansetron should not be used.
Antidepressants and Serotonin Syndrome
Ondansetron works by blocking serotonin receptors in the body. That’s how it controls nausea. But when you combine it with other medications that also affect serotonin levels, the neurotransmitter can build up to dangerously high concentrations, triggering a condition called serotonin syndrome.
The FDA specifically flags several classes of serotonergic drugs that raise this risk:
- SSRIs (such as fluoxetine, sertraline, escitalopram)
- SNRIs (such as venlafaxine, duloxetine)
- MAOIs (such as phenelzine, tranylcypromine)
- Mirtazapine
- Lithium
Serotonin syndrome can range from mild to fatal. Early signs include confusion, agitation, restlessness, rapid heart rate, sweating, shivering, and tremor. In severe cases it can progress to high fever, loss of consciousness, coma, and death. If you take any antidepressant and are prescribed ondansetron, that doesn’t automatically mean you can’t use it, but your prescriber needs to know about the combination so they can weigh the risk and watch for symptoms.
Pain Medications That Affect Serotonin
It’s not just antidepressants. Two commonly prescribed pain medications also increase serotonin activity and carry the same serotonin syndrome warning when paired with ondansetron:
- Tramadol, an opioid-like pain reliever
- Fentanyl, a strong opioid used for severe pain and in surgical settings
This matters because ondansetron is frequently given alongside opioids, for example after surgery or during cancer treatment, precisely when these pain medications are also in use. The combination isn’t necessarily avoided in every case, but it requires awareness and monitoring.
Drugs That Affect Heart Rhythm
Ondansetron can lengthen a specific electrical interval in the heartbeat called the QT interval. When this interval stretches too far, it raises the risk of a dangerous irregular heart rhythm. Combining ondansetron with other medications that have the same effect on the heart compounds that risk.
The most common QT-prolonging drugs co-prescribed with ondansetron, based on data from a large hospital system, include SSRIs (which carry a double risk since they affect both serotonin and QT interval), macrolide antibiotics like azithromycin and erythromycin, fluoroquinolone antibiotics like levofloxacin and moxifloxacin, and certain antidepressants in the SARI class like trazodone. Antiarrhythmic heart medications are another concern, though they’re less commonly encountered.
Low potassium or low magnesium levels make QT prolongation more dangerous. If you take a diuretic (water pill) that depletes these minerals, or if you’ve been vomiting heavily (which itself depletes electrolytes), the cardiac risk from ondansetron goes up. The FDA has emphasized that QT prolongation with ondansetron is dose-dependent, meaning higher doses carry greater risk. For intravenous ondansetron, no single dose should exceed 16 mg. A previously used 32 mg IV dose was pulled because of this cardiac concern. Standard oral doses have not been changed.
Medications That Make Ondansetron Less Effective
Some drugs don’t cause a dangerous interaction but instead reduce how well ondansetron works. Your liver breaks down ondansetron using a specific enzyme pathway, and certain medications speed up that process dramatically, clearing ondansetron from your bloodstream before it can do its job.
Three well-documented culprits are rifampin (an antibiotic used for tuberculosis), carbamazepine (a seizure medication), and phenytoin (another seizure medication). In one study, rifampin cut ondansetron’s blood levels by nearly half. Patients on carbamazepine or phenytoin showed similarly significant drops. If you take any of these, your prescriber may need to adjust the approach to managing your nausea since ondansetron simply won’t work as well.
Alcohol
No formal drug interaction has been identified between alcohol and ondansetron, and databases list no direct pharmacological conflict. That said, both ondansetron and alcohol can independently cause headaches, dizziness, and drowsiness. Drinking while taking ondansetron is unlikely to cause a dangerous reaction, but it can worsen the side effects you’re already managing, especially if you’re taking the medication because you’re already feeling unwell.
Methylene Blue
This one catches people off guard. Methylene blue, a dye sometimes given intravenously during certain surgeries or to treat specific poisonings, is a potent serotonin booster. The FDA includes it on the list of serotonergic drugs that can trigger serotonin syndrome when combined with ondansetron. Since methylene blue is typically given in hospital settings, this interaction is more relevant for surgical teams than for patients managing medications at home, but it’s worth knowing if you’re heading into a procedure.
How to Think About These Interactions
Not every interaction on this list means you absolutely cannot take ondansetron. The apomorphine combination is the only one that’s a hard no. For serotonin-related interactions, the risk depends on the specific drugs, the doses, and how long you’re taking them. A single dose of ondansetron for post-surgical nausea carries a different risk profile than taking it daily alongside a high-dose SSRI.
The practical takeaway: before taking ondansetron, make sure whoever prescribes it knows about every other medication you’re on, including antidepressants, pain medications, antibiotics, seizure drugs, and heart medications. Many of these combinations can be managed safely with proper awareness, but some require a different anti-nausea option entirely.
