Herpes is one of the most common infections in the world. Globally, more than 1 in 5 adults between the ages of 15 and 49 have a genital herpes infection, and when you include oral herpes, the majority of the world’s population carries some form of the virus. In the United States alone, about 48% of people aged 14 to 49 have oral herpes (HSV-1), and roughly 12% have genital herpes (HSV-2).
Two Types of Herpes, Two Sets of Numbers
Herpes simplex virus comes in two forms. HSV-1 traditionally causes oral herpes (cold sores), while HSV-2 is the primary cause of genital herpes. But HSV-1 increasingly causes genital infections too, which is why global health organizations now track genital herpes from both types separately.
The World Health Organization estimated that in 2020, around 846 million people aged 15 to 49 were living with genital herpes. Of those, 520 million had genital HSV-2 and 376 million had genital HSV-1. About 50 million people had both types simultaneously, since carrying one doesn’t prevent you from catching the other.
These numbers only count genital infections. When you add in oral HSV-1, which the vast majority of people pick up during childhood through non-sexual contact, the total number of people carrying herpes worldwide climbs far higher. Estimates consistently place oral HSV-1 prevalence above 60% of the global population under 50.
Prevalence in the United States by Age
The most detailed U.S. data comes from the CDC’s National Health and Nutrition Examination Survey, which tests participants’ blood for herpes antibodies regardless of whether they’ve ever had symptoms. The 2015-2016 survey found that prevalence rises steadily with age for both types.
For HSV-1, infection rates climb from 27% among teenagers (ages 14 to 19) to 41.3% in the 20s, 54.1% in the 30s, and 59.7% by the 40s. For HSV-2, the pattern is similar but at lower numbers: 0.8% of teenagers, 7.6% of people in their 20s, 13.3% in their 30s, and 21.2% by their 40s. That means roughly 1 in 5 Americans in their 40s carries genital HSV-2.
This steady climb reflects the cumulative nature of herpes. Once you’re infected, the virus stays in your body for life. Each additional year of sexual activity adds more exposure opportunity, so older age groups always show higher rates.
Why Women Are Affected More Often
HSV-2 infects women nearly twice as often as men. The reason is biological: sexual transmission is more efficient from men to women than the reverse. Research on couples where one partner has genital herpes and the other doesn’t has shown this disparity clearly. In one study tracking these “discordant” couples, 16.9% of susceptible women with an infected male partner acquired the virus, compared to just 3.8% of susceptible men with an infected female partner.
Prior immunity to HSV-1 offers some partial protection. Women who already had HSV-1 antibodies acquired genital HSV-2 at a rate of about 9% per year, while women with no prior herpes antibodies of either type had an annual acquisition rate of nearly 32%. Having encountered one type of the virus appears to give the immune system a head start against the other, though it clearly doesn’t prevent infection entirely.
Most People Don’t Know They Have It
One of the most striking things about herpes prevalence is how many carriers are completely unaware of their status. The majority of people with HSV-2 have never been diagnosed. Many experience no noticeable symptoms, or their symptoms are so mild they get mistaken for something else: a small bump, minor irritation, or a brief rash that resolves on its own.
This is a major reason the virus spreads so effectively. People who don’t know they’re infected aren’t taking precautions and aren’t on antiviral medication, yet they can still shed the virus from their skin on days when they have no visible sores. Standard STI panels typically don’t include herpes blood testing unless you specifically request it, which means many infections go undetected indefinitely.
How These Numbers Compare Over Time
Interestingly, HSV-1 rates in younger Americans have been declining for decades. Better hygiene and less crowded living conditions mean fewer children are exposed to the virus early in life. That sounds like good news, but it carries an unexpected consequence. Young adults who never caught HSV-1 as children have no pre-existing antibodies, making them more vulnerable to acquiring HSV-1 as a genital infection through oral sex later on. This shift is one reason genital HSV-1 has become increasingly common.
HSV-2 prevalence in the U.S. has also declined modestly since its peak in the late 1990s, when roughly 21% of people aged 14 to 49 tested positive. The current figure of around 12% represents real progress, likely driven by increased condom use and greater awareness. Still, the absolute number of people living with genital herpes remains enormous, and the virus continues to spread in every demographic group.
Transmission Risk Between Partners
If you’re in a relationship where one partner has genital herpes and the other doesn’t, the annual risk of transmission depends on several factors. Without any precautions, studies of heterosexual couples have found overall transmission rates of about 10% per year. Daily antiviral medication cuts that risk roughly in half, and combining antivirals with consistent condom use reduces it further, to an estimated 1 to 2% per year for male-to-female transmission.
These numbers mean that with proper precautions, many discordant couples go years or even decades without the virus passing to the uninfected partner. The risk is never zero, but it’s considerably lower than most people assume when they first learn about a partner’s diagnosis.
Herpes in Newborns
Neonatal herpes is rare but serious. The estimated incidence is roughly 1 in every 15,000 deliveries, with some studies reporting rates as high as 1 in 3,500. The risk is highest when a mother acquires a new genital herpes infection late in pregnancy, because her body hasn’t yet produced the antibodies that would partially protect the baby during delivery. Women with long-standing herpes infections who are managed appropriately during pregnancy and delivery face a much lower risk of transmitting the virus to their newborns.
