What Should Progesterone Be at 4 Weeks Pregnant?

At 4 weeks pregnant, progesterone typically falls somewhere between 10 and 44 ng/mL, which is the accepted range for the entire first trimester. Most women at exactly 4 weeks will be on the lower end of that range, closer to 10 to 20 ng/mL, because progesterone rises steadily as pregnancy progresses. A single number matters less than the overall trend, but certain thresholds can signal whether a pregnancy is on track.

Why Progesterone Matters at 4 Weeks

At this stage, your body is entirely dependent on a small structure in the ovary called the corpus luteum to produce progesterone. This is the leftover shell of the follicle that released your egg, and once conception occurs, it gets a signal from HCG (the same hormone a pregnancy test detects) to keep producing progesterone. Its job is to thicken the uterine lining and create a stable environment for the embryo to implant and grow.

The corpus luteum handles all of your progesterone production for roughly the first 12 weeks of pregnancy. After that, the placenta takes over. This means early pregnancy is a vulnerable window. If the corpus luteum underperforms, progesterone can drop, and the uterine lining may not support the pregnancy well.

What the Numbers Actually Tell You

Progesterone levels vary widely between individuals, even in perfectly healthy pregnancies. A reading of 12 ng/mL and a reading of 30 ng/mL at 4 weeks can both represent viable pregnancies. What clinicians look for are patterns at the extremes.

A progesterone level below 6 ng/mL excludes a viable pregnancy in about 99% of cases. Below 10 ng/mL, the likelihood of a non-viable pregnancy rises significantly, with one large analysis finding that threshold predicted a failing pregnancy with roughly 97% specificity in women who were experiencing symptoms like bleeding or pain. On the other end, levels above 20 ng/mL are strongly associated with a normal intrauterine pregnancy. Only about 2.6% of ectopic pregnancies show progesterone that high.

The tricky zone is between 6 and 20 ng/mL. A value in this range doesn’t confirm or rule out a problem on its own. Your doctor will typically pair it with HCG levels (checking whether they’re doubling appropriately) and an early ultrasound to get the full picture.

Low Progesterone and Miscarriage Risk

Low progesterone has been linked to higher miscarriage rates, but the relationship is complicated. It’s often unclear whether low progesterone is the cause of a failing pregnancy or simply a reflection of one. An embryo with chromosomal problems, for instance, may send weaker HCG signals to the corpus luteum, which in turn produces less progesterone. In that scenario, the low number is a symptom, not the root problem.

That said, some women do have genuinely insufficient progesterone production despite an otherwise healthy embryo. This is more common in women with a history of recurrent miscarriage and in certain IVF protocols where no corpus luteum forms at all.

Progesterone Supplementation

If your levels are low and you have risk factors, your provider may prescribe progesterone support. The most studied approach uses vaginal micronized progesterone, taken twice daily through about 16 weeks of pregnancy. A major clinical trial (the PRISM trial) found that women with early pregnancy bleeding and a history of miscarriage saw a meaningful benefit from this supplementation.

Progesterone supplements are not routinely prescribed for every pregnancy with a lower-than-expected number. They’re most commonly used for women with recurrent losses, those who are bleeding in early pregnancy, and nearly all IVF patients on certain protocols. If you’re concerned about your levels, the conversation with your provider should focus on your specific history and symptoms, not just the number in isolation.

IVF Pregnancies Are Different

If you conceived through IVF, particularly a frozen embryo transfer, your progesterone levels work differently. In a programmed transfer cycle, ovulation is suppressed and no corpus luteum forms. That means your body produces little to no progesterone on its own, and you rely entirely on prescribed progesterone (vaginal, injectable, or both) to maintain the pregnancy.

Because of this, your blood levels reflect the medication dose and absorption rather than natural production. Research on frozen embryo transfers suggests that a progesterone level above about 15 ng/mL around the time of transfer is associated with better pregnancy outcomes. Your fertility clinic will monitor your levels and adjust your protocol accordingly. Do not compare your IVF progesterone values directly with ranges meant for natural conception.

Twin Pregnancies and Higher Levels

If you’re carrying twins, your progesterone will likely be noticeably higher than singleton ranges. Studies show progesterone levels in twin pregnancies run approximately 60% higher throughout gestation. So a reading of 25 to 35 ng/mL at 4 weeks in a twin pregnancy is well within expectations. If you had IVF and transferred multiple embryos, a higher-than-expected progesterone reading can sometimes be an early hint of multiples before ultrasound confirmation.

When a Low Number Suggests Ectopic Pregnancy

Very low progesterone in combination with other warning signs can point toward an ectopic pregnancy, where the embryo implants outside the uterus. Among women with confirmed ectopic pregnancies, fewer than 3% had progesterone above 20 ng/mL. Conversely, only 0.3% of women with progesterone below 5 ng/mL turned out to have a viable intrauterine pregnancy.

Progesterone alone cannot diagnose an ectopic. It’s one data point alongside HCG trends and ultrasound findings. But if your level is very low (under 5 ng/mL) and your HCG isn’t rising normally, your provider will investigate ectopic pregnancy as a possibility, typically with a transvaginal ultrasound.

What a Single Test Can and Cannot Tell You

One progesterone reading at 4 weeks is a snapshot, not a verdict. Levels fluctuate throughout the day and can vary depending on when you ovulated, how strong your corpus luteum is, and even the lab’s specific assay. A value in the normal range is reassuring. A value that’s borderline or low warrants follow-up testing, not panic. The combination of progesterone, serial HCG measurements, and eventual ultrasound gives a far more reliable picture of pregnancy health than any single lab draw.
